ME Podiatrist Builds Global Initiative
One Maine doctor has taken his passion and turned it into a global initiative. James Fullwood, DPM is expanding his leadership skills to advance healthcare in Maine. “Maine leaders need to make sure that we have the appropriate education and training,” said Dr. Fullwood. “As the market changes, we need to change.” Fullwood has worked at Northern Light Sebasticook Valley Hospital since 2013 as a podiatrist and surgeon.

While his education has served a great purpose, he wanted to learn more. He traveled to Nigeria to build diabetes awareness. “It was a different world,” said Fullwood. “It was beautiful, it was engaging. There was so much to do, so much to learn.” He and 10 other doctors saw the need for diabetes access and education. They founded the International Limb Salvage Foundation, a non-profit that helps West Africans.

Critical Shortage of Podiatrists in Bahamas
The Bahamas faces a critical shortage of podiatrists, with only four practitioners serving a population of nearly 400,000 people, according to Monique Mitchell, DPM, MPH, a local podiatrist working with the Ministry of Health. She said the shortage is particularly pressing given that nearly 20 percent of Bahamians have diabetes, a major contributor to foot-related health issues. She said without proper care, foot complications could lead to amputations, and up to 70 percent of people who undergo such procedures may not survive beyond five years.

Dr. Mitchell, who co-founded the Foot Fest initiative with Shanika Hill, DPM in 2020, underscored the urgent need for greater podiatric care in The Bahamas, explaining that the initiative, now in its fourth year, started as a response to the aftermath of Hurricane Dorian and has since grown to involve 54 volunteers, providing free foot care and screenings to over 1,000 patients annually.

Query: Insurance Reimbursement
Charges were submitted to a patient's insurance for care they received in mid-February 2024 and the insurance paid us shortly afterwards. We recently received a request from the insurance company to reimburse the amount paid a year ago. They state we were paid by mistake as the patient's benefits had terminated prior to the date of services.
Reimbursement amounts to a few hundred dollars. Are we obligated to reimburse the insurance for their mistake as it would be difficult to bill this patient for services a year later?

Allen, the cases you cite are not typical results and are primarily outliers. Of course, an egregious case led to an excess verdict, that is one above a doctor’s insurance coverage. But most podiatry cases settle for amounts below the typical $250,000 limits. Of course, venues matter when it comes to jury awards.
Personally, I do not subscribe to low limit policies for malpractice coverage but mentioned it because a local group of general surgeons all reduced coverage to $250,000 per doctor and saw a dramatic reduction in lawsuits.
Also, besides asset protection, which I strongly suggest, most excess verdicts can be handled through bankruptcy. Not a great alternative but where available, it's a cure.

The Evans calcaneal osteotomy is a procedure that I have utilized throughout my career. I was told early in my residency training that the Evans is an extremely powerful osteotomy as it can provide triplane correction to an unstable flatfoot. I am not saying anything that those reading this do not already know, and this is not meant to be a treatise on the Evans calcaneal osteotomy. We all know when done correctly this osteotomy greatly helps those with a painful unstable flatfoot.
In my practice, the majority of the people receiving this osteotomy are those who suffer from posterior tibial tendon dysfunction, and it is done in concert with other procedures. Once again, I am not saying anything that my colleagues do not already know. What I and perhaps many of us do not know are the details of the osteotomy performed in the aforementioned litigation. Were the ligaments of the calcaneocuboid joint compromised? Was the long plantar ligament...

A fortunate gift best describes my introduction to Mike Boxer 48 years ago. At the time, I was an undergraduate student exploring several career options. Mike welcomed me into his office which was also part of his home. Because my visits were frequent, I also came to know his charming wife, Karen.
Mike was a generous mentor and devoted friend. Our relationship extended through the full measure of a professional career and beyond. Mike will be revered and remembered by the thousands of students he helped educate and the countless patients whose lives he improved. My sincere condolences to Karen and the entire Boxer family.

I had the privilege and honor of being one of Dr. Boxer's "selected" students when I was at NYCPM. That meant that you were invited to his office. I remember sitting in his office until midnight every Tuesday seeing patients. Just watching him interact and treat patients gave me a unique perspective on what it meant to be a "podiatric physician".
He was a "doctor" in a time when most podiatrists simply were not. As Director of my 2nd year residency program at Peninsula Hospital, he also taught me what is was like to be a true member of the medical staff. He was respected by all of his allopathic colleagues. He looked the part, walked the walk, and talked the talk. He elevated our profession at a critical time in our history. Rest in peace Mike.
