Tiger Woods Might Likely Need an Ankle Replacement: FL Podiatrist
Michael Zuri, DPM, a doctor of podiatric medicine and board-certified physician who specializes in sports injuries, explained the severity of the injuries Woods suffered last week. "He's going to have multiple surgeries over the next week or two," said Zuri. "This is definitely of a limb-threatening nature. Do I think he'll lose his leg? Probably not. ... The next week is crucial. Infection is a major concern. I would say a month is when they'll kind of know what's going on as long as he doesn't have a bone infection or soft tissue complication. This is a tenuous thing for a while."
|
|
Dr. Michael Zuri |
"Even if the leg heals, all the bones heal, everything heals, he's going to be dealing with arthritis from this," said Zuri. "These types of crush ankle injuries, if it's the ankle that was shattered as well, those have a high percentage of arthritis within a five-year period. It's likely he might need an ankle replacement. He's going to have multiple surgeries over the next year and probably surgery in 10 years. This is going to be a long, long thing for him. These types of multi-traumas to the leg are just devastating."

NY Podiatrist Provides Tips on Buying Shoes For Bunions
When it comes to running shoes, a comfortable fit is always important but it's even more critical when you have bunions. Mohammad Rimawi, DPM says, "A wide toe box will help alleviate some of those symptoms by accommodating for the boney bump caused by the bunion."
|
|
Dr. Mohammad Rimawi |
Flexibility is another important feature, both in the toe box and around the shank. If a shoe is too stiff, your foot will rub and chafe more, causing pain and potential blistering, Dr. Rimawi explains. A flexible toe box, on the other hand, allows the big toe to move more freely while you run, helping to reduce irritation.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Improve Your Patient Experience With These Steps - Part 1
1. Make Your Practice Look and Feel Engaging
Patients will judge your practice before they even meet you. Making a good impression starts with a well-designed, patient friendly website where the public can learn about your practice: including your mission; the credentials of your physicians; the range of services you offer; your location(s) and office hours. Also don't forget to include interactive tools that allow patients access to even more information via pull-down menus, a map and directions to the office, and a portal for existing patients to make payments, schedule follow-up appointments, and communicate through secure e-mail.
Remember that a welcoming practice also depends on an appealing, clean, and comfortable office with amenities to ensure patients feel cared for and important, in both the reception area and the exam room.

| QUERIES (CLINICAL) | |
Query: MIS Surgery on a Patient with Sickle Cell Anemia
I have always avoided operating on anyone with sickle cell (not trait) anemia. However, I would like an opinion from some of my colleagues on an unusual situation. I have a 16 year old patient who is having real psychological problems because of severe painful hallux valgus deformities. She is bullied, made fun of, and refuses to go to school because of her feet. The parents are all for having it done, though I explained the risks, and though I have not spoken to her PCP yet, he gave her a medical clearance. Does anyone have experience operating on a sickle cell patient, or should this be avoided at all costs. The procedure would be performed in an outpatient ASC using minimally invasive surgical techniques.

RE: Podiatric Associations Should Provide Free CME Webinars
From: Judith E. Rubin, DPM
One thing none of my colleagues have ever written about is the expense we have to pay for credits to maintain our licenses. $529 for 20 credits is ridiculous. I pay over $1,000 in my state of Texas to be a part of the TPMA and AMA. When people are struggling to get PPPs to keep their practices going, I think it is ridiculous to pay these kind of prices for credits. MD and DO credits cost them nothing. They can use the UpToDate app and get all the credits they need for free. Can we do the same? They can’t believe we have to pay $529 for a webinar. Their webinars are free as are the lectures the hospitals put on for free. Don’t you feel, my friends and colleagues, that for the fees we have to pay to be in our states' podiatric medical associations, we should get a 25 credit seminar/webinar for free? These expenses are not deductible on your PPP loans.

