NJ Podiatrist Dead After Apparent Suicide/ Attempted Homicide

Dr. Victoria Vovchik

Police are investigating a possible attempted murder-suicide in Rockland County. Authorities said a woman, identified by Clarkstown police as Dr. Victoria Vovchik, 45, stabbed her 7-year-old daughter several times, and then killed herself inside her home. The little girl was taken to Westchester Medical Center in critical condition.

NY Podiatrist Provides Cold Weather Foot Health Skin Care Tips

During this time of year, many of us have a tendency to develop unusual soreness and pain in our feet. This is primarily due to the cold temperatures that dry out our skin, coupled with our footwear choices and activity levels. While many of us attempt to stay active and follow our New Year resolutions to exercise more often and lose weight, we need to take better care of our feet to prevent injury and reach our goals. Dr. Bruce Pinker provides the following cold weather foot health skin care tips:

Moisturize feet regularly: "You need to maintain healthy skin on your feet. Applying moisturizer after bathing can reduce dry, cracked, painful skin. This is especially important for diabetic patients," says Pinker. Avoid dehydration: Consuming eight glasses of water daily is still one of the best recommendations for supporting proper water balance.

Woman Sentenced for Embezzling $471K from IN Podiatrist

A Muncie woman was sentenced to 51 months in federal prison this week after she admitted to embezzling $471,946.85 while employed as business manager in a Muncie podiatrist’s office.

Kandice S. Inskeep, 41, was ordered Monday to “immediately” repay the stolen money — to her victim and former employer, Dr. Thomas Freeman — by U.S. District Court Judge Jan Magnus-Stinson. However, the judge also ruled that upon her release from prison, Inskeep must pay “not less than 15 percent of (her) gross monthly income” toward any remaining restitution during the five years she will serve on probation.

Sequester's Medicare Cuts Would Start with Services Provided in April

If sequestration kicks in Friday as planned, the 2% payment reduction to Medicare providers and insurers will be for services provided on or after April 1, an HHS spokesman confirmed Wednesday. When asked if HHS has already alerted providers and insurers about the date of the payment cuts, the spokesman replied in an e-mail, “If sequestration occurs, official notifications will be made.”

The lack of any notice from HHS left provider groups to wonder when their members will see those reductions. An official for the American Health Care Association—which represents skilled-nursing facilities and assisted-living providers—said there has been confusion among the organization's staff and lawyers about when those cuts would take place, while the American Hospital Association, Federation of American Hospitals and National Association of Public Hospitals and Health Systems all anticipated the cuts would begin in April.

Query: PQRS Reporting Period

I understand that CMS is asking us to report on specific conditions (diabetes) with three "Measures." They have to be reported once in a reporting period. I understand that this period is one year. If that is true, does the year run from the first time the measures were reported to a year later, or is the one year period based on a calendar year like shoes for qualified diabetic patients?

Response: Actually, the requirement, assuming you are using the individual claims method of reporting, is to report on at least 3 measures and for each measure report on at least 50% of eligible patients. There is no requirement with this method that the measures be on a specific condition. You can pick any 3 measures that you choose that are within your scope of practice and are applicable to patients that you treat. The 3 measures related to the diabetic foot are just generally very easy for podiatrists to report.

With regard to the reporting period, it is the calendar year--January 1, 2013 through December 31, 2013. Fortunately, the 3 measures related to the diabetic foot--126, 127, and 163--need only be reported once during the reporting period for each eligible patient.

RE: The Passing of Harry Shapiro, DPM

It was with sadness and nostalgia that I learned of Harry’s death. While I could not be considered a close friend, I have fond memories of our intermittent relationships. When I first started my practice in 1965, Harry was one of the first of my colleagues to invite me to his office, to assist in procedures, and learn the skills to run my practice. He was cordial to everyone and was always welcoming both in his practice and his home which was the site of many podiatry meetings. He was always the gentleman. My condolences to his family and to the profession which is enhanced by his participation.

RE: Fixation Vs. Non-Fixation of Osteotomies (Gino Scartozzi, DPM)

I too echo Dr. Scartozzi's comments regarding a met head "seeking" its own level. With the exception of some salvage procedures and some specific neurological deformities, there are very few metatarsal osteotomies that I would ever do without fixating them in some manner. All you need is one or two complications early on in your career to teach you that lesson, regardless of what the literature says or what orthopedists have been doing for years with the "V" osteotomy.

Another life lesson learned early on is that there is one thing you never want to mess with and that is the metatarsal parabola. Once that is done, you have other problems that can last years (transfer lesions, ulcerations, bursitis, stress fractures, lawsuits, etc.), and there are few things worse in private practice than to have the same patients come back to the office month after month, year after year, so you can manage the problem you just caused.

Do yourself a favor. When in doubt, fixate it. You will sleep better and so will your malpractice carrier.

I thought it was very interesting that one podiatrist cited Dr. Mercado as an excellent surgeon and example. Dr. Mercado has lectured for the Academy of Ambulatory Foot Surgery and in his excellent books, he has described several minimal incision techniques that he himself used! In the other letter published, somehow I am taken to task about local vs. national standards. I simply never wrote that. However, as a healthcare attorney, I enjoyed the lecture by someone with no legal training. The letters are filled with conclusory "observations" and war stories. Practices "market" themselves that utilize all types of incisions and surgical techniques.

A year or so back, I found I was unable to order WBC-tagged bone scans to differentiate a diagnosis of osteomylitis from a possible Charcot event. When I contacted my local hospital's nuclear medicine department, I was told that the Medicare-approved payment was less than half of the procedure cost and the hospitals could no longer afford to provide this service.
