SC Podiatrist Honored with the ACFAS Distinguished Service Award
 
Edwin Blitch, DPM, FACFAS received the 2014 ACFAS Distinguished Service Award at the Annual Scientific Conference in Orlando. This prestigious award—one of the College’s highest honors—is presented annually by the ACFAS Board of Directors to recognize long-term, behind-the-scenes volunteerism to the profession and the College. Blitch becomes the eleventh “unsung hero” to receive this award.

Blitch’s volunteer efforts with the College span nearly two decades, beginning right after he completed his residency. CA Podiatrist Comments on New Possible Breakthrough for DFUs
 
Treatment with polydeoxyribonucleotide (PDRN), an adenosine A2A-receptor agonist, nearly doubled the rate of complete healing of difficult-to-treat Wagner 1 or 2 diabetic foot ulcers compared with placebo, achieved earlier ulcer closure, and resulted in an "impressive" reduction in ulcer area as early as 8 weeks after the start of therapy, a new study indicates. The results are published online ahead of print in the Journal of Clinical Endocrinology & Metabolism .

Lee C. Rogers, DPM, medical director of the Amputation Prevention Center at Sherman Oaks Hospital, Los Angeles, California, noted that the study failed to provide adequate off-loading, the standard of care in such cases. "In a clinical trial, these [offloading] devices are typically provided to patients without charge," he observed. But this critique does not mean PDRN has no potential, he added, rather that future studies would require better adherence to standard of care and adequate off-loading. PDRN "has a unique mechanism of action, and the delivery of the drug avoids some of the pitfalls in other diabetic foot ulcer treatments," he pointed out.

The Patient Protection and Affordable Care Act and Centers for Medicare & Medicaid guidelines and rules resulted in a record number of investigations and enforcement actions in 2011. AZ Podiatrist Lectures at Mexico NIH-AAAS Meeting
 
Dr. David Armstrong lecturing on next-generation diabetes technology at Mexico's National Institute of Medical Science
 
The University of Arizona's Dr. David G. Armstrong, Professor of Surgery and co-Director of the Southern Arizona Limb Salvage Alliance (SALSA) became the first podiatric surgeon to lecture at Mexico's National Institute of Medical Science. The lecture was part of a historic Mexico-NIH-AAAS symposium on next-generation technology-based strategies in the management of diabetes. Armstrong spoke on "Healing and Preventing The Diabetic Wound."

QUERIES (CLINICAL)
Query: Skin Manifestation
 
A 14 year old female presented to our office last week with multiple painful non-pruritic SubQ nodules on both feet that have been bothering her for nearly two months. New ones are popping up every few days. We I&D'd (1cc of purulence) and cultured the one on the right forefoot, but the culture came back negative (no growth). A week of Augmentin has not helped. 
Skin Manifestation
 
She is a thin, very light skinned girl with no systemic complaints and with normal vitals. She has moderate edema in both feet, which  are very cool with mottled skin. She takes no medications and there have been no changes in diet, activity, or footwear. She does have a dog and a cat, but they are outdoor animals only. Any suggestions?

I had an exact same type of patient presentation with the exact same MRI two years ago. You'll never forget it when you see it. It was an aggressive Ewing's sarcoma in an otherwise healthy 21 year old female. Do not bother with bone biopsies. Get this patient to an orthopedic oncologist ASAP. You may save a leg, if not a life. Update us on what you find.

This appears to be a case of macrodactyly. Several years ago, I had the opportunity to perform reconstructive surgery for this deformity in a teenager. It was done in stages and was very successful, making a huge difference in the patient's self-image and social interactions—not to mention his footgear.

This was not repaired at an earlier age, not because of a medical reason but due to family financial issues.The most amazing aspect of this case was not the surgery itself, but the effect that having a relatively normal toe had on this young man’s personality. I was fortunate enough to have some training in this technique thanks to Larry Oloff, DPM. Otherwise, I would have referred this to a surgeon with experience in this type of delicate reconstructive technique.

I think that Dr. Grapel overreacted. I think the patient should have asked if it is okay first, but I can see a patient being afraid that they will be too nervous in a pre-op consult to remember everything. I wouldn't take it as an attempt to trap you. I would welcome it and maybe ask for a copy, as it proves that you did the informed consent correctly. It might not be a bad idea for you to record informed consents.