RE: Fauci and Double Masking
From: David Secord, DPM
I was pilloried by a number of individuals the last time I pointed out that the actual (not make believe) science demonstrates that cloth and O.R.-type masks are ineffectual for impeding the aerosols which carry viruses and that the grand majority of studies quoted by the cognoscenti are talking about droplets and not aerosols and, therefore, have no relationship to the question of whether masks work.
Now that media darling and pathological liar Anthony Fauci has decided that if one worthless mask isn't enough, two (or maybe a dozen) would be a lot better (and the CDC confirmed this absurdity) and has decided that you still need to wear a worthless mask even after receiving the vaccination for the COVID-19 virus (which makes absolutely no sense). I thought it was time to take another whack at it. I've attached a nice article about the subject, which no one will read. It has links to several studies by Xiao and Konda (which no one will read) which conclude that masks have no effect upon transmission or reception of the aerosols which carry viruses.
Mask wearing has had no effect upon the contagion rate, death rate or transmission of the virus. Lockdowns have had no effect, except to destroy the economy. Social distancing has no effect upon the area under the curve of a closed set cohort. It is time to stop being sheep and believe the (actual) science.
David Secord, DPM, McAllen, TX

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 3 | |
From: Lance Malusky, DPM, Thomas Silver, DPM
Sorry Dr. Ressler, I agree more with Dr. Gurnick on some carpet for flooring. We mandated vacuuming corns, nails, and calluses rooms between patients. My patients preferred the warmer carpet, to walk barefoot on, after removing their socks to walk to the exam chair.
Lance Malusky, DPM, Dayton, OH
In my old office, we had carpet in hallways and treatment rooms with linoleum in the procedure room. The carpet trapped everything from dirt, dust, and stains - including blood, Betadine, etc., and showed a lot of wear over time. It was a major job to clean the carpet and we couldn't do it often enough. The linoleum discolored over time and had stains and multiple nicks in it.
In our new office, we have large white tiles in the reception area, hallway, and procedure room. In the treatment rooms, we have wood laminate that has a warm, rich look to it. They are all incredibly easy to clean, never stained, and only a few very small nicks on the laminate over the past 10+ years. As a result, patients constantly comment on how clean our office looks! The only areas we have carpet are in the low traffic areas: business office, storage rooms, and my private office.
Thomas Silver, DPM, Golden Valley, MN

| RESPONSES/COMMENTS (NEWS STORIES) | |
From: Priya Parthasarathy, DPM
I read with dismay the response from Dr. Morelli to the article including podiatric medicine and surgery on the list of “odd jobs that pay well.” Dr. Morelli notes that if state and national associations would produce fewer "knock your socks off and do your feet smell campaigns,” podiatry might be better understood by the public. I urge Dr. Morelli to take the time to familiarize himself with recent APMA campaigns. APMA has never produced a campaign on foot odor, and “Knock Your Socks Off” was an award-winning campaign promoting the need for regular diabetic foot exams. In the past five years, APMA through its member Communications Committee has produced incredibly successful public education campaigns on topics including peripheral neuropathy, surgery, limb- and life-saving diabetes care, and peripheral vascular disease.
Our most recent campaign, launched in November 2020, urged the public to keep their regular diabetes care appointments during the COVID-19 pandemic. That campaign generated 70 million media impressions, 3.4 million advertising impressions, and 1 million impressions on social media, among other outstanding results. The relationships our member spokespeople build with reporters when they discuss “lighter” topics such as footwear pay significant dividends when we reach out to those same media to pitch our campaigns.
Each and every one of APMA’s public education campaigns includes an easy-to-use member tool kit with multiple resources for members to help spread the word about podiatric medicine and surgery at the local level. Member involvement in these campaigns is a critical component of their success in educating the public. Dr. Morelli, I hope you’ll take advantage of the tool kit for our upcoming April Foot Health Awareness Month campaign, which will focus on podiatry’s critical role in sports medicine.
Priya Parthasarathy, DPM, Chair, APMA Communications Committee
