DMU Receives Bako Grant for Podiatric Scholarships

Des Moines University’s College of Podiatric Medicine and Surgery (CPMS) has received a grant of up to $100,000 from the Bako Medical Education Foundation Inc. (BMEF) to establish an endowed scholarship that supports podiatric medical education. The grant will be paid over four years. “We are honored to receive this important grant from BMEF,” said Dean Kevin Smith, DPM, PhD. “The support from the foundation will help to reduce student debt and create educational opportunities for future podiatric physicians. ”

(L-R) Drs. Kevin Smith and Steven Berlin

“BMEF is pleased to provide this grant to Des Moines University’s College of Podiatric Medicine and Surgery,” said Steven J. Berlin, DPM, a board member and chair of the Bako Medical Education Foundation’s Grants Committee. “CPMS is an important educational and training institution for our future clinicians. We are honored to play a small role to further this important endeavor.”

PODIATRISTS IN THE NEWS - PART 1
NY Podiatrist Discusses Supination

If you’re someone who tends to put extra pressure on the outer parts of your feet while walking or running, your feet supinate. Supination happens when your weight rolls to the outside edges of your feet. It’s associated with people who have high arches, says New York City-based podiatrist Casey Ann Pidich, DPM.

Dr. Casey Pidich

“If someone has a high foot arch, the arch doesn’t flatten out when standing,” Dr. Pidich says. “Supination can be a result of genetics; it’s also from specific muscles overpowering others in your legs and feet.” For example, tight posterior tibial muscles (deep muscles in the backside of your lower leg) overpowering weak peroneal muscles can contribute to supination, she says.

PODIATRISTS IN THE NEWS - PART 2
Edema of Feet and Ankles Often a Vascular Issue: NY Podiatrist

Swelling in the feet and ankles is a common problem that can cause discomfort and pain. There are many different reasons why your feet and ankles might swell, and treatment will depend on the cause. Circulation decreases as we age, which can cause blood to pool in the veins of the feet and ankles, resulting in swelling, says Bruce Pinker, DPM, a foot and ankle surgeon at Progressive Foot Care.

Dr. Bruce Pinker

How to treat it: Store-bought compression socks can help reduce swelling by returning blood from the extremities, like the feet, back to the rest of the body. Regular exercise, like walking, can also help reduce swelling and improve circulation as you age, Dr. Pinker says.

PODIATRISTS IN THE COMMUNITY

NY Podiatrist Recognized By Mercy College

In celebration of Woman’s History Month, Mercy College is recognizing accomplished women such as Kathy Reilly-Fallon, DPM who are making a difference in healthcare. A busy schedule as a healthcare provider does not deter her unrelenting drive to give. Even after making a generous donation to support Ukraine last week, she and her foundation’s board members are currently organizing to hand-stuff teddy bears for the pediatric cancer patients at Ohmatdit, the largest children’s hospital in Ukraine, and  to donate funds directly to the hospital.

Dr. Kathy Reilly-Fallon

“When you do good, it comes back to you in unexpected ways, and it moves you in such an emotional way that your heart is never the same,” Dr. Fallon explained. “My hope is to always do great things with love.”

INTERNATIONAL PODIATRISTS IN THE NEWS
UK Podiatrist Shocked by Study Showing Poor Foot Hygiene

Did you know that a shocking 40% of Brits do not change their socks every day, with 5% admitting they change them less than once a month, a new survey out reveals. The survey, which was commissioned by Nailner for National Feet Week, also highlighted that 39% of Brits check the health of their feet and toenails less than once a month, with more than 1 in 10 (13%) admitting they never check the health of their feet and toenails.

Dina Gohill

Well over a third (40%) of all Brits do not realize that problems with feet such as cracked heels, dry skin, odor, and verrucas could be a sign of a health condition including low immunity, diabetes, or poor circulation. Podiatrist Dina Gohill says, “It’s shocking to learn how little care people are taking of their feet as they are great indicators of your overall health and well-being. Changing your socks every day is one of the key steps to keeping our feet and toes in great condition."

PRACTICE MANAGEMENT TIP OF THE DAY
5 Things to Consider When Selling Your Practice

2. Your EHR system has value; paper charts do not.

We did an evaluation for a solo ear-nose-throat (ENT) physician who had not converted to an EHR system. He had a healthy practice in an upscale New York City suburb. However, some interested ENT physicians walked away when they discovered the paper charts. They recognized the immense task of converting these charts - one by one, as these patients came in - to an EHR system.

CODINGLINE CORNER
Query: Losing DME Revenue

With higher deductibles, rising insurance costs, and the popularity of online sites like Amazon, we have seen an increasing number of patients refusing DME such as pneumatic walkers and braces, and opting instead to go on Amazon. I have concerns for this, as patients do not know what type of device to get (other than if we printed a picture) and also sizing themselves. From a business perspective, we are losing this revenue. How do you address this? Do you offer a cash option, of course, one that is not a “better deal” than insurance? Can we at least give them an affiliate link to the exact product we recommend, but then have them return for a fit check if they are opting for Amazon? I appreciate any input.

Codingline Member, Hickory, NC

Response: This is becoming a nationwide issue, competition with “Dr. Google”, and finding pricing less for products such as DME. A key point is that patients save the money perhaps but get substandard or the wrong devices for their problem… then want you to fix it.

Suggestion: find the best product for your patients, discuss in detail why it is the right one to use, and perhaps have the logo of your practice added. If you have the courage to label as the best one from your practice, it can help to show the confidence you have in the device and why you think it is best for your patients. I cannot tell you it fixes all, but can make the discussions a bit easier in some cases.

RESPONSES/COMMENTS (CODINGLINE CORNER) - PART 1A
From: Michael King, DPM

Jeff, I think the APMA leadership can answer that better than I can; however, I know there is an ongoing dialogue regarding your query. Yes, we feel it is ridiculous for skin issues/lesions to be lumped into nail debridements when on the DIPJ, or distal to the PIPJ. I know APMA has been in an ongoing debate over this and has made some headway. Needless to say, when some payers have the opportunity to bundle such things as "debridement" into one bucket, they do so. I can tell you, the coding committee members of the APMA are not in favor of such bundling.

Michael King, DPM, Nashville, TN

RESPONSES/COMMENTS (CODINGLINE CORNER)- PART 1B
From: Ed Prikaszczikow, DPM, David Freedman, DPM

Dr. Kass asked, "What am I missing?" He also asked, "Why has podiatric leadership not challenged and corrected this abusive rule created by Medicare? Dr. Kass may not be aware of the many steps that APMA has taken to improve this situation, and we want to share with PM News readers APMA’s advocacy and education efforts on this important topic. More information and resources are available for APMA members in APMA’s -59 Modifier and Routine Foot Care Claims Toolkit at apma.org/59toolkit.

Several years ago, APMA recognized that a code bundling problem existed involving CPT 11720/11721 and 11055-11057 when performed on the same patient on the same date of service receiving at-risk foot care. 11055-11057 would be paid, but...
