TX Podiatrist Discusses Causes and Treatments for DFUs
Identifying and addressing foot problems as a result of diabetes should be a priority for diabetics. A podiatrist with Baylor College of Medicine outlines the types of foot issues that can be encountered and how they can be treated. “Diabetes is a multi-system disease, and the circulation in the feet and legs may be diminished because there are problems with blood vessels that get narrowed or clogged as a result of the diabetes,” said Ronald Lepow, DPM, assistant professor of orthopedic surgery at Baylor. “A major cause of foot problems in diabetics is lack of blood flow.”

A common foot issue in diabetics is that pressure areas, such as callused or injured areas, do not heal as rapidly due to poor circulation as they would in a person who is not diabetic, Dr. Lepow said. This can lead to ulcerations or a variety of infections in these traumatized areas. To treat an ulcer on the foot, a podiatrist will take all precautions to ensure there is no pressure in the area of the ulcer. This can mean wearing an orthotic, special shoe, or even a boot during the time the ulcer is treated.

Aussie Podiatrist Opts for Shopping Center over Medical Center
People seeking help for treatment of foot disorders or wanting custom-made shoes for sports have a new option. MyFootDr, part of a national chain, has opened in the Orange City Centre. Podiatrist Chris Lofts said they offer diagnosis and treatment, as well as selling orthotic-friendly shoes. “We’ve got a bio-mechanical and sports medicine focus,” he said.

“We offer a full scope of services from comprehensive foot assessments, gait analysis, and custom foot orthortics to treatment for fungal nails and nail surgery. We also make custom-made cricket shoes, and golf shoes.” Lofts said they wanted to be in a shopping center rather than a medical center.

Fed Initiative to Put Patients in Control of their Health Data
The Trump administration is attempting to put patients in control of their own health data and to spur healthcare innovation as part of its broader efforts to cut red tape in the federal government. The Centers for Medicare and Medicaid Services announced on Tuesday at the HIMSS18 conference in Las Vegas the launch of the MyHealthEData initiative, which CMS Administrator Seema Verma claimed is the first time any administration has set in motion a coordinated initiative to truly make patients the focus of the healthcare system by using the federal government's full authority to ensure providers and insurers give patients control of their medical records.
According to Verma, patients might be able to obtain some information from providers within a healthcare system but there’s no way to aggregate that data, and when providers don’t have all the necessary information at hand, they repeat tests, driving up costs and causing medical errors—which threaten patient safety. “The MyHealthEData initiative will enable doctors to spend more time with their patients and less time looking at their computer once our policies go into effect,” added Verma. “It will declare to hospitals and insurers that the practice of holding patients hostage in their systems and blocking their data will no longer be tolerated.”

Meaningful-use Overhaul is Coming: CMS Administrator
CMS Administrator Seema Verma said her agency is planning on overhauling its meaningful-use requirements, after years of provider complaints that the program was too burdensome and hard to implement. Specifically, the CMS is looking to reduce time and compliance costs associated with the program. Verma didn't provide any additional details during her speech at HIMSS' annual conference on Tuesday.
The agency's moves come just weeks after President Donald Trump signed a funding bill that included measures to ease meaningful-use requirements and expand telehealth access for Medicare beneficiaries.The meaningful-use bill, introduced last summer, could make meeting electronic health record meaningful-use requirements easier since they no longer have to become stricter over time.

2. Review management
Your online reputation is influenced by your online reviews. There are numerous review platforms such as Yelp, Healthgrades, Vitals, YellowPages, etc. that allow consumers to post reviews and others to read what users have experienced. So you need to proactively check review sites for any patient reviews and reply accordingly.
You should check what people are saying about your competitors and how they are handling reviews. Also, take a look at their ratings on various review sites. Knowing what your competitors are doing better than you can help you formulate strategies and improve your practice’s marketing and operations. Don’t forget to ask for reviews from your happy patients when they visit your practice or through mail and messages.

In conversations with my billing company E-Professional Technologies, we have researched this situation and, yes, this is a national issue and we are equally outraged. We can still obtain information on Medicare deductibles for regular beneficiaries. However, there is a fairly new program for Medicare/Medicaid beneficiaries called Qualified Medicare Beneficiary (QMB).
For QMB patients, providers are not permitted to charge the patient anything. So the government, in their infinite wisdom, has decided we are not entitled to know how much we are paying on behalf of each patient! If we can't collect the money, then essentially we are paying the bill on their behalf. Is it $1 or is it the entire $183 deductible? We are not allowed to know. If you see 100 patients at a nursing home in one week, that could mean $18,300!
I called First Coast Service Options (which is the Medicare MAC for Florida) and asked them how we could determine the amount that we would be responsible for, and I was told we are not allowed to know. I have written my congressman about this. It is literally outrageous that a doctor is responsible for paying a patient's deductible and they are not allowed to know how much is at stake. I am now playing a game of holding all QMB claims until I see that all other patients have met their deductible and I'm going to assume they have too, but that's not very accurate.

RE: The Importance of Challenging Medicare

So, I noticed that my Medicare allowable was less than the Novitas website posted allowables for my claims coming back for 2018. There is a Novitas mistake that they were following the WRONG fee schedule up to 2/23/18, and they are working on resubmitting corrected claims. This is one problem fixed.
And, I notice that the increased payment adjustment is in the form of a positive ($xx.xx) adjustment on our EMRs. This is quite a software accounting nightmare. How are others handling this? These are a few of my questions because I am NOT receiving increased payment from the many, many....

RE: Lack of Podiatric Authors in Our Journals

Many have lamented and seek answers on the lack of high quality peer-reviewed published podiatric research. Some blame the colleges, others blame APMA. I take a different view. Why does there have to be blame on any level? The importance of original academic research in medicine is self- evident. However, it is still a minority in all fields who churn out the research.
Be that as it may, since 1977, when I started podiatry school, there was NEVER a milieu for research in any of our colleges. Original research is much more complicated than one might think, and very costly. Since working in a very research-oriented academic institution, I can tell you that “research” is done on weekends and nights, with armies of...
