NC Podiatrist Discusses How to Tell a Wart from an IPK
 
According to Ainsley Roberson Rusevlyan, DPM, "plantar warts are caused by a virus. However, there are other types of growths that can affect the bottoms of our feet as well. One of those is called a porokeratoma (plural: porokeratosis), which is a type of hard callus that is easy to mistake for a wart. It is sometimes known as a “seed corn” because of its appearance. A podiatrist is able to determine the difference between the two conditions."

"The way to identify a plantar wart is for a podiatrist to remove the top layers of dead skin exposing many small drops of blood in the center of the lesion. This is referred to as capillary pinpoint bleeding and is characteristic of a wart. Pain with lateral compression, or squeezing the area from side to side, is another sign of a plantar wart. A porokeratoma will usually feel hard and smooth, without the roughness of a wart. Most of the time, it is quite small, no larger than a sesame seed. Warts are often larger. The pain can be unpleasant, feeling as though you have a splinter caught in your foot, or that you stepped on a piece of glass," says Dr. Rusevlyan.

According to Greg Pound, DPM, "The best way to avoid wounds is prevention. Self-screening and visual inspection of your feet will go a long way. Small and/or painless wounds on the feet can very quickly become larger, more dangerous wounds and infection sites if not monitored and promptly attended to. It is much easier to treat and heal a small wound than to try to heal a larger wound that may involve pain, discomfort, infection, scarring, and limb loss."

"Knowing what to look for is important. When you, a family member, or friend evaluate your feet, look for any open wound. Even a small one, particularly in the diabetic patient, should be addressed immediately. If there is yellow, foul-smelling drainage, swelling, pain, redness, or red streaking, your podiatrist should be consulted immediately," says Dr. Pound.

The COVID-19 pandemic radically altered the U.S. economy, with unemployment peaking at 14.7% in April 2020, according to the U.S. Bureau of Labor Statistics (BLS). While jobs have been gained every month since, lowering the unemployment rate to 8.4% in August 2020, millions of out-of-work Americans are still searching for jobs.
 
Podiatrists #22 on 100 Highest-Paying Jobs List
 
There is a strong correlation between advanced education and a higher salary—workers with higher education levels have higher wages and lower unemployment rates. Using 2019 data (last updated March 31, 2020) from the BLS, Stacker ranked the 100 highest-paying jobs in America.

Making charitable contributions are a tried and true tax-reduction strategy. But, in order to receive credit for your generosity, you need to be able to produce a receipt for each and every donation. If you didn’t take advantage of donating last year, here are a few things to keep in mind for 2021.
  • Donations don’t always have to be cash.
  • Consider opening a Donor-Advised Fund to lump multiple years’ worth of donations into a single filing year for a larger upfront deduction.
  • Consider donating appreciated securities, such as stocks or bonds, to get a double tax-advantage—you’ll get both the charitable contribution deduction and avoid capital gains tax.

RE: Painful Hand and Foot Plaques

A 56-year-old man presented to the Dermatology Clinic of the University of New Mexico School of Medicine with painful plaques he had for years on his hands and feet. He worked in construction and attributed the pain in his hands and feet (7/10) as the causes of the problem. He had tried acitretin to treat the plaques but stopped when his skin turned red and became dry and itchy. A diagnosis of hand and foot psoriasis with classic thick scale and erythema on his palms and soles was concluded. Additionally, on the sole toward the heel, he had hyperpigmented macules called mahogany spots, another hallmark of psoriasis. Pitting and distal onycholysis were also visible on his right ring finger.
 
Painful Hand and Foot Plaques
 
While UVA or narrowband UVB light therapy is a treatment option, it requires 3 visits per week, not conducive to his work schedule and daily activities. Acitretin was prescribed to decrease the abnormal proliferation of keratinocytes; however, he experienced dry skin and itching. Initially, however, he was treated with topical calcipotriene (a vitamin D derivative) and clobetasol (high-potency topical steroid) without adequate improvement. In addition, screening lab tests showed elevated liver enzymes, precluding treatment with methotrexate (and acitretin, which he’d received previously). He was started on apremilast, which can can cause diarrhea, depression, nausea, and headache. But other than some loose stools, he tolerated apremilast well and showed significant improvement in his psoriasis at a 3-month follow-up visit. (Painful hand and foot plaques, J. Fam Prac.
