PODIATRISTS AND THE WAR IN UKRAINE
MD Podiatrist Plans to Donate Equipment to Ukraine

Gene Hudyma, DPM, a first-generation Ukrainian-American and retired podiatrist, is planning to donate equipment to Ukraine from his former medical practice in Glen Burnie. He said he has to do something to help the people of Ukraine because their struggle reminds him of what his relatives went through 80 years ago to escape the conflict of World War II.

Dr. Eugene Hudyma

“We are children of people who were refugees in World War II,” said Hudyma, 66, who now lives in Millersville. “Between the Nazis and the Russians, they were caught between a rock and a hard place, and they had to survive in very much the same story that’s going on in Ukraine today. We’re looking to get together some supplies to hopefully send a container to Ukraine and find the right people who can get it into their hands because they’re going to need all the help they can get,” he said.

TX Podiatrist Promotes Disc Golf

A new disc golf course under construction at Lake Nacogdoches is expected to draw players from across the state once it is completed. Eager to see results from years of planning, members of the Nacogdoches Crushers Disc Golf Association spent a recent Saturday on a test run, pausing to clear fallen limbs, move portable goals, and point out remnants from its history as an archery range.

Dr. Mark Sowell

“A fourth course in Nacogdoches will increase capacity for tournament play, provide more options for play, and use a part of the park that has not been used in years,” says Dr. Mark Sowell, a podiatrist and member of the Crushers who serves on the city’s advisory board for parks and recreation. “Depending on how we schedule play next year, the new course could increase capacity by 80 players in one event.”

PRACTICE MANAGEMENT TIP OF THE DAY
5 Mistakes to Avoid When Texting Patients

Some of the highest-quality healthcare strategies nowadays include remote components and patients are increasingly wanting to communicate with their providers via text messages. While texting can make things a lot easier (and more profitable), it can also be easy to unknowingly violate HIPAA regulations. Be sure to not make these five common mistakes.

2. Texting Non-opt-in contacts

Before texting a patient, you need to make sure that they’ve given their consent to being texted by you. Texting patients who haven’t consented to text message communication can be a major violation of HIPAA standards, not to mention other regulations set by the Federal Communications Commission (FCC). So how do you get patients to opt-in? It’s easier than most people think. You can start by encouraging inbound traffic. Prompt patients to text you first, which you can do from your website. For example, use an SMS Chat on your homepage, or say “text us at [phone number]”.

Another way to get patients to opt-in is just to simply ask them. On your web form, include a disclaimer that providing contact info gives you the right to communicate with them through those channels for an appointment and care-related communications. Add the same checkbox to any patient paperwork, and you’ll be surprised how quickly your opt-in list grows. Patients also need to be able to opt-out of communications at any time. This holds true for text, but also for any other kind of communication. Most text systems include some sort of opt-out message or function.

CODINGLINE CORNER
Query: Prescribing Oral Antifungals

I order liver enzymes prior to prescribing an oral antifungal. The blood work is a part of the risk and complications to determine the level of E/M. Can I also include the prescribing of the oral antifungal as part of risk and complications while waiting for the lab results?

Jeff Leibovitz, DPM, Indianapolis, IN

Response: Whether it is a new patient or an established patient for this specific problem, onychomycosis for this patient is to be treated via an oral antifungal medication. When you look at the Level of Medical Decision Making Table, 2 out of the 3 Elements of Medical Decision Making need to be met. Under “Number and Complexity of Problems Addressed,” onychomycosis in all likelihood would be classified as “1 stable chronic illness.” None of the criteria are met under “Amount and/or Complexity of Data to be Reviewed and Analyzed.” Since lab work is being ordered, specifically liver enzymes, under “Risk of Complications and/or Morbidity or Mortality of Patient Management,” there is a low risk of morbidity from additional diagnostic testing or treatment.” This would translate to a CPT 99203 for a new patient/initial encounter or a CPT 99213 for an established patient encounter.

The patient is rescheduled to return after the lab results are reported in order to determine whether or not this patient can be started on the oral antifungal medication. When you look at the Level of Medical Decision making Table, 2 out of the 3 Elements of Medical Decision Making need to be met. Under “Amount and/or Complexity of Data to be Reviewed and Analyzed.” it makes perfect sense that there needs to be an “Independent interpretation of tests” by the ordering physician. An independent interpretation of test performed by another physician/other qualified health care professional (not separately reported) would certainly be appropriate.

This would entail the review/interpretation of the liver enzymes that were performed by an outside laboratory. Under “Risk of Complications and/or Morbidity or Mortality of Patient Management,” by starting this patient on the appropriate regimen of oral antifungal medication, wouldn’t this be a moderate risk of morbidity from additional diagnostic testing or treatment? Of course it would. This is an example of “Prescription drug management.” This subsequent encounter would qualify for an established patient level 4 E/M service, CPT 99214.

PODIATRISTS AND THE CORONAVIRUS PANDEMIC
RE: Effects of COVID on Two Podiatrists
From: Robert Spalding, DPM

Did anyone read about the passing of Dr. Dennis Bizzoco who practiced in Chattanooga, Tennessee? Apparently Dr. Bizzoco died from the results of COVID (but not fully confirmed) in October of 2021. I must have missed the local obituary and was only notified about his passing from one of his local long standing patients who I knew well in my youth who sought me out to replace Dr. Bizzoco for foot care.

Dennis was 68 and only 3 years older than me. Dr. Bizzocco had been involved in podiatry long before I opened my practice. Dr. Bizzoco with the Tennessee Podiatry Board approved my entry into practicing podiatry in my state in...

RESPONSES/COMMENTS (ACFAS NEWS)
From: Paul Taylor, DPM, Peter J. Bregman, DPM

I remember Scott Malay visiting my office before he started podiatry school.  This is an example why we need to encourage young people to enter our profession; seeing someone who has become an outstanding member of our podiatry community.

Paul Taylor, DPM, Silver Spring, MD

Dr. Malay started out as my professor and I knew then he was the real deal. We then became friends and colleagues. Scot is the epitome of what this award represents and should be proud that he was recognized for his achievements and dedication to the profession, and all of the research he has either fostered or participated in. Well deserved!
