Freischlag, MD has lived a life filled with accomplishments and unexpected turns. During her keynote address at the inaugural Western University of Health Sciences College of Podiatric Medicine Lawrence B. Harkless Dean’s Distinguished Lecture, March 3, she encouraged others to find their own path, become leaders, and make their mark on those they meet.

IL Podiatrist Elected Chair of Podiatric Licensing Board
 
Joseph Borreggine, DPM, a podiatrist from Charleston, Illinois was recently elected to the Chair of the Illinois Podiatric Medical Licensing Board until his term expires in January 2018. His nomination and election to the position occurred at the Illinois Department of Finance and Professional Regulation meeting held in Chicago on March 4, 2016. Dr. Borreggine has served on the board since 2013 and was reappointed in Jan 2015.

PA Podiatrist and Plastic Surgery Use Liposuction to Restore Fat Pads
 
Imagine feeling excruciating pain every time you take a step. For some cyclists and runners, years of devotion to their sport results in something called fat pad atrophy; fat at the balls of the feet wears away, leaving nothing but bone. Now doctors are studying a new procedure to cushion the feet using a patient’s own cells. Podiatrist Beth Gusenoff and plastic surgeon Jeffrey Gusenoff of the University of Pittsburgh Medical Center in Pittsburgh are married and blending their expertise to test a new procedure for foot fat atrophy using liposuction cells from a patient’s abdomen.

Dr. Beth Gusenoff told Ivanhoe, “We’re hopefully cushioning the bone so that when they walk, they aren’t constantly bruising it and banging it against the ground." The doctors explained because they are using a patient’s own cells, it’s unlikely they will have a reaction to it. They said the grafts take about four to six weeks to take effect. Beth and Jeffrey Gusenoff are also starting a second trial to measure the way the fat distributes itself in the foot after transplant.

When you’re out and about, consider treating yourself to a new pair of shoes. Not replacing shoes often enough is the No. 1 cause of crippling overuse injuries (like plantar fasciitis or tendonitis). As they wear out, shoes can provide uneven support—or worse, none at all—leaving your muscles, tissues, and bones more vulnerable to injury.

HIPAA UPDATE
HHS Reinforces Limits on Fees for Patient Medical Records
 
The Office of Civil Rights (OCR) in the Department of Health and Human Services (HHS) has released new guidance about patients' rights to obtain their medical records and how much providers can charge them for copies. The new interpretations, presented in a "frequently asked questions (FAQ)" format, may force some practices to change their policies. OCR's guidance interprets the Privacy Rule of the Health Insurance Portability and Accountability Act (HIPAA). It is OCR's responsibility to enforce the HIPAA Privacy Rule by sanctioning or fining practices.
 
According to OCR, copying fees should be reasonable. They may include the cost of labor for creating and delivering electronic or paper copies; the cost of supplies, including paper and portable media such as CDs or USB drives; and the cost of postage when copies of records are mailed to patients at their request. But OCR specifically stated, "Labor for copying does not include costs associated with reviewing the request for access; or searching for and retrieving the PHI [personal health information], which includes locating and reviewing the PHI in the medical or other record, and segregating or otherwise preparing the PHI that is responsive to the request for copying."

ACA UPDATE
Insurer Cuts Doctor Pay After ACA Losses: Will Others Follow?
 
The move by insurer Highmark to reduce physician rates in health plans offered under the Affordable Care Act (ACA) to make up for massive losses has sparked anger, as well as worries that other insurers will follow suit. Highmark, a Blue Cross and Blue Shield affiliate that operates in Pennsylvania, West Virginia, and Delaware, said it lost $221 million on its health plans in ACA marketplaces, or exchanges, in 2014. It expects to lose another $500 million in 2015 because enrollees have required more care than anticipated. That has caused the insurer to pay more in claims than it collected in premiums.
 
Last month, Highmark announced it would cut provider rates on average in Pennsylvania by 4.5%, effective April 1, to keep these plans viable, rather than shutting them down. Physicians in the insurer's ACA networks in Delaware and West Virginia were spared the axe, at least for the time being.

I have been in the same office location for the last 40 years. I have to move as the current landlord is going to demolish the building and build a bigger one. I am looking for a "project manager" to hire for 6 months to help with the transition and make it as smooth as possible. I have found the new location about 3.5 miles from my current one. Does anyone have any idea of where I could find the project manager?

Kudos to Dr. Mann for posting on "The PROMISE Cohort." I am sure we all regularly see patients presenting with neuropathy symptoms, who upon further questioning, report they were once told they were "pre-diabetic" or "borderline." 
 
Neuropathy in the face of the increasing metabolic dysfunction leading to pre-diabetes is interesting. The pathogenesis appears to be different than that of T1DM. There is an intriguing paper on this topic, "Diabetic Neuropathy: One disease or two?" In this paper, the authors present evidence that "obesity, hypertension, dyslipidemia, inflammation, and insulin resistance are contributory to T2DM diabetic neuropathy to an equal if not greater degree than hyperglycemia." It appears that insulin resistance degrades myelin. 
 
We podiatrists know all-too-well the life-changing pathology that diabetes can cause. In light of this, I believe we are especially well-positioned to speak to pre-diabetics and those with metabolic dysfunction. After all, we are the keepers of the structures directly facilitating movement which is so integral to a healthy metabolism. I believe our profession should be playing a more vocal, proactive public health role in the context of pre-diabetes and metabolic dysfunction in the context of lower extremity health.
