PODIATRISTS IN THE NEWS
CA Podiatrist Discusses Choosing Shoes for Plantar Fasciitis

According to Holly Spohn-Gross, DPM, podiatrist and past president of the California Podiatric Medical Association, the best shoes to manage plantar fasciitis are ones that will support the arch of the foot and reduce pressure on the inflamed tendon.

Dr. Holly Spohn-Gross

“If the patient has a low arch or flat foot, I usually recommend a motion control or stabilizing shoe,” Dr. Spohn-Gross said. “If the patient has an average arch, then I lean towards a neutral shoe. Finally, if the patient has a high arch, then I will recommend a neutral or cushioned shoe.” She  typically suggests that patients wear running shoes. And in some cases, the need for custom orthotics or orthotic inserts will arise.

PODIATRISTS AND THE LAW
NY Podiatrist Gets Prison for Tampering With Evidence

A former podiatrist has learned his fate after pleading guilty to evidence tampering in connection with a second murder-for-hire plot against his wife, prosecutors have announced. Rockland County District Attorney Thomas E. Walsh II announced on Friday that 51-year-old Ira Bernstein, of Montebello, was sentenced to one-and-a-half to three years in prison for tampering with physical evidence. He pleaded guilty to the charge on Aug. 25, 2025.

PRACTICE MANAGEMENT TIP OF THE DAY
9 Ways to Smooth Collections at Your Practice

Here are nine actionable strategies to help your practice get paid faster, reduce write-offs, and keep patients satisfied in the process.

1. Verify insurance before every visit

It sounds basic, but skipping eligibility verification remains one of the costliest mistakes a front desk can make. When a practice fails to confirm active coverage, network status, and patient responsibility ahead of time, the result is predictable: denied claims and revenue that vanishes into the re-work queue. The American Medical Association recommends that administrative staff contact the patient’s insurer to verify active policy, physician network status, procedure coverage, and prior authorization requirements before the appointment. Building this step into your scheduling workflow, not just your check-in workflow, gives your team time to resolve issues before the patient arrives.

FINANCIAL TIP OF THE DAY
5 Retirement Tax Traps to Watch in 2026

Even in retirement, some income sources can unexpectedly raise your federal and state tax bills. Here's how to avoid costly surprises.

2. Social Security benefits and how they're taxed

Up to 85% of Social Security benefits might be taxable depending on your "combined income." Other retirement income, such as IRA withdrawals, pensions or investment dividends, increases that amount, which can make more of your Social Security subject to federal income tax.

Key points about taxes on SS benefits:
  • Combined income equals AGI plus nontaxable interest plus half of Social Security benefits.
  • Even a modest 401(k) withdrawal could push you from 50% to 85% of your benefits being taxable.
Tax tip. Monitor your combined income. Consider timing withdrawals from taxable accounts to reduce the taxable portion of your benefits.

SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM
Treating Female Athletes

Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:

1.  What Is the Female Athlete Triad?

The Female Athlete Triad is an older term used to describe a spectrum of interrelated conditions involving low energy availability, menstrual dysfunction, and decreased bone mineral density. At its core, the triad is driven by inadequate energy intake—whether intentional or unintentional. Screening should include a history of menstrual irregularities, stress fractures, restrictive eating or dieting behaviors, overtraining, and personality traits such as perfectionism, obsessiveness, or heightened anxiety.

Amenorrheic athletes are at significantly higher risk for low bone mass (premature osteoporosis) and stress fractures. In fact, stress fractures occur 2-4 times more often in female athletes than in their peers. A 2021 study on self-reported restrictive eating behaviors found that the prevalence of menstrual irregularities among high school female athletes ranged from 18.8% to 54%, highlighting just how widespread and under-recognized this issue remains.

CODING & BILLING Q&As FROM CODINGHELPLINE.COM
Query: Do podiatrists get paid for code CPT 77071?

My associate used this code and I want to know its parameters.

Codinghelpline com Subscriber

Response from Codinghelpline.comThere is no distinction between providers who may bill for this code. The description of the CPT code 77071 is as follows:

The manual application of stress to a joint by a physician or qualified healthcare professional during radiography, including the contralateral joint, if performed. It is used to diagnose joint instability, ligament tears, or fractures. This code is reported alongside a conventional x-ray code for the same area. You should reach out to the specific carrier for coverage limitations.
  •  The medical record must explicitly indicate the reason why this was done and that the stress was applied by a physician or other qualified health care professional (e.g., NP, PA)
  • It is reported in conjunction with the specific x-ray code
  • It is a separate charge from the X-ray, and typically reimbursed at a lower level.
Alan Bass, DPM, Manalapan, NJ

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Why have podiatrists surgical fees decreased over the past 10 years?
From: Sev Hrywnak, DPM, MD

Doing surgery is not the financial answer!

* Payer pressure and cost containment private insurers and government programs have pushed for lower reimbursement rates for procedures, often through fee schedule updates and value-based contracts. Global budgeting and utilization controls in some regions limit total spending, compressing per-procedure fees.

* Shift to value-based and bundled payments Bundled payments for episodes of care incentivize cost efficiency, which can reduce the negotiated amount paid per surgery as part of a total-care package. Emphasis on outcomes and standardized pathways tends to favor predictable, lower per-procedure fees over high, variable...

Editor's note: Dr. Hrywnak's extended-length letter can be read here.

RESPONSES/COMMENTS (MEDICAL-LEGAL)
From:  Allen M. Jacobs, DPM

With regard to the predicate that maintaining less malpractice coverage might discourage some malpractice litigation for lack of potential recovery, there is the alternative. I had the honor of serving as an expert witness in the defense of a very prominent podiatric surgeon in which a triple arthrodesis resulted in an amputation for an alleged (never proven) infection. Although underinsured, the plaintiff and his legal council pursued the malpractice action. A defense verdict was returned by the jury. However the potential for personal loss should a jury have made a large award destroyed this accomplished surgeon.

I watched the severe anxiety take its toll as the plaintiff attorney threatened the defendant doctor and his estate. Even with the defense verdict, thanks to the outstanding work of the defense representative (Joseph Brooks) and a truly attentive and analytical jury, the doctor retired as the result of the mental anguish. In another case, a wrongful death from a thromboembolism following an Austin bunionectomy, the concern of the defendant over potential personal payment of damages led to significant psychiatric consequences and retirement.

I am not an attorney and I understand that the ability to recover judgments from personal or corporate assets is varied from state to state. However, I would consider seriously the premise that underinsured will discourage malpractice actions and act as a firewall. My understanding is that malpractice damage awards have been increasing. You should seek the advice of your carrier or an independent healthcare attorney regarding this matter.
