Beware of Medical License Scams

Recently, PM News has received several reports from podiatric associations and individual podiatrists regarding attempted medical license scams. In most cases, the podiatrists received a phone call from an individual claining to be from the FBI or from the state licensing board. Fraudsters perpetuating these schemes use sophisticated techniques such as spoofing the name or phone number you see on your caller ID to make it look like it originated from a government agency. 

The scammers will also tell you your NPI and license number, which are readily accessible on the Internet. Please note that legitimate government agencies do not call doctors - they send letters. Furthermore, no government agency will ever request financial information or demand payment over the phone. In the event that you receive such a call, either hang up or separately call the agency from which they purport to represent and report receiving the call.

Stubbed Toes Can Result in Nail Bed Injuries: NY Podiatrist  
 
As it turns out, we do things all the time that could result in a stubbed toe — except that when we’re wearing shoes, we don’t actually feel it, according to Michael Trepal, DPM, a podiatrist in New York City. In fact, a stubbed toe means that “in the course of walking, you hit your forefoot against an object while barefoot or wearing an open-toed shoe.”

If you hit your toenail during the stub, bleeding under the nail—aka a subungual hematoma—may occur. “There is a lot of bacteria around the nails, and so there’s a risk of infection with nail bed injuries,” Dr. Trepal explains, noting that if this happens, you should make sure to keep the area clean. In situations where your nail or nail bed was injured and a blood blister forms under the nail, Trepal says that a podiatrist should drain it to reduce the risk of losing your nail completely.

Podiatric Care Can Prevent Complications of Diabetes: PA Podiatrist 
 
According to Samantha Newstadt, DPM, with Coordinated Health, part of Lehigh Valley Health Network, many people who have diabetes develop some degree of neuropathy or nerve damage in their legs or feet, which causes a loss of sensation. “When this happens, you have a higher risk for being injured without knowing. If left untreated, even small injuries can progress into something more significant,” she says.

Dr. Newstadt says another reason that people who have diabetes are more susceptible to issues with their feet is because many have some element of vascular disease in their lower extremity, which also can contribute to complications. The good news is that there is plenty you can do to prevent diabetes–related foot conditions, starting with seeing a podiatrist. “If you have diabetes, it’s important to be established with a podiatrist and see them regularly. A podiatrist can evaluate any changes in your vascular health or for the development of neuropathy,” she says.

California podiatrist and healthcare attorney, David Mullens, DPM, JD was the speaker during a one-hour The American Bar Association (ABA) webinar on February 26, 2021. The program was titled: “When a Client Wants You to Appeal a Medicare Denial…” and is based on Dr. Mullens’ book How to Win Medicare Appeals - published in 2020 by the ABA.

Dr. Mullens was a professor in the basic science and podiatric medicine departments at the California College of Podiatric Medicine and served as a clinical associate professor in Stanford’s dermatology department and as a clinical assistant professor in Stanford’s orthopedic department. He also established the podiatric surgical residency program at the Palo Alto VA Hospital and the podiatric surgical service at Stanford University Hospital. Although retired from his podiatric practice, he continues to actively practice law.

Research has demonstrated the connection between better communication scores and improved patient adherence, lower re-admission rates, improved mortality rates, lower malpractice risk, and reduced cost per case. No other area of medical knowledge or technical skill has a greater impact on our patients. Try some of these suggestions.
 
9. Leave on a Good Note
 
How you end a visit is as important as your first impression. During the visit, it is helpful to continually orient the patient to the process of care. Think ahead to the conclusion of the visit. Summarize diagnosis, treatment, and prognosis and then review next steps: future visits, phone calls, communication of test results, and handoffs. End the visit with an expression of optimism or hope, such as, “I hope you will be feeling better soon,” or, “hang in there; we are making progress,” to bring the visit to a satisfying conclusion.

Sorry, Dr. Malusky but carpet in treatment rooms is old school. When you visit your dermatologist, ophthalmologist, or internist - look down. You will see some form of tile which is washable.The C and C rooms should be vacuumed. Nothing will drive a patient away than stepping on an old toenail.

