RELEVANT RESEARCH
Mohs May Produce Lower Recurrence Rates Than Wide Local Excision for Melanoma

Mohs micrographic surgery for the treatment of melanoma may be associated with lower recurrence rates than wide local excision, according to a meta-analysis presented at the 2019 American Academy of Dermatology Annual Meeting, which took place from March 1st to 5th. A study author warns, however, that randomized controlled trials are needed to confirm these findings.

To find out how Mohs compares with wide local excision, the investigators conducted a systematic review and meta-analysis of 74 studies comparing local recurrence rates of melanomas treated with each of these treatment modalities. Of these studies, 15 were comparison studies with two arms comparing patients with melanoma and a history of Mohs or wide local excision as the form of treatment. The other 59 were single arm studies reporting one form of treatment. The single-armed studies included 29 on wide local excision and 45 on Mohs. Taken together, these studies revealed a local recurrence rate of 2% with Mohs and 7% with wide local excision.

PRACTICE MANAGEMENT TIP OF THE DAY
4 Steps to a Successful Post-Merger Integration Process - Part 2A

In order to be successful, a merger must be followed up with a detailed course of action. Here are several specific places where physician groups routinely stub their toes when merging. But, if you get them right, your success rate will soar.

2A. Patients - Attention to patients should seem like a priority at all times, but it is particularly true during periods of change and uncertainty. Because mergers are times of great internally-focused thinking and activity, an outward focus on patients can suffer during this time. Competitors will seize any open opportunity to lure your patients away, and practice upheaval is one such door. They may spread misinformation and regale your key referring physicians with the dangers of your merger and the glory of their own practice. Do not let that happen.

QUERIES (NON-CLINICAL)
Query: Custom Shoes for Patients with Gout

I have a very unique diabetic patient who has the most severe case of tophaceous gout I have ever seen. The patient's hands are affected as well, so the use of laces is out.  My question is:  Are there any other custom shoe makers that can make a shoe for a really, really deformed foot in constant danger of ulceration, especially around the tophi?
