ME Podiatrist Joins Auburn Hospital Practice
Auburn Community Hospital has announced the addition of Aroob Moin, DPM, to affiliate Auburn Podiatry Associates. Dr. Moin joins the Auburn practice from Penobscot Community Health Care at Brewer Medical Center in Maine.

Because they're designed not to hinder your foot's natural movement, minimalist shoes can be great for those with plantar fasciitis or neutral feet. But, for those who are used to running in a traditional, somewhat cushioned shoe, it will take some adjusting.

lerEXPO Sets Record For Most Comments
This past Saturday, lerEXPO hosted and moderated a free 3 CECH event entitled The Orthotic Theory Roundtable. "With the podiatric schools de-emphasizing biomechanics in their curriculum, we felt it was time to open up the conversation,” said Rich Dubin, CEO lerEXPO. We were able to bring together Kevin Kirby, DPM, Ed Glaser, DPM, and Dennis Shavelson, DPM to present three different theories/philosophies and then bring them together for an open Q&A roundtable forum moderated by Jason Kraus and Rich Dubin.

The conversation was very lively and thought-provoking, and stimulated a lot of comments. lerEXPO once again set records. This time is was the number of comments. With 357 attendees the expo had over 200 comments. "The lerEXPO platform makes learning fun and engaging," said Rob Conenello, DPM. The Western University School of Podiatric Medicine partnered and supported this event.

If You Must Wear Flip-Flops, Here's What to Look For: GA Podiatrist
Most podiatrists recommend avoiding flip-flips due to their flimsy nature. If you decide to pick up a pair regardless, you'll want to look for a few features. Gregory Alvarez, DPM, a podiatrist at the Ankle & Foot Centers of America, suggests looking for a pair with cushioning and arch support. "Flip-flops with a wide, supportive base will help reduce strain on the feet and ankles when walking or standing," he says.

You should also examine the straps. "Make sure that the straps and any other closures fit snugly but comfortably around the foot; this will also help improve stability when walking," Alvarez explains. Finally, look for shoes with shock absorption technology or memory foam footbeds. "These features can help reduce the impact on joints," he says. Styles with rubber soles can give additional traction and grip on wet surfaces, which can help prevent slips and falls.

I read with great interest the 2023 AACPM Curricular Guide for Podiatric Medical Education and have some serious concerns as there seems to be a significant disconnect in what we are expecting podiatric medical students to learn and what podiatrists actually do in private, group, and clinical practice. Specifically, the dearth of dermatology competencies and educational learning objectives is the greatest oversight and weakness in the document as the average podiatrist treats skin and nail conditions far greater than any other disorder (well over 50% of all patient encounters, according to Medicare data), yet the AACPM does not even include dermatology as a “Core Competency” like radiology, orthopedics, surgery, geriatrics, or behavioral medicine, and buries relatively few dermatology learning objectives under a small section of general medicine.
There are approximately 4,000 individual and specific learning objectives covered in 248 pages of the document, however there are only 58 learning objectives for dermatology and 12 for dermatopathology that are contained in less than 2 pages of...

A Northwestern team of surgeons and engineers has invented an electric bandage to speed wound healing. Racial Variation in Treatment and Outcomes Among Patients with PAD
Peripheral artery disease (PAD) is a common condition with significant racial variation in the rate of diagnosis, treatment, and outcomes. To build upon the existing literature, researchers conducted a comprehensive, retrospective study evaluating trends in diagnostic testing, treatment patterns, and patient health outcomes after diagnosis of PAD between commercially insured Black and White patients in the United States.
Compared to White patients, Black patients were observed to have higher disease severity leading up to the PAD diagnosis and increased risk of long-term cardiovascular and limb complications, despite increased diagnostic testing and medication use following PAD diagnosis. The findings suggest that Black patients with PAD experience substantial disease burden under the current standard of care. Future interventions should be tailored to facilitate earlier diagnosis of PAD and to improve the standard of care for Black patients with PAD, which, in turn, could reduce disparate adverse outcomes following diagnosis.

I am entering this discussion from a totally different perspective. For the last three years of my parents' lives, I was their caregiver. I drove behind many ambulances and often took them to urgent care facilities and plenty of doctors' offices. Combined with my being an allergy and breast cancer patient, I have had a lot of experiences with PAs. Some urgent care facilities are completely run by PAs and some hospital ERs are also run by PAs with a physician behind the scenes okaying whatever they do. What I have gleaned from these rough years is that some PAs are well-trained and have far more patience and good bedside manner than the MDs they work for. In other cases, the visit with the PA was a waste of time.
On Thanksgiving, I was in urgent care as a patient and the PA was honest with me. He had never worked with a patient with my symptoms and urged me to either go to the main ER or see my primary care doctor the next day. When my mom broke her pelvis, the PA at the hospital ER properly diagnosed it but the treatment plan she gave my mother bordered on negligence. We had to return to the ER the next day where we saw an MD certified in ER medicine. He addressed things properly and let us know that the PA mismanaged my mother's case. Should podiatrists aspire to become PAs? Will it make us better foot and ankle specialists? I doubt it.

