News Stiletto Designed as Wedge Offers More Stable Base: CA Podiatrist
Comfort and style are not easy to find in a stiletto, but entrepreneur Dolly Singh wants to re-invent the heel. Singh said her fundamental goal is to make a stiletto that feels like a wedge. The former SpaceX employee said a metal insert can be found in most stiletto shoes, which forces the foot into a precarious position. "You are basically walking around on your toes in a high-heeled shoe today - 80 percent of your body weight is on the balls of your feet," she said.
|
|
Dr. Robert Lee |
Commenting on Singh's shoe, Dr. Robert Lee, the head of podiatry at Santa Monica's UCLA Medical Center, said compared to a spike heel, a shoe designed like a wedge should offer a more stable, wider heel base, with much less of an incline. "There does appear to be a slight wedge construct in the forefoot. And if that's what it is, then it should be more comfortable and maybe does help to off-load that forefoot weight," he said. Better weight distribution should equal less pain, but Lee warns standing for hours in even the most comfortable high heel can lead to foot problems.

FL Podiatrist Charged With Writing Prescriptions Improperly
St. Lucie County sheriff's officials arrested Dr. Douglas Howard Kahn on 30 felony charges after accusations he used the professional information of a retired doctor to write or call in prescriptions, according to records obtained Monday. Retired Dr. Aaron Shevlin contacted sheriff's investigators after noticing "suspicious activity" from Kahn. Shevlin said Kahn bought his practice nearly five years earlier. He said he didn't give Kahn permission to use his information, such as his special Drug Enforcement Administration numbers, which are "identifying marks" for each doctor in the continental United States, records state. Kahn said he used Shevlin's professional information to write prescriptions because he didn't renew his own DEA number.

RE: Dealing with an Incompetent Attending
We often look at the lack of residency programs, but what about the quality of the ones that do exist? As a resident, I have a lot to learn. I know this. But, what do I do when I have an attending affiliated with my residency program that is horrible in the OR? By horrible, I mean very poor in technique, illogical in selection of procedure as well as execution. He amputates things that don’t need to be amputated, fuses joints on people that don’t need to be fused. The hospital I work in has a very large population of low-income minority patients. I know this attending only wants to help, but I’m beginning to feel that they have NO regard for these people, but rather just view them as guinea pigs.
I’m witnessing these events and it is so frustrating. I feel so lost, and I don’t know what to do. I have not learned a single thing from this attending; only what not to do. This attending is a wonderful, easy going, friendly attending, but in my eyes: an incompetent surgeon. Without being too specific, I witnessed a procedure where I believe more harm was done than help. Despite my protest, there was nothing I could do but stand there and watch it happen. I’m writing to vent - but also to seek advice.

From: Eric J. Lullove, DPM
I applaud Dr. Lai’s informative discussion of the need for DPMs to join as members of wound care organizations, but as the podiatric physician board member of the Association for Advancement of Wound Care, I find the need to reply directly.
AAWC is the largest non-profit, multi-professional association of wound care specialists with over 2,200 members. Our membership represents practicing wound care specialists including physicians (MD, DO, and DPM), nurse practitioners, clinical nurse specialists, physical therapists, and certified wound care nurses. In addition, we have two-hundred patient and lay-caregiver member advocates.
In response to Dr. Lai, the AAWC not only made...

RE: Continued Noridian CPT 11055 Denials (Richard Papperman, MBA)
From: Philip Larkins, DPM
The response to this post misses the point. The point of the poster is to say that, somehow, magically, Noridian Medicare was able to stop paying for qualified foot care as of October 1, 2015. Coincidentally, that was the switchover date to ICD-10. Somehow, what was covered by Medicare the day before, is now no longer covered. Of course, my tongue is in my cheek. This is a mistake by Noridian, and evidently they have no desire to fix it properly. The code to use for 700, is now L84, but Noridian does not seem to grasp this concept. I have now sent all my denials to an ALJ and I intend to waste their time with these nonsense appeals. Maybe someone else can shed light on this.
Philip Larkins, DPM, San Diego, CA

From: Elliot Udell, DPM
Kudos to Dr. Miller for pointing out the association between low levels of vitamin D and stress fractures. This gives reason to clinically monitor vitamin D levels in many of our patients, especially those presenting with a history of a stress fracture. We have picked up many cases of vitamin D deficiency in our patient population. There is a problem that we have been encountering in our geographic area. Many insurance companies, including our local Medicare carrier will not approve blood testing for vitamin D levels unless you can document that the patient already has osteopenea or osteoporosis. This creates a practice management problem when the patient gets a large bill from a blood lab saying that the test was denied. I'd be interested in knowing how practitioners in other localities are dealing with this problem.
