ABPM International Board and Exam Committee Meets in Malta
The ABPM International (ABPMi) Board of Directors (BOD) and Exam Committee held their first in-person meeting hosted in Malta. The University of Malta is home to one of Europe's premier podiatry programs. ABPMi BOD celebrated the first certification exam completed earlier this year. The ABPMi offers a board certification examination for podiatrists who have completed education in the United Kingdom, Ireland, Malta, Spain, Portugal, South Africa, Australia, New Zealand, and the UQTR in Canada.

Those in attendance included the BOD, Representatives from the RCPSG, the International Federation of Podiatrists (FIP), and ABPMi Exam Committee members, including those of the host nation and the University of Malta.

Query: Problems With Bunions and Modifiers Again
What is the correct modifier for an Austin bunionectomy done on the right foot in 2023? Anthem is requiring a modifier. Previous similar posts on Codingline have left me confused. One respondent stated to use the -RT and -LT modifier. While another respondent stated to use a T modifier. The T modifier does not seem appropriate as the procedure was not done on a digit, but a metatarsal.

Response: You are right... to require the T modifiers is not appropriate for bunionectomy CPT codes. Many commercial payer rules do not make sense. However, to get paid, their rules have to be followed. Anthem requires the T modifiers on bunionectomy codes which are now defined by procedure itself rather than the older names. Those codes include CPT 28750, CPT 28289, CPT 28291, CPT 28292, CPT 28295, CPT 28296, CPT 28297, CPT 28298 and CPT 28299.

Dr. Bill hit a home run by pointing out in great detail the strange loss of focus between the Ivory Tower and those who do the work. When I founded DermFoot years ago, the incredible void in dermatology education was evident. On pages 4-7 of the Curricular Guide, in the list of content groups, it’s amazing to me that dermatology (skin and nails), a high percentage on every survey of what DPMs treat, is omitted.
I hope with the next rewrite, what the students will learn (what they need to be successful) is given more consideration. I can always wish.

Podiatry has tried for so long to achieve parity with allopathic and osteopathic medicine and has continued to come up short. Ideally, revamping the APMLE exams to be independently evaluated and made comparable to USMLE would be my first hope in bringing that initiative of parity to fruition. However, short of achieving that goal, the combining of a PA and DPM degree is a worthy alternative route. Physician extenders are gaining more independence and the future points to more independent practice as a Physician Associate (PA). That combined with the DPM degree would be of real value - initially perhaps with an online alternative as proposed, and later as an integral part of the DPM curriculum whereby a podiatry student would obtain a PA degree after 24-30 months of their joint podiatry/PA education studies, and then a DPM degree after 48 months, and then onto residency
.
This would allow some to leave after receiving the PA degree while others can continue on to a DPM and if at any time wish to practice as a PA rather than a DPM, they have that option. It would also provide for more options for people who might choose at any time to stop practicing podiatry and have a viable option as a licensed PA. Frankly, I am surprised that optometrists (OD) have not gone that route as many of their colleges currently provide a PA degree already, i.e. Ketchum in California and Salus in Pennsylvania.

I agree with Dr. Udell, a PA degree will not make us a better podiatrist. The million dollar question is if the PA degree will allow better reimbursement from insurance companies who discriminate against podiatrists with lower reimbursements for the same services provided by an MD. It would also allow for a greater scope of practice in those states that limit podiatrists to the foot but not ankle.

I’m not sure what the controversy would be in a podiatrist obtaining additional training or degrees. I see no negatives but a number of positives. In my state, PAs can see patients independently, even opening their own practices. For a podiatrist, this would mean being able to do their own H&P on their healthy patients, bypassing the need for shuffling to other providers. Even though orthopedics can do their own H&P, they often refer the pre-operative patient with comorbidities to primary care for clearance.
It’s really a travesty that we podiatrists cannot administer the simplest of injections if it isn’t in the foot, or in my state, even trim a fingernail. We would still need MD or DO oversight with the PA degree, but that is easily accomplished. We would no longer have to practice below anatomic structures for simple treatment of wounds, contusions, or sprains to name a few. If I had to venture a guess, an advanced degree would be seen as detrimental to the podiatric powers that be, who rely on contributions to drive the endless legislative fights. The advanced degree would negate that need.

At 87 years of age, and having practiced podiatry for over 55 years, I can think of only a handful of California podiatrists who have given as much of themselves to our profession as Franklin Kase, DPM.
Every podiatrist in the state of California owes a debt of gratitude to Dr. Kase for his tenacious efforts on our behalf with the private insurance companies of our state and with the California Insurance Commission itself. His pursuit of professional equity with the CMA, the COA, the California political structure (both state and local), with Medicare and Medi-Cal, have benefited every podiatrist in the state of California. In fact, every state has benefited by his endeavors by simply following his lead. He deserves more than a twenty-one gun salute. God love you Frank. Heaven needs a leader like you.

RE: The Passing of Franklin Kase, DPM (Michael Levi, DPM)
There is not much more that one can say that has not already been said about my dear friend and colleague Frank Kase. The monument of accolades already written of this giant of a person are no doubt a demonstration of what this person meant to all of us professionally as well as to his friends and family. He taught me a great deal about public policy and what he taught me was only a snippet of what he knew.
He embodied my grandfather's mantra- Zay a Mensch. If nothing else, be a Mensch. Frank was no doubt a Mensch, a noble and righteous person and someone to emulate in more ways than I can put into words. My condolences to Beth and all of Frank's family and friends.
