TX Podiatrist Puts Emphasis on Prevention, Not Amputation
 
“The lifestyle in the Valley is high-work, low-income,” said Oscar Corral, DPM, a podiatrist in McAllen. Dealing with devastating wounds is routine, because caring for oneself often takes a backseat to looking after family and putting food on the table.

Dr. Corral recently stopped doing amputations because it was too upsetting. He focuses now on preventive foot care. “Everybody’s big on the amputations, but not big on the preventive care part. They’re so busy doing hospital consults on patients that already need an amputation...  They don’t have time to do the preventive care.”

Syeda Roshan, DPM has joined FHN Family Healthcare Center. Roshan is a graduate of the Dr. William M. Scholl College of Podiatric Medicine in North Chicago. She completed her residency in podiatric medicine and surgery at Sacred Heart Hospital and Swedish Covenant Hospital in Chicago, where she earned an added credential in reconstructive rearfoot and ankle surgery.

NY Podiatrist  Promotes Proprioceptive Insoles to NFL
 
One of the many people courting the NFL and the players at the NFL Combine is Emily Splichal, DPM, a Manhattan-based podiatrist and human movement specialist. Splichal is the founder of Naboso Technology, a two-year-old company that makes textured shoe insoles and training mats that claim to help improve athletic performance, reduce the risk of injuries, and improve injury rehabilitation. The Naboso insole isn’t like other insoles on the market. It has a unique textural design that features a series of little raised pyramids that are spaced to the specificity of a nerve on the bottom of the foot. “That nerve on the bottom of the foot is the same nerve that helps blind people read Braille," Dr. Splichal said. “We are essentially stimulating the same nerve, but in the feet."

Unlike other insoles, Naboso’s doesn’t have an arch. “I try to get them to buy into the idea of an insole that doesn’t have an arch in it," Splichal said. “It’s counter to what people think. But it’s approaching movement from a slightly different perspective." “Every [other] insole that is on the market is designed to approach function or foot pain or foot support from a bio-mechanical perspective, which means there’s arches put in them. “Our insoles are the only one on the market that are based solely on sensory stimulation with respect to foot function, and really, to total body movement and motor control."

7. Develop an incident response plan - Despite best efforts, breaches will occur. Identifying, responding to, and containing a breach quickly can significantly reduce the cost and impact of a breach. An incident response plan (IRP) establishes a framework for detecting, acting upon, and limiting the effects of a data security breach. An IRP typically addresses who within an organization is responsible for investigating a security incident, what resources (technical, forensic, legal, public relations, etc.) are available, and enumerates the incident assessment, countermeasures, and corrective and notification actions to be taken.

With all due respect to Dr. Rettig, I would challenge him and others performing bunionectomy procedures to critically look at the long-term outcomes of their work. As I said in my post on the subject, I agree that patient satisfaction is acceptable in “most” cases after distal osteotomy. It doesn’t change the fact that we take a straight bone and make it crooked with a distal osteotomy. Yes, it’s technically much easier, and yes it’s easier on the patient post-operatively. I was simply stating that long-term, it doesn’t hold up to a 1st metatarsal cuneiform arthrodesis. 
 
It doesn’t mean I don’t perform distal osteotomies. I just don’t push them to their limit because it’s an “easier” procedure. With regard to Dr. Rettig’s comment on counseling his patients, I don’t agree in “trade-offs” and “allowing my patients to choose” which procedure to use on them. They come to you as a specialist to make that decision. Otherwise, thank you Dr. Rettig for commenting on my post.

“Surgery is an art” as my colleague Dr. Lowell Weil Sr. explained in our academic books. I'm one to agree that the surgery we perform on our patients, albeit the need to meet book guidelines, is to meet the quality of aesthetics and functionality. After performing surgeries over 25 years, I have seen angles that are very high that were corrected with distal osteotomies, and because there was improvement over what the patient had prior, clinical improvement in most cases was the important factor over radiographic concerns.
