Bahamas Podiatrist Discusses Differences Between Men's and Women's Feet
 
According to Monique Mitchell, DPM, MPH, "Of all the differences between men and women, feet probably aren’t the first to come to mind. But women’s feet are different from men’s feet in a number of ways. Women and men are not equal in regards to anatomic structure and function of the lower extremities. This can cause variations in every day walking, exercising, and shoe wear patterns between the genders. These variations can explain certain medical conditions affecting the foot and ankle and there are lots of foot issues that affect women more than men."

"Because of the anatomical (structural) and biomechanical (functional) differences between women and men’s feet, there are a number of foot issues that women are more likely to suffer from. These include bunions, corns and calluses, hammertoes, Morton’s neuroma, conditions resulting from wearing high heels, runner’s knee, and stress fractures, says Dr. Mitchell.

According to Bruce Pinker, DPM, a specialist in foot and ankle sports medicine and associate of the American College of Foot and Ankle Surgeons, “Running socks are beneficial as they can help provide shock absorption and cushioning, which will reduce impact on the feet, and with the proper blend of materials, they can also help keep the feet relatively dry and healthy.”

According to Dr. Pinker, “Added cushioning in running socks is preferred, especially for long distance running, as it helps manage the impact on the feet which can help prevent injury. A blend of cotton and polyester is recommended (65% cotton and 35% polyester), and the polyester can be substituted for other synthetic materials, such as rayon or nylon.”

Query: AI Medical Scribes
 
I am curious as to what my esteemed colleagues feel about AI medical scribes. There's a lot to discuss. Are there medical-legal ramifications about the note (can one just blame the AI in a deposition?) Do you think patients want their notes taken in that way? Has anyone out there tried it, how does it pick up on a biomechanical exam with ROM measurements and x-ray reports? 
 
Will notes have more errors or less? If we will need to re-read the notes that the AI made at the end of the day to make edits, does it really save us time? Are there ethical implications? Informed consent ramifications? Or any other unintended consequences?

I couldn't agree more with Dr. Thorckmorton's comments. In our cherished goals of becoming equal to MD/DO orthopedic surgeons with 3-year mandatory surgical residencies, we have inadvertently given the keys to the store to others. There are many other providers who now wish to fill in the vacuum left by podiatrists abandoning biomechanics for the scalpel. In addition to our MD/DO colleagues, we now have other specialties such as dermatology publishing biomechanical predictors and NPs wishing to fill this gap.
 
What can set podiatrists apart from others is the application of biomechanical principles which we held so near and dear in the past. My fellow 65+ year old practitioners will easily tell you that if you follow your patients long enough, you will find that the results you achieve on the OR table may not stand up if you don't understand, address, and correct/respond to the biomechanical pathologies of your patients.

I would like to add to what John Throckmorton, DPM said about biomechanics. Biomechanics provides a window to the lower extremities and lower back. Conservative care is always a preferred benefit when successful. Sports medicine along with biomechanics allows treatment for the lower extremities' muscles and joints. Overuse injuries are a big part of this.
 
Our training in biomechanics is unsurpassed by other professions. Orthopedists frown having us around because we can provide non-surgical relief. After all, they are called orthopedic surgeons for a reason. The lack of exposure and access to the lower extremities and lower back is where podiatrists are limited. 
 
The orthopedist would rather get paid for the surgery than not. It comes down to money not what podiatrists can do for their patients. I tried unsuccessfully to expose to other professions what a podiatrist can accomplish at medical schools. As you might have surmised, they were not interested. We have to remember that we have a talent other professions do not present.

Waiting for Godot? Vladimir and Estragon waited and waited. As you know full well Dr. Tomzack, Godot never arrives. The play was an offering of the theatre of the absurd. Is this the arena in which we as a profession now function? Yes there are “haves and have nots”. The Joshua tree you refer to (actually a plant and not a tree) has branches which include rather complex surgical interventions performed by some podiatrists. Charcot joint reconstructions, deformity corrections with external fixation, distal leg and ankle trauma management are a long way from the DSC days you fondly recall.
 
Our first responsibility is to protect the public and assure that those providing advanced care with significant responsibilities possess adequate training and experience. To a large extent, DPMs are entrusted with the authority to...