On Saturday, APMA lost a giant, Dr. Franklin Kase, and I lost a dear friend. Over my 33-year career, I have been blessed to have had many mentors; none like Frank Kase. When my father passed in 2007, Frank became someone that I could speak to and get sage advice as I did from my dad, and for that I will be forever grateful. For those who knew Frank, there were 4 things that were most important to him: his family-wife Beth, daughters- Alison and Marissa and their families—his faith-his friends-his profession. Very few people gave as much as this man without expecting anything in return. He was a fiercely loyal individual and protective of the people he loved.
I had the pleasure of awarding him with the President’s Award when I finished my term as APMA president. With the help of former APMA President Jeff DeSantis, we were able to organize it as a surprise and have his family present. I believe it was the first time I saw Frank Kase speechless. Those who knew him know that that was nearly impossible to do.
There is no doubt that Franklin Kase left his mark on the world, both personally and professionally, and I will miss him greatly. My condolences to the Kase family. I want to thank them for sharing Frank with the podiatric world. Rest in peace Frank and see you on the other side.

It’s very sad to hear of the passing of Frank Kase, and last week Leonard Levy. Our field has lost two major opinion leaders who had a significant impact on propagating podiatry and moving it forward in many ways. It is an unfortunate loss, and both will be greatly missed professionally as well as by their families. I share my sadness with my colleagues who knew them.

RE: The Passing of Franklin Kase, DPM (Michael Levi, DPM)
It’s hard to concisely describe Dr. Frank Kase’s influence on the world. One word that comes to mind when describing Frank is devoted. Another is advocate. Another is friend. Another is healer. He was a devoted family man, but I mostly knew him as a friend and advocate for his patients and for our profession. He was a leader in California and nationally. He was tireless when seeking to correct wrongs or injustices that affected the practices of podiatrists.
It seemed that he had insurance company executives on speed dial and that he had befriended many of these executives. It was easy to like this good-hearted man. I also know that he had the rare quality of someone who could advocate for our profession tenaciously while still gaining the respect and friendship of those whose minds he was trying to change. As Mike King stated, he was a Titan and as Mike Levi said, he was a mensch. He will be missed by his innumerable friends. May his memory be a blessing.

Query: Prior Authorization
Medicare Advantage services require prior authorization be obtained for a visit. If a podiatrist spends time on a phone (15 minutes to 20 minutes) obtaining a prior authorization for a visit, before the visit is done, and the phone call for prior authorization is on the day of the visit, does this time spent count towards the level of evaluation and management billed?

Response: Generally obtaining an authorization would not require the podiatrist’s level of skill, though, that is not always true. I think it would be risky to include that time since it would likely be denied in an audit.
I suppose if the reason you handled the authorization call yourself was because the clinical issue was highly complex, then perhaps you could make a good argument, but it may not be worth the time and effort for a few extra dollars.

RE: The Passing of Franklin Kase, DPM (Michael Levi, DPM)
Frank Kase was a gift to us from above and his passing is a very painful loss for all of us in the profession. We were friends when I was practicing in California and became even closer in the 1980s when I was struggling with establishing a podiatry licensure in Israel. He always helped by presenting a different approach to every problem that I would run by him. And he always contacted me when he came to Israel. He was insightful, perceptive, generous, warm, and down to earth. He had many personal attributes from which we all benefited including his great devotion to the profession. Farewell, Frank. You will always be loved and remembered by all of us who were honored by having had you in our lives.

RE: MI Podiatrist Sentenced for Billing Medicare $1.8M Under False Identity

The May 8th issue of PM News contained two more published reports about podiatrists who have committed insurance fraud. Regular readers have been seeing an upsurge in these punitive actions. For various reasons, every podiatric practice billing insurance, and particularly Medicare and Medicaid, is now under high risk of audits and potential adverse consequences.
Forewarned should be forearmed. For many years, the Federal Office of the Inspector General (OIG) has recommended that solo and small group practice physicians implement its insurance billing compliance programs for providers and staff. The OIG recognizes that not all aberrant billing is intentional and the fault of the provider. In fact, the OIG states that having a compliance plan in place is a "mitigating factor against punitive action."
It is sad that the vast majority of honest podiatrists have to go through audits to literally prove their honesty, but that is the reality we face. Having an OIG recommended practice compliance program in action when an audit occurs should be a standard operational practice in today's regulated insurance reimbursement environment.
