CA Podiatrist Discusses Treatment of "Turf Toe"
 
“Turf toe” could be a few different types of injuries - not just one specific condition or anatomical structure. The primary method of prevention is use of stiffer shoes/cleats and taping (particularly for sports where one is barefoot)," says Amol Saxena, DPM.

"Most turf toe injuries can be treated without surgery. Mild sprains usually need some relative rest, ice, and stiffer shoes and/or insoles. Oftentimes, a specific foot insert (orthosis) or turf toe insole is used to help an athlete return to play. These keep the big toe from bending too much. Sometimes, a cleat is directly under a sesamoid; removing the sesamoid can be helpful. Moderate cases usually need a boot as well as non-weight-bearing," says Dr. Saxena.

Softer Shoes Can Impact Balance: FL Podiatrist
 
"Shoes with thick foam liners or soles can often effectively provide enough cushion for the foot to ease the pain of walking or standing for many. Shoes with thin fabric uppers with fewer stitched seams can often adequately provide the stretch needed to accommodate bunions and hammertoe contractures for many. These softer-soled or cushion shoes decrease what is known as the proprioception in our feet," says podiatrist David Raynor, DPM.

"One risk of proprioception compromise is falling, which can be potentially devastating. Many times, a loss of proprioception is not noticed as its progression is so gradual. EMG/NCV testing by a neurologist can objectively confirm compromise to the nervous system. Self-evaluation, observation by family/friends, or a health/gait examination can also indicate balance issues," says Dr. Raynor.

Fenofibrate Reduces Uric Acid Concentrations, Gout in T2DM
 
Fenofibrate reduces uric acid concentrations by 20% and cuts the number of gout events by nearly half over a 5-year period in patients with type 2 diabetes (T2DM). This is the first long-term evidence that fenofibrate can reduce gout events over the course of a 5-year period.
 
This was a post hoc analysis of the double-blind randomized controlled FIELD trial evaluating fenofibrate (200 mg/day; n=4895) vs. placebo (n=4900) for 5 years to prevent gout in T2DM. For all gout events during 5 years, fenofibrate cuts the risk by almost half (HR, 0.48; P<.0001).

4 Areas for Practices to Focus on in 2018 - Part 4
 
In working with a variety of providers on solutions to address today’s biggest market pressures, we’ve identified four key areas in which practices should invest time and resources to be successful: 
 
4.  In-Office Dispensing
 
Patient care and outcomes are optimized when prescriptions are as close to the physician as possible. In-office dispensing, which can include either a physician dispensing program or a retail pharmacy, enables providers to play a greater role in the pharmaceutical care of their patients. Practices looking to implement an in-office dispensing program need to be ready for the increased regulatory requirements to support the program and ensure they are properly structured to take on the additional workflow and responsibilities of dispensing.
 
Having an in-office dispensing program where clinical staff is part of the care team allows the practice to better manage the patient. If both the patient and the prescription are close to the provider, it has a positive impact on patient outcomes and provider measurement under a value-based care model. While the challenges that practices face this year and beyond are formidable, by creating smart solutions and strong partnerships, providers can come out on top. Focus on inventory management, MACRA, clinical research and physician dispensing – or find a partner who can help you address these four opportunities – and you, too, will soon find ways to expand offerings and remain financially viable while also achieving the ultimate goal of providing patients with the highest quality care.

Query: Rapid Onset of Sweaty Feet in a Diabetic
 
I had a patient come in this week who is in his mid-twenties. He has been an insulin-dependent diabetic since age 5. He's about 80 pounds overweight. His chief complaint is sweaty, smelly feet that started only a month ago. My usual first line of treatment is to use an over-the-counter antiperspirant spray on his feet. This did no good. 
 
He changes his shoes frequently so I don't see that this is caused by any pair of shoes. Has anyone heard of a hormonal aspect to hyperhydrosis?

Query: Inexpensive Program for ePrescribing
 
Currently, I use Practice Fusion only for electronic prescribing. As we all know, they are moving to a paid subscription model on June 1, priced at $99 per month, and with a yearly commitment. This seems a bit much for just electronic prescribing. Can anyone recommend a more reasonable (or free) software program for just electronic prescribing?

Regarding the paucity of research in podiatric medicine and surgery. PLEASE. Let us call it like it is. It is all about money. It is all about funding (or lack thereof). Unlike other allopathic medical sciences, there is no funding for research in podiatry. As someone who pursued research with an otherwise well-funded residency program for 10 years, I can confidently say that there is minimal funding for research in our profession. Neither the APMA nor ACFAS provides adequate funds. We are our own worst enemy. After 10 years of vigorous effort with minimal support or reward, I gave up. Why would I expect more from anyone else?
