PA Podiatrist Discusses Causes of Bunions
 
A bunion is a bony bump that sticks out from the side of your foot at the base of the big toe. The word bunion comes from the Greek word for turnip – perhaps because a bunion can look swollen and reddish. “The common wisdom is that women get bunions because they spend too much time in narrow-toed high heels,” says Brian Stahl, DPM, a podiatrist and foot and ankle surgeon with LVPG Podiatry–Hecktown Oaks.

“Tight shoes can certainly be a contributing factor,” he says, “but bunions can be caused by anything that puts extra pressure on your big toe joint, such as the way you walk, standing for a long time, and even some health conditions.” Heredity or previous foot injuries also can be a factor, Dr. Stahl says.

FL Podiatrist Publishes Case Series on Nutritional Approach to Long COVID Brain Fog
 
Richard H. Mann, DPM, Chief Scientific Officer of Realm Labs, has published a peer-reviewed case series examining a nutritional approach to cognitive symptoms associated with Long COVID. The article, titled “Improvement in Long COVID Brain Fog After Benfotiamine and Methylcobalamin Supplementation: Case Report Series and Review of the Literature,” appears in the Medical Research Archives, the Journal of the European Society of Medicine (Volume 14, Issue 2, February 2026).

The publication describes three patients with persistent cognitive symptoms following COVID-19 infection who received oral supplementation with benfotiamine (a derivative of vitamin B1) and methylcobalamin (vitamin B12). In these cases, patients reported improvements in cognitive clarity within approximately one month.

3. Prune what doesn’t matter to invest more in what does
 
There is a saying: “You have to keep pruning the rosebush if you want to create beautiful blooms”. It’s easy to keep adding new features (e.g. services, technologies) to your practice, and adding new projects for your practice to work on. What’s more powerful is to trim away the bloat and excess, and only focus your resources on the small number of things that really matter in pursuit of your strategy. The key to effective strategic leadership is to figure out what is truly "core", vs what is "context" and "non-core" in terms of your activities and service/technology offerings.

2.  Set a Timer
 
Back in undergrad and podiatry school, I swore by the Pomodoro Technique. For anyone unfamiliar, it’s a structured method that uses a timer to break work into 25-minute intervals, followed by a 5-minute break. After four “Pomodoros,” you earn a longer 15-30-minute break—the productivity version of a water station at mile 101. This method was the most effective way for me to study, especially when I rotated subjects every few sessions to avoid mental burnout.
 
I still rely on timers to manage my workflow. Once I’ve made my list and set my priorities, I rotate through my top tasks in short, focused bursts. It helps me stay engaged and prevents me from getting stuck on one project for too long. That said, everyone has their own rhythm. Some people prefer to tackle one task start to finish before moving on.

RE: New Supplier Enrollment Moratorium

Last week, President Trump announced a new war against fraud during his State of The Union Address. It did not take long after that announcement for the hammer to come down against DME fraud. In a rather stunning announcement, CMS effectively blocked new supplier enrollment for 7 categories of medical suppliers for at least 6 months. Physicians (which includes DPM, DO, DPM, and optometrists) as well as other professional service providers (e.g. pharmacies, orthotists, prosthetists, and pedorthists) are exempt from this moratorium. This bears repeating, physicians, including all categories of physicians, are exempt from this moratorium.
 
The moratorium will affect medical supply companies that have physicians as their CEO, Medical Director, CFO, etc. Medical supply companies have their principal business supplying DME. Such IS NOT the case for physicians whose main job descriptions are medical/surgical patient care. In other words, the moratorium does not affect physicians whose main business is not providing DME, but who are providing DME as an ancillary service to their professional medical practice. Thus, this moratorium will have little, if any, negative impact on physicians who wish to add a practice location or have no DME license and wish to enroll as a DME provider. In fact, this moratorium may free up personnel at the DME enrollment carriers and allow them to process professional enrollees much more expeditiously. The moratorium is in place from 2/27/26 through at least 8/27/26.
 
Something had to be done about the amount of DME fraud. While this may impact many DME supply companies who wish to add an additional location or block new honest practitioners from entering the market—unfortunately, they are the collateral damage as have been us, the taxpayers who fund the Social Security and Medicare Trust Funds. Hopefully, this moratorium will work!

It was with a heavy heart that I read the ordeal you experienced at the hands of the corrupt FBI/DOJ, for I was also threatened by them in 1999. Congratulations on fighting and not surrendering. 
 
I won't go into detail, but they tried, and did, go after several doctors in the Tampa Bay area for obscure violations of the Stark law. This was conducted under the Clinton/Janet Reno tenure. The prosecutor they appointed to Tampa Bay declared openly at a legal conference that his office would go after about 300 doctors. He further stated that 100 would be dropped, 100 would plead, and 100 would be incarcerated. I was fortunate to have had very good documentation and was not pursued. However, others were not so fortunate and their lives were changed forever. 
 
The irony here is that while the FBI was busy investigating honest, dedicated doctors, some of the 9/11 terrorists were learning to fly planes out of the Venice, FL airport.

It is with great sadness that I learned of the passing of Don Chantiles, DPM, from Yorktown, PA. Don was not only a respected individual but also a compassionate physician, deeply committed to his practice and his patients. I had the privilege of visiting Don at his office, where I witnessed firsthand the gentle and professional connection he had with his patients. He made sure that each one felt heard and well cared for.
 
Don also fostered an admirable and respectful bond with his staff, creating a positive and collaborative work environment. His genuine, caring nature was evident in every aspect of his practice. Throughout my years and experiences in the podiatry profession, I had the privilege of making many valued friendships. Don was truly one of the special ones. His good nature and kindness will always be remembered and cherished by those who were lucky enough to know him. My deepest sympathies to his family.

The World's Oldest Prosthetic is a Big Wooden Toe
 
Likely the world's oldest prosthetic device, an artificial wooden toe was found on a 3,000-year-old mummy of an Egyptian woman in 1997. Now, with the advancement of microscopy, x-ray technology, and computer tomography, researchers from the University of Basel in Switzerland have learned a great deal about how the toe functioned, what it's made of, and how it was fabricated.

Similar to today’s prostheses, it appears to be precisely tailored to the woman who wore it - having been refitted to her foot several times. The toe consists of three sections - two of wood and one of leather - and likely aided her ability to walk.
