IA Podiatrist Uses Toe and Flow Collaboration to Save Patient's Toe 
 
About six months ago, Anna Edwards broke the little toe on her left foot. No matter what the 51-year-old Sioux City woman did, she couldn't seem to get it to heal. Her foot felt cold, went numb, turned a bluish color and then black. After a visit to her family doctor, Edwards was referred to Dr. Kosta Antonopoulos, a podiatrist with UnityPoint Clinic Podiatry, who examined her foot and discovered that it wasn't getting any blood flow. "I couldn't feel her pulses in her foot," Antonopoulos said. "My antenna went up right away that no matter what I did, I wasn't going to be able to heal this toe ."

Antonopoulos called Dr. Wade Kang, a vascular surgeon at UnityPoint Clinic, to express his concern about Edward's toe. The risk of amputation, he said, was high as Edwards was a smoker since age 8. Kang ended up implanting a small stainless steel tube called a stent into Edwards' artery to open it up. Antonopoulos refers to his frequent collaboration with Kang as a "flow and toe" model of care. After Kang restores blood flow, Antonopoulos can treat ischemic changes and ulcers. With blood once again reaching Edward's foot, Antonopoulos said her toe began regaining its color. Six weeks later, after some local wound care, he said it was nearly healed.

Women Runners Prone to Neuromas: AZ Podiatrist
 
According to Scottsdale foot and ankle surgeon and ACFAS Annual Scientific conference presenter Kris DiNucci DPM, FACFAS, active women who enjoy running are prone to neuromas, especially if they regularly wear narrow shoes and have flat feet. Dr. DiNucci, who is a Fellow member of the American College of Foot and Ankle Surgeons, says the symptoms begin gradually and, if left untreated, progressively worsen. Relief for the symptoms may come by massaging the foot, wearing wider everyday shoes with low heels, and avoiding running and other activities that aggravate the condition.

"Over the past few years," Dr. DiNucci explains, "there have been many advances improving the outcomes of foot and ankle surgery. In the treatment of interdigital neuromas, many new techniques have been developed from in-office procedures to minimally invasive techniques to allow our patients to return to their running pain-free in the shortest timeframe."

NY Podiatrist Receives Freedom  Award 

Rockland County honored area podiatrist and former U.S. Army Medical Service Corps captain, Dr. Katherine Ward with its annual Freedom Award. The award is given out each year to female veterans who have made a demonstrable impact on the local community. County Executive Edwin Day, who presented the honor to Ward, explained that women often share the same physical and emotional burdens of war as their male counterparts on the front line.

Ward received a podiatry degree from the New York College of Podiatric Medicine and completed her surgical residency at Catholic Medical Center in Queens before training at Walter Reed Army Medical Center, serving as the chief of podiatry at Walter Reed and Fort Hood and as a captain in the Army during the Gulf War, and in Somalia and Bosnia.

This 18 month old is tripping and having difficulty walking. My current thought is to get the child a special shoe with proper accommodation, and after skeletal maturity, do the reconstruction. Any other thoughts are welcome.

Query: Video Recording By Patients 
 
I am getting accustomed to having patients use their smart phones in the treatment room to photograph x-rays or their own feet, particularly at the first post-operative visit. However, last week, I had a new experience which I was unprepared for. A 62 year old female was being consented for revisional Achilles surgery after her first surgery failed due to non-compliance in keeping the operated extremity non-weight-bearing during the post-operative period. The patient asked if her 30 year old daughter could be in the room while I explained how the complication occurred, and what surgery would be required to fix the problem. I complied, and as I began my explanations, the daughter took out her phone and began video recording the entire encounter. My gut reaction was to ask her to stop, but better judgment took over, and I allowed her to continue. I am curious what others would do in this situation?

Interestingly enough, I am seeing a greater number of patients with symptomatic Raynaud’s and pernio. My treatment of choice in addition to the usual warm soaks, avoiding caffeine, and warm socks is Nitro-Paste 0.2% (topical nitroglycerine). I also use a Catapres TTS patch for more localized areas.

We are seeing more cases of pernio (chilblains) in the Northeast this year because of the cold, damp weather which is mimicking cold damp weather in parts of England, where chilblains are more common. I have had success in treating this condition with dihydropyridine calcium channel blockers such as nifedipine XL, combined with recommending 8-hour toe warmers which can be purchased in any sporting goods store. This year, they are selling them in hardware stores and even 7-Elevens. It is important to note, however, that since the dihydropyridines are FDA-approved to lower blood pressure, it is imperative that we monitor a patient's blood pressure before and after taking the medication.
 
I am treating a patient right now who has a history of Raynaud's and has presented with a painful chilblain on her left hallux which has started to ulcerate. She is very thin and suffers from low blood pressure. Even though nifedipine and its class of drugs are not "big guns" in terms of lowering blood pressure, I felt it risky to lower her pressure even five or ten points. Hence, I am treating her with topical meds and the toe warmers, and she is doing well.

Beyond the standard "keep it warm" approach, I have a simple therapy for Raynaud's, chilblains, pernio, or similar vasospastic phenomena. While using a medication for diabetic neuropathy, I found an interesting side-effect. Surprisingly, these pathologies can respond well to Metanx. I prefer the pure form of L-methyl folate found in Deplin, supplemented with Jarrow brand methylated B12. There are few cross-reactions with the supplement and even fewer side-effects. Anti-seizure meds can be amplified, but otherwise they are a minimal issue. It can be used with children and adults. In my opinion, it beats the recommended calcium channel, or alpha blockers and their side-effects. I found that those who have trouble only in the winter months can get away with just seasonal use.

I very much feel similar to Dr. Borreggine, so much so that I went to the e-advocacy on APMA.org and sent letters to my congressman about the legislation requesting hardship exemption from EHR penalties for small practices. Hopefully, others will do the same, everyday until they hear us. I never felt specialists should be reporting the same as PCPs/internists. If the reporting was more tailored to our needs, it would be easier to take all of this. 
 
I may be incorrect, but have been told that stage 2 MU has significant hurdles to overcome, including costs for encryption and additional costs for patient education from a “certified vendor.” It was not enough that...
