PA Podiatrist Discusses Plantar Fasciitis
Plantar fasciitis is a painful inflammation on the bottom of the foot. It's often accompanied by heel spurs which are calcium deposits that cause bony protrusions on the underside of the heel bone. "99 percent of the time conservative treatment works," said Commonwealth Health podiatrist Laura Virtue-Delayo, DPM. The treatment starts with cold compresses. Dr. Virtue-Delayo said, "I like when patients freeze a water bottle and roll their heel on it because that will massage it and ice it." Simple stretches several times a day can also spell relief.

Dr. Virtue-Delayo also recommends wearing running shoes even if you don't run. They're designed to take the wear and tear off your feet. Look for signs of excessive wear on the heels or soles and consider getting orthotic shoe inserts which may help give you the heel pain relief you need.

Tourniquet Time Linked with Repaired Achilles Tendon Infection
Achilles tendon ruptures treated surgically had a low risk of infection and re-rupture, but these risks were greater in the presence of certain patient and surgical factors, according to study results. “Achilles tendon repairs are associated with a pretty low risk of infection or re-rupture,” Toufic R. Jildeh, MD, of the Henry Ford Health System, told Orthopedics Today. “However, patients with longer tourniquet times and higher estimated blood loss [EBL] and those who had a history of smoking were found to be at increased risk for surgical site infections [SSIs]. Moreover, patients with longer operative times and tourniquet times were also found to have a higher risk of re-ruptures.”
Among 423 patients who underwent surgical intervention for Achilles tendon ruptures between 2008 and 2014, the infection rate was 2.8% and the re-rupture rate was 1%. “With regard to infection, we found that those who were current or former smokers had about a 4% to 4.5% increased chance of superficial infections,” Jildeh said. He noted longer tourniquet times and greater EBL led to an increased rate of deep SSI, factors that could be increased by the hypovascular state of the patients’ Achilles tendons.

4 Steps to a Successful Post-Merger Integration Process - Part 3
In order to be successful, a merger must be followed up with a detailed course of action. Here are several specific places where physician groups routinely stub their toes when merging. But, if you get them right, your success rate will soar.
3. Culture - Culture is woven into the fabric of a workplace over time. That makes it difficult to see and even more difficult to change. The list of deals that have famously failed because of mismatched cultures is epic. The first, and perhaps most important, rule in dealing with culture during a post-merger integration is to compare the existing cultures and see how they will be combined. Do not downplay the culture as something that will work itself out. It won’t.
The second rule in merging cultures is to know thyself. It is not possible to integrate two cultures based on an understanding of only one of them. Culture assessment should begin way before staring down the path of a merger—and well before the demands of a deal set in.

I have given up on carbocaine. No excuse not keeping up with demand, or at least letting physicians know what's going on. I switched to 2% lidocaine and add sodium bicarbonate. I’m happy to report that it works just fine.

I don't really have a suggestion of a shoe for this person and don't know how they would be affordable, as it would likely be a custom-made job and not reimbursed by insurance. I did just read this article and thought it topical.

One of the great pedorthic custom shoemakers, Arnie Davis, practices in San Francisco. Hopefully, your patient can get there. Eneslow celebrates 110 years in New York City helping patients like the one described with custom shoes for Charcot and any other deformities requiring custom made footwear. Often the best route for the patient and doctor is to send patients to pedorthists who specialize in customizing ready-made shoes and custom shoes for Charcot and other deformities outside the available ready-made shoes on the market. The American Board for Certification in Pedorthics and Pedorthic Footcare Association are resources to help find skilled career pedorthic professionals and custom shoemakers.

I agree with Dr. Borreggine's posting that this program may be a blessing to our profession. However, the word chiropodist comes from the Greek words "chiri" and "podi" which means hand and foot. Chiropodists used to treat hands and feet; in some states, podiatrists are still allowed limited treatment of the hands. "Pedi" is the Greek word for child, hence the name "pediatrician".
