TX Podiatrist Part of Hospital's Amputation Prevention Team
 
Memorial Northeast Hospital has set its sights on stopping a growing trend by using expert, collaborative care to prevent amputations and save lives. Amputation surgeries performed in Texas increased 15 percent from 2007 to 2012, according to data published by the Amputee Coalition. More than 86 percent of Texans who underwent amputation surgery in 2012 were 45 years or older, the age group most affected by diabetes and peripheral arterial disease (PAD), which play a role in 54 percent of all amputation surgeries.

“Loss of limb is a devastating event in a patient’s life,” said Matthew Sheedy, DPM, a podiatrist and surgeon affiliated with Memorial Hermann Northeast. “Recovering, if possible, can be extremely challenging physically and emotionally. Our goal is to prevent amputations by working closely with each other and intervening as early as we can.”

AZ Podiatrist Seeks Cuban Drug to Treat DFUs 
 
Two University of Arizona physicians want to bring a prescription drug developed in Cuba to the United States to treat diabetic foot ulcers. Such ulcers can lead to death in some diabetics. UA’s Drs. David Armstrong and Marvin Slepian recently traveled to Cuba to partner with medical colleagues to bring the drug called Heberprot-P to the United States. They hope the normalization of relations between the two countries will help speed clinical trials and lead to approval by the Food and Drug Administration.

The drug is used in 27 countries. It fosters rapid wound healing and a 75 percent reduction in amputations, the physicians say. Dr. Armstrong said it costs more to treat severe diabetic wounds than the five leading cancers in the U.S. Later this month, Armstrong will speak to the Congressional Caucus on Diabetes about Heberprot-P and other research at the Southern Arizona Limb Salvage Alliance at the UA.

AAPPM Starts 2016 with “Best Event in Years”
 
Earlier this year, the leadership of the American Academy of Podiatric Practice Management (AAPPM) unveiled its new strategic plan which included ideas to reform and greatly improve its conferences. Based upon the results from the 2016 Midwinter Conference, which took place last weekend, the AAPPM clearly is off to a great start. According to one participant, a longtime member and frequent attendee of AAPPM events, “this weekend was one of the best Academy meetings I have been to in years!”

“I am so pleased that the changes we made to this year’s Midwinter Conference were so well received,” said AAPPM Conference Chairman Andrew Schneider, DPM. “We had to accommodate overflow seating in seminars, we enjoyed a great venue, we returned to interactive workshops, and we accomplished a lot as we were providing more practice management ideas our members expect from the AAPPM.”

Podiatrists Well Represented at Council of Nail Disorders Meeting
 
Recently in Washington, DC, a group of the leading dermatologists and podiatrists gathered together to share knowledge and research on the more advanced diagnosis and treatment of nail pathology. For the 20th year, the Council for Nail Disorders met the day before the American Academy of Dermatology meeting.

Bryan Markinson, DPM, the first podiatric board member of the Council, was elated to see the room full to hear some incredible lectures on nail pathology. Tracey Vlahovic, DPM presented two outstanding lectures that were well received. Joel Morse DPM, president of American Society of Foot & Ankle Dermatology said, "the highlights from this event will be synthesized and re-delivered in a podiatric format at the DermFoot.com seminar in April.

Query: Diabetic Charcot Joint ICD-10 Coding
 
Our patient has diabetes with neuropathy (E11.42) and has developed a Charcot joint. When you look up Charcot joint, ankle and foot, you get the following codes depending upon laterality:  M14.671 right M14.672 left. But this code has an Excludes1 which is Charcot joint in diabetes mellitus. (E08-E13 with .610). I would probably choose the E13.610, other specified diabetes mellitus with diabetic neuropathic arthropathy. This has an inclusion term of other specified diabetes mellitus with Charcot joints. Do you bill with E13.610 and E11.42 which doesn't really specify Charcot, or do you bill with M14.671 (or 2) and E11.42? This then documents which foot and specifies the Charcot. If I understand the coding book, I'm not supposed to use the M14.671 and the E13.610 together.

Response: If you have a Type 2 diabetic (E11-) patient, they CANNOT be an E13. The key with ICD-10 is that it provides additional options (co-morbidities) for you to apply to the patient's conditions. In your example, you already have the diagnosis of E11.42 (Type 2 diabetes mellitus with diabetic polyneuropathy). If they developed a Charcot joint, look at the same series, E11- for the specific code. There you will find E11.610 (Type 2 diabetes mellitus with diabetic neuropathic arthropathy; Inclusion Term: Type 2 diabetes mellitus with Charcot joints). 
 
Your statement about M14.6 (Charcot joint, Excludes1: Charcot joint in diabetes mellitus [E08-E13 with .610]) is correct. You DO NOT use the musculoskeletal coding because of the Excludes1 rule.

Query: Grand Rounds Second Opinion Service
 
Last year, I performed a posterior tibial tendon reconstruction on a patient who had recalcitrant posterior tibial tendonitis associated with a flat foot (PTTD). Prior to the surgery, she exhausted PT and was non-weight-bearing for episodic periods. When I initially saw her, she was in peroneal spasm but after a common peroneal block, the STJ had about 30 degrees of motion. I visualized this with a mini “C” arm. 
 
I ultimately did a PTT advancement and used a tendon anchor to re-attach the tendon. Prior to the surgery, we discussed the options of flat foot reconstruction vs. repair the tendon and use a brace or orthoses. We opted for a tendon reconstruction to avoid joint fusions at this time (30 y/o). The patient had some post-operative edema and went to PT for several months where STJ ROM was again documented as inversion of 18 degrees and eversion of 8 degrees.

RE: Pre-diabetes and Peripheral Neuropathy: The Promise Cohort Study

A recent study, Peripheral Neuropathy and Nerve Dysfunction in Individuals at High Risk for Type 2 Diabetes: The PROMISE Cohort, Diabetes Care 2015;38:793–800, from a team of Canadian researchers, found that the prevalence of peripheral neuropathy in newly diagnosed diabetics and pre-diabetics was virtually the same, 50% and 49% respectively. They concluded that pre-diabetes is an independent risk factor for peripheral neuropathy and that peripheral neuropathy begins in the early stages of diabetic pathogenesis.
 
The study confirms that risk factors for diabetic neuropathy, such as age, height, and glycemia, are associated with peripheral neuropathy, even in the absence of a clinical diagnosis of diabetes. They conclude that these results support efforts toward early detection and intervention as a means to reduce the incidence of peripheral neuropathy and its downstream consequences in high-risk populations.
 
Those of my colleagues treating patients with a polyneuropathy of unknown etiology would do well to consider these findings in their clinical work-up and evaluation of these patients.

It isn’t just diabetic shoes you can submit for. Don’t let your DMEPOS billing privileges expire, as they include other DME, like fracture walkers, etc. These fall under the same category. I just went through the re-activation process because I too got dropped for inactivity. I was billing, but NEVER getting paid because of problems with our software agent not getting the bill to the DEPMOS carrier... or so DEPMOS said! The billing company said that bills were being sent.
 
We have been calling Medicare DEPMOS and faxing, etc. for over a year to get paid on a couple of fracture walkers. We are getting the runaround and then being informed that we were told we were de-activated for lack of billing in a calendar year. Talk about frustration. So, I filled out the 855 forms and got re-activated, but still haven’t gotten paid for the fracture walkers that I dispensed to patients last year. So, don’t let your privileges expire, and check on them regularly if your billers aren’t.