I built out my last office in 2013. I selected rectified 24” ceramic tile. It was a great selection and we were always getting compliments from the patients. Rectified tile allows you to almost butt the tile edges up to each other which allows for a very narrow grout line (around 1/16”). Go with the largest tile possible, at least 24” to 36”. They do make larger but it becomes more costly to install. With larger rectified tile, the floor looks almost like a large sheet, providing your selection doesn’t have too much pattern or veining. Larger tile also makes the rooms appear bigger.
 
When choosing a ceramic tile, it is better to get a ceramic tile from either Italy or Spain as opposed to a Chinese product. Chinese ceramic tile tends to warp which will raise the corners of the tile edge. I used a Chinese ceramic tile and some of the edges of the tiles were slightly raised. It was never a problem and I’m the only one that really noticed it. The larger the tile, the more warping could occur. Flooring in an office is often a second thought but careful selection will make for a great appearance and provide long-lasting wear and satisfaction.

What type of flooring is best for a podiatry office may differ depending on where your office is located, but also the type of practice you have or hope to achieve. I would break down office flooring decision into five areas 1) The patient reception room  2) The treatment rooms 3) Office hallways 4) lab, storage and staff rooms and 5) doctor's personal office (desk area). 
 
Here are the considerations that go with this. 1) Patients expect to see a clean office; dirty flooring leaves a bad impression. Vinyl or linoleum shows scuffs and needs to be washed, cleaned, and/or waxed and polished periodically to remain free of scuffs and also fresh-looking. 2) Hard flooring such as wood laminates, vinyl, and tile are much easier to keep clean and wear well over time. 3) Carpeting is more comfortable to walk or stand on but will show dirt and stains easier and is more difficult to keep clean and will show wear sooner over time than hard flooring. If I was re-doing an office here are my suggestions:
 
1) Reception room and treatment rooms: vinyl,  linoleum, or wood laminates 2) Office staff area: vinyl or wood laminate 3) Doctor personal office: dark color carpet 4) Hallways where patients walk barefoot to observe gait: also dark colored carpet (it is just more comfortable for our patients coming in with foot pain to walk on a harder surface).

It’s disappointing to me that a fellow podiatrist recommends going to a running store to get orthotics. It’s bad enough that our profession has to compete with outside groups who have limited understanding of biomechanics of the foot and are diluting the public’s perceived value of functional foot orthotics by selling “custom” devices. In many cases, they make the patient’s foot hurt more. We are supposed to be the specialists, and even if our own members aren’t advocating that, it makes it more difficult to explain that to the public. If you don’t provide orthotics yourself, please refer to a qualified podiatrist in your community who does.

Dr. Morelli expressed displeasure about the way our profession was portrayed in a recent article from MSN Money Talks News. I will not comment on the content of that article; however, I take strong issue with his characterization of how local and national leadership represents the profession. Before becoming president of the New York State Podiatric Medical Association, I served on the Board of Trustees for many years. During that time, one of the committees that I co-chaired was the Public & Population Health Task force. This task force has been working hard for several years gathering data on how podiatry plays a significant role in major public health areas such as diabetes mellitus, obesity, the opioid crisis, and falls prevention. We are currently conducting a pilot program in falls prevention at Lenox Hill Hospital. This is an initiative in which the CMS has expressed great interest. This hard data will be used to demonstrate to anyone interested the absolutely vital role that podiatry plays in these areas.
 
Charles, I can state that throughout all my years in promoting and discussing the role of podiatry with high-level stakeholders at the local, state, and national level, I have not, nor have any of my colleagues,  brought up the topic of “flip-flops are bad for your feet”. Charles, I have known you for many years. If you truly do not know what NYSPMA and APMA do for you and our profession, please reach out to me. I assure you that I can talk your ear off on these matters as I have made them a cornerstone of my time in office.
