AT THE COLLEGES - PART 1

New ‘Paul & Margaret Brand Research Center’ at Barry Aims to Reduce Diabetic Amputations

The Barry University School of Podiatric Medicine is establishing the new “Paul & Margaret Brand Research Center.” Doctors Paul and Margaret Brand are pioneers whose groundbreaking research, from Louisiana to India, made it possible for insensate patients, including those with leprosy and diabetes, to function normally despite enormous handicaps. The landmark “Paul & Margaret Brand Research Center” at Barry University furthers the Brand’s legacy with collaborative research to reduce diabetic foot complications including ulcers and amputations.

Researchers at Barry University

“The mission of the ‘Paul & Margaret Brand Research Center’ is to create a world class research site utilizing the collaborative efforts of basic and clinical sciences,” said Dr. Jeffrey Jensen, dean of the Barry University School of Podiatric Medicine. “The research will result in publications in peer-reviewed journals, enhancing Barry University’s academic and scholarly reputation as well as the resident and student experience at Barry.”

Baucus Says Senate Won't Confirm Berwick

The Senate will never vote to confirm Dr. Donald Berwick as CMS administrator, according to the Senate Democrat leading his nomination. Berwick is currently serving through a temporary recess appointment.

Sen. Max Baucus (D-MT), chairman of the Senate Finance Committee, told reporters Tuesday that he has discussed Berwick's nomination with Republican senators and they plan to oppose Berwick under any circumstances. “Republicans won,” he said about the nomination. Baucus said alternative nominees to the post are "up to the president."

PRACTICE MANAGEMENT TIP OF THE DAY

Be a Low-Tech Presentation Master - Part 2

While everyone else is jumping on the high-tech bandwagon and overusing presentation options like PowerPoint, you can make a great impression in a low-tech way. All you need is a flip chart, some markers and tape—and these tips:

3) Practice tearing off pages and taping them to the wall. The key to flip chart use is the ability you gain to save thoughts and brilliant ideas to inspire participants later. You may want to start your tearing before the session or to prepare strips of masking tape and place them ready to use on the wall. You will speed the process and look more polished too.

4) Write large and legibly. Everyone in the room should be able to read your writing. That means you will need to print neatly. If you cannot write both quickly and neatly, recruit a participant to write ideas while you lead the session.

AACPM National Residency Development Facilitator Position

QUERIES (CLINICAL)

Query: Painful 1ST Met-Cuneiform Joint in Runner

I have a 28 year old male patient, an avid runner, who is having pain in the first met-cuneiform joint. He is 6 feet, 180 lbs. He only has pain while running. He took off a few months and biked, and is now trying to get back into running but still has the discomfort, described as a sharp pain. His foot type is a fairly aggressive cavus/supinated foot with ‘global’ convexity at the midfoot. There is motion/movement available to the first met and I am able to dorsiflex to the level of the second.

I could not elicit any pain with hands-on manipulation. He has been wearing orthotics for just over two years and wears them for ‘everything.’ I have made the recommendation for possibly an MRI or CT scan to specifically see if there is any joint destruction or soft tissue pathology. He has declined both. He was advised that the orthotics could potentially be modified to attempt limitation of first ray movement, though no guarantees can be made. Any help would be appreciated.

QUERIES (NON-CLINICAL)

Query: IMEX 3000DX Interference Problem

I have an IMEX 3000DX vascular analyzer. The Doppler portion has a speaker. That speaker now has a lot of interference--radio-static, making it difficult to hear the sounds. Although the strip chart works well, still, I would like to shut down the interference.  How can I do that? Do I need a new speaker? Does anyone know if IMEX still makes these speakers for these old machines ? Any help would be appreciated.

RE: Hyperkeratosis in an 87 year old (Elliot Udell DPM)
From: Gino Scartozzi, DPM

The patient certainly reveals an instability of the medial column with hypertrophic bone formation at the plantar aspect of the first metatarsal-medial cuneiform articulation. The patient would be well served with a multi-density device (leather, plastizote, cork, and 1-2 mm polypropylene shell with cut-out corresponding to callus site and a suede bottom layer.)  The use of the 1-2 mm of polypropylene fill with a "cut-out" corresponding to the callus would allow for both accommodative and semi-functional features of this orthotic.

In addition, it provides the orthotic an increased life span as compared to traditional cork-leather devices which very quickly "bottom out" and wear within a short period of time. A callus marked on the plaster cast for the orthotic lab to accommodate with the removal of polypropylene in this region can be accomplished by marking the skin with a surgical marking pen. The ink from the pen will transpose onto the plaster cast when removed for the laboratory to make the "cut-out" modification. I would then place the patient in a "rocker-bottom" type shoe to alleviate the dorsiflexory stress onto the first metatarsal-medial cuneiform articulation.

RE: Donation of Therapeutic Shoes
From: Chuck Ross, DPM

I would appreciate knowing if any of your readers know where I might donate a stock of therapeutic shoes that I have accumulated from various shoe company displays. I will no longer be providing therapeutic shoes, for a variety of reasons, and hate to just "throw them away." They are of various sizes, mens and womens, but unfortunately, ONLY single shoes/sneakers/boots, as they are the display shoes and NOT pairs.

I suspect that there is some organization that would, hopefully, be able to find a use.  I would be pleased to send them, at my expense, to anyone who is interested.

RE: Fraud and Abuse (Simon Young, DPM)
From: Philip E. Larkins, DPM

Dr. Young is correct. Open any magazine (most recent Bloomberg Business), skim any newspaper article, listen to any TV news. Doctors are the bad guys. We are the ones SOLELY responsible for the complete and utter collapse of healthcare in America. I guess all of that money that the health insurance companies pay out in salaries and bonuses and huge unnecessary payrolls is good because...

RE: OR Podiatrist Embraces Zero-Drop Shoes (Robert Steinberg, DPM)
From: Ray McClanahan, DPM

If we consider the medical literature regarding Achilles tendonitis, it is clear that this problem has increased since the introduction of elevated heels in athletic footwear. Jarvinen, T., et al. Achilles Tendon Disorders:Etiology & Epidemiology. Foot Ankle Clinics, 2005;10:255-66.

So, I understand how Dr. Steinberg, and many podiatrists have well-intentioned ideas about how elevating...
