RELEVANT RESEARCH
Scholarly Activity Among VA Podiatrists: A Cross-Sectional Study

Background: US Department of Veterans Affairs (VA) podiatrists advance clinical practices and ensure expert foot and ankle care for veterans. Quantifying the research expertise of VA podiatrists will enhance the confidence of the veteran population regarding the care they receive from highly knowledgeable and research-active clinicians, and provide podiatrists insight into their professional development.

Methods: This cross-sectional analysis aimed to describe the expertise of VA podiatrists; those with a history of peer reviewed publications were analyzed. Sex, region of practice, salary, academic appointment, number of publications, and h-index were recorded. Descriptive statistics were reported, and associations between variables were assessed. The threshold for statistical significance was set at P ≤ .05.

Results: Of 819 podiatrists, 157 met the selection criteria for final analysis, with most being male and located primarily on the West Coast. Significant differences were found in the median number of publications and the Hirsch index among academic ranks, but no relationship was observed between salary and these metrics. No statistically significant difference was noted in average salary among different academic appointments, although compensation did vary.

Conclusions: A substantial proportion of VA podiatrists have published their research, providing advanced foot and ankle care to veterans. These findings underscore the expertise available to veterans and highlight its role in enhancing clinical practices and professional growth among VA podiatrists.

PRACTICE MANAGEMENT TIP OF THE DAY
Stop Following the Herd: 5 Counterintuitive Habits of Winning Practices

Winning medical practices don't follow trends; they make bold strategic choices. These counterintuitive habits set market leaders apart. Consider these steps to quickly determine whether your practice understands what a real strategy is and whether or not they actually have a strategy that can be successful.

Step 1: Identify your long-term strategic moves

I often challenge physicians by asking, “Do you have a real strategy for long-term success?” Strategy requires disciplined analysis and a deep understanding of how not only the healthcare industry but also how your specialty is likely to play out in the coming years. Then you need to get very clear on the few, key strategic moves your practice needs to make in order to position yourself for future success.

Strategy requires tradeoffs. You cannot be everything to everybody; you cannot just blindly copy the moves of your competitors and hope to win. You have to figure out what to say "YES" to, and what to say "NO" to. You make clear cut choices about how you will compete in the future, and then allocate your time and resources accordingly. Keep in mind that less is more when it comes to strategy execution. I recommend that you whittle your wish list down to only a few strategic moves that will have the greatest impact, and say “No” to everything else.

FINANCIAL TIP OF THE DAY
A Tale of Three Audits

The IRS has three main types of audits—some being more painful than others.
  • Correspondence audit: The most common IRS audit, this typically is conducted by mail and sent for reasons such as missing information, small balances owed, or identity verification. (Note: Be aware that scammers have been sending fraudulent letters to collect funds from unsuspecting taxpayers. Identifying fakes isn't always easy, but typical red flags include demand for immediate payment, threats of jail time, and grammatical errors and misspellings. If you're unsure of a letter's authenticity, you can contact the IRS at 800-829-1040 or visit its website to confirm a letter's legitimacy or report an incident.)
  • Office audit: In this type of audit, taxpayers receive a written summons to go to an IRS office to meet with an agent. This generally happens with more complex tax returns or if the return has multiple items being questioned.
  • Field audit: The most comprehensive type of audit, this in-person meeting typically occurs at the taxpayer's business or home to conduct a thorough examination of the return in question. These audits can often last several weeks or even months, depending on the issues being reviewed and the complexity of the tax return.
For every type of audit, the IRS will provide in advance a written request for the specific documents it wants to see.

SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM
Time Management

Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:

5.  Keep a 24-Hour Log of How You Spend Your Time

Sometimes, you end the day wondering how on earth it disappeared. You were so busy, yet somehow didn’t get half of what you wanted done. Enter my latest self-improvement experiment: keeping a 24-hour time log.

The point isn’t to micromanage every minute—it’s about awareness. Once you ACTUALLY see how your day is spent, you realize you have a lot more time than you thought. You just might not be spending it where you think you are.

CODING & BILLING Q&As FROM CODINGHELPLINE.COM
Query:  Pressure Ulcers Underneath the Toes

If a patient has non-blanchable erythema with dried up bleeding underneath the distal plantar aspect of a hammertoe, with an intact epidermis, should a podiatrist avoid the ICD-10 code L89.611 for an evaluation and management service if the epidermis is intact?

The dermis might have been penetrated. If a corn or callus has bleeding inside the epidermis, is this coded using L84 with a secondary diagnosis of hammertoe M20.41/M20.42? Do you use an evaluation and management service if off-loading is provided and/or discussed and the chart states non-blanchable erythema with intact skin that did not fully penetrate the epidermis?

PM News Subscriber

Response from Codinghelpline.comIf your examination documents non-blanchable erythema with intact skin that has not penetrated the epidermis, this meets the definition of a Stage 1 pressure injury. The correct code would be: ICD-10-CM L89.611 — Pressure ulcer of right heel, stage 1 (Includes terms such as healing Stage 1 pressure ulcer and pressure-related skin changes limited to persistent focal edema of the right heel.) You would not assign L84 (corns and calluses) based on the description you provided, as that code does not reflect pressure-induced tissue...

QUERIES (MEDICAL-LEGAL)
Query: Why does an insurance company require high limits of liability for providers who are being credentialed?
From: Joseph Borreggine, DPM

Recently, I have come across information indicating that health insurance companies may mandate providers on their panel care high liability limits regarding malpractice insurance. Certain companies based on geographic area or nationally are requiring a $1 million to $3 million liability limit to even consider credentialing a physician. I am uncertain whether this requirement applies universally or is specific to certain insurance companies. However, I find this development intriguing, as there are many podiatrists who do not participate in surgery and primarily just practice general podiatry.

This situation is appropriate regarding podiatrists who are foot/ankle surgeons, but I am confident that there are many of us who continue to practice general podiatry without performing surgery for various reasons. This limits access to a number of qualified experienced providers with certain carriers who do not carry these high limits of liability on their policy. Additionally, I am also curious to see if this has affected any of my colleagues and prevented them from being credentialed?

A search on the Internet yielded the following response to my query regarding obtaining high liability limits: “Credentialing organizations often mandate higher liability limits to ensure that healthcare providers can cover potential claims. This requirement helps maintain a standard of care and protects both the provider and the institution.”
