Don't Waste Money Buying Orthotics in Stores: NC Podiatrist
According to Elizabeth Daughtry, DPM, “Most shoes don’t have enough arch support for the average person, so additional arch support or cushioning may be necessary if you’re standing or if you walk for long hours or long distances. Plus, arch supports don’t need to set you back a lot of money. In general, over-the-counter inserts should only cost about $50 to $70.”

She also said you don’t need to waste money buying orthotics in stores, but rather “consult a qualified professional if you want a true custom-molded orthotic, where a mold of the feet is captured without gravity or body weight (no standing position) into a box or on a plate to capture the image of your feet. The inserts are then made in the laboratory to be specific to each individual’s foot.” However, Dr. Daughtry warned: “Be careful of excessive padding in the sole of the shoe, as this can lead to balance problems for people with neuropathy or at risk of falling.

Foot and Ankle Care by Podiatrists in the 3 Months Before DFU Diagnosis
This study revealed substantial rates of mortality (44.6%) and major amputation (28.3%) in this high-risk population. Of the cohort, 18.4% (n = 2736) received foot and ankle care by podiatrists within 3 months before the index DFU diagnosis. Patients who received care by podiatrists were more likely to be male, to be older, of White race, receiving pre–kidney failure nephrology care, in need of assistance with daily activities, and institutionalized compared with those who did not. They also had higher Charlson Co-morbidity Index scores, had a longer duration of dialysis dependency, a greater likelihood of receiving peritoneal dialysis, and extended hospital stay.
In this comprehensive analysis of a national cohort comprising Medicare beneficiaries with kidney failure and DFUs, the receipt of foot and ankle care provided by podiatrists within the 3 months preceding a DFU diagnosis was associated with reduction in the risks of major amputation and death among patients with kidney failure who were receiving dialysis. While future fully powered studies are needed to further support these findings, these results suggest positive potential benefits for preventive foot and ankle care to mitigate complications in this high-risk population.

The implementation of these billing tips can be a transformative step towards improving the financial health of your primary care practice.
6. Proper secondary insurance Filing
Understand the proper procedures for filing claims with secondary insurance providers. Make sure you know the coordination of benefits (COB) and which insurance is the primary one. Claims can get stuck in COB where both insurances are claiming they aren’t primary. Often, the patient may not even be entirely clear as to which is the primary. In those cases, it is important that the patient contacts their insurance payers and verifies the primary vs. secondary insurance.
Furthermore, secondary payers often require physical documentation of the primary rejecting the claim first and this physical explanation of benefits documentation must be mailed along with the claim. A patient with access to multiple insurance plans is ultimately a benefit to the patient and provider but does require extra work to properly file claims.

RE: PEMF Treatment of a Toe Fracture

Pulsed electromagnetic therapy (PEMF) had been used since the mid-1970s as a beneficial tool for fracture. In the last decade, more powerful units have been developed to help not only fractures but neuropathy, arthritis, and tendon problems. Recently, VA hospitals added PEMF units as part of their protocols to treat neuropathy. Treatment in the office offers the benefit of immediate care. Insurance pays for wearable electromagnetic or ultrasound units once there has been adequate documentation of an at-risk anatomic area or delayed union. Having an in-office option can benefit our patients and our practices.

I have used Nextgen for 10 years until I recently was forced to change to a hospital-based EMR. Nextgen was great for my single doctor practice. It was fully customizable at each level of charting with the ability for the user to change/create whatever was needed very easily. You could create letters to docs, merge patient data into them, and save the original template. You can create favorite tabs for diagnoses, procedures, care plans, etc. that include the CPT code modifiers, macros; and it puts it into the billing module. The eRx module allows creation of favorite meds AND multiple favorite sigs (e.g. BID) for that medicine.
These features eliminate much of the typing and clicks needed to fill the chart, but still allow for alteration. As with any system, it requires work to set it up the way you work. This takes a few months or hair pulling, But once done, it is possible to create unique charting in minutes and be done. My front office had no complaints about the billing module .We sent out all claims at the end of the day. Their clearinghouse was fine. You need to spend some time with each system and try to use it yourself. Don’t just allow them to “show how quickly you can work” in a system they have already set up.

RE: A Short History of Podiatric Discontent and Frustration

A significant portion of the older podiatric profession are the notional progeny of chiropodists. For many of us, the first exposure to our future profession was afforded by individuals with the letters DSC behind their names. They reduced calluses, managed toenails, and applied pads to the toes and feet. There was often a preliminary foot whirlpool to soften calluses and cleanse feet. Plus it felt good to the patient. DSC stood for Doctor of Surgical Chiropody, the term surgical merely decorative. Scalpels were used parallel to the skin to remove calluses. Pads protected friction points and unloaded pressure sites.
Merton Root showed up on the chiropody scene after WW II and podiatry began to evolve with the advent of biomechanics. Biomechanical orthotics dispersed stress points and took the place of padding, making padding somewhat obsolete. Biomechanics evolved into podiatric orthopedics and maybe primary care, but real biomechanics experts discussed into the early morning hours how...
