TX Podiatrist Discusses Tiger Wood’s Injury
Tiger Woods required what his doctors called a lengthy surgery to repair significant injuries to his right leg after his car rolled over outside Los Angeles. “The more concern I have is with the ankle and the foot,” says Zach Barnett, DPM, expert in sports and trauma medicine. Reports indicate wood’s leg suffered comminuted open factures, meaning the bones broke in more than two places and broke through the skin. But surprisingly, Dr. Barnett believes they are recoverable injuries. “They ended up putting a rod that goes inside canal of the tibia and that generally does an excellent job of stabilizing that bone and getting those fractures to heal.” But Dr. Barnett, who specializes in foot and ankle surgery, said news that Woods had to have his ankle stabilized with screws and pins is concerning.
|
|
Dr. Zach Barnett |
“The problem with the foot is there are 28 bones in the foot, and they all work together, and if some of those fractures involved the joints, you really can develop early onset arthritis.” Dr. Barnett says for a golfer, pain and restriction in the foot and ankle can impact their game because a golf swing involves making an internal and external rotation of the leg. “The ankle and the foot really work together for motion, so if he loses some of that motion either from arthritis or fusions, it can impact the swing.” Dr. Barnett thinks at a minimum, Woods will likely be sidelined for up to 6 to 8 months.

IL Podiatrist Discusses Metatarsalgia
While many underlying causes of ball of foot pain resolve with rest and at-home treatments, like taking non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen and naproxen, some need to be addressed by a doctor and treated. Otherwise, they'll get worse over time and may progress to the point that they interfere with life, says Brian Burgess, DPM, a board-certified podiatric surgeon at Illinois Bone & Joint Institute.
|
|
Dr. Brian Burgess |
Wearing good footwear can help you avoid a lot of the ball-of-foot-pain culprits. Think: cushioned sneakers with arch support, not flimsy flip-flops with barely anything between your feet and the ground. Orthotics can also be helpful to wear before issues arise, Dr. Burgess says. He recommends going to a specialty running shoe store to get some advice on both shoes and orthotic inserts. "If you're at a store where they can properly fit you, more likely than not, they have quality devices that will be supportive," he says.

RPM Helps Detect Signs of Inflammation Earlier: CA Podiatrist
Siren, a health technology company that pioneered the development of microsensor-embedded textiles for medical applications, announced that it is augmenting its Siren Socks product offering by launching Siren Staffing, a remote patient monitoring (RPM) program. Initial customers include StrideCare and Village Podiatry Centers. Siren Socks continuously track foot temperature and were developed to help patients living with diabetes manage their foot health more effectively. Temperature monitoring has been clinically established as an important physiologic marker of injury and is recommended as a tool to detect and prevent diabetic foot ulcers (DFUs).
|
|
Dr. Gavin Ripp |
"Implementing Siren Staffing has made my practice more efficient and effective," said Gavin Ripp, DPM, of Premier Podiatry & Orthopedics in Northern California. "The team works closely together to monitor patients and coordinate care with our systems. The result is that I am able to quickly act with greater information. This comprehensive solution allows us to monitor more patients to detect signs of inflammation and injury earlier and intervene effectively."

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Nine Ways to Improve Your Patient Communications - Part 8
Research has demonstrated the connection between better communication scores and improved patient adherence, lower re-admission rates, improved mortality rates, lower malpractice risk, and reduced cost per case. No other area of medical knowledge or technical skill has a greater impact on our patients. Try some of these suggestions.
8. Ensure Buy-in Through Shared Decision-Making
Buy-in is a critical element of shared decision-making. It’s an invitation to the patient to collaborate in decision-making around the goals, options, and plans for treatment. Patients who are consulted and engaged in their care report enhanced satisfaction, understanding, and confidence in their physicians’ decisions. Developing shared goals for treatment will also help motivate patients to adhere to the treatment plan. Through education and shared decision-making, patient anxiety decreases, there’s a stronger physician-patient partnership, and more satisfying relationships for both parties.

| QUERIES (CLINICAL) | |
Query: Dermatitis or COVID-19?
A 29 year old healthy female presented with this rash on top of her foot. She had a pedicure 2 days prior, where gel polish was removed with acetone wraps on her toes. Acetone did not contact the top of her foot. She felt immediate burning on the affected foot upon placing her feet in a basin of warm water. The lesions appeared almost immediately and became progressively worse. The other foot shows some very mild lesions.
|
|
Dermatitis or COVID-19? |
The foot does not itch but is painful and tender. 1% hydrocortisone cream didn't help and caused burning. There is no cellulitis, lymphangitis, or lymphadenitis. She received the second Pfizer shot about 10 days prior. She developed reactions consisting of flare of cold sores, flare of eczema, mild fever, and fatigue that lasted 1-2 days. My question is could this be a reaction to the COVID-19 vaccine? I started her on antibiotics. If there is no improvement next appointment, I plan on a biopsy. I told her to call the phone number on the vaccine card about this possible vaccine reaction. Any thoughts would be appreciated.

RE: Best Type of Flooring for Podiatry Office (Claudia Siegel, DPM)
From: Lance Malusky, DPM, Jim Shipley, DPM
A linoleum that feels warm to the touch (or with a thin cushioned material beneath), with lines or squares, is best in the gait analysis halls. This helps visualize gait patterns without the patient feeling awkward or uncomfortable. For your treatment rooms, a thin, randomly patterned, multicolored, high traffic carpet is best, to hide CNC debris.
Lance Malusky, DPM, Dayton, OH
After having linoleum in my first office (big mistake), I had tile installed in all my other offices. Yes, there is a clicking sound every time an ultrasound cart is rolled down the hallway, but the wearability of tile can't be beat; no matter what chemical or bodily fluid is dropped on it. All my tile was bought at the big box hardware stores for under $1/sf and looks really good. I highly recommend it.
Jim Shipley, DPM, Mt. Airy, NC

| RESPONSES/COMMENTS (NEWS STORIES) | |
From: Charles Morelli, DPM
What an insulting article. "Odd jobs that pay well"? Maybe they can combine podiatry with sheet rock and painting fences? They make it sound as if anyone, from any walk of life, can go on Indeed.com, look for a position as a podiatrist, and start working. Or, they can go to one of those night schools that advertise on the back of matchbooks and get a degree that way, although a degree was never mentioned.
Being a podiatrist is a "great way to start", but at least we "know a lot". This is how the general public sees our profession. It has always been this way and the fault lies at the feet of our local and national "leadership" who have failed us time and time again. Maybe we can start to see less "knock your socks off" and "do your feet smell" campaigns and a more targeted crusade for the medical, diagnostic, pediatric, sports medicine, and surgical maladies we treat. With spring coming, I can’t wait for PM News to publish more articles by podiatrists telling the public why flip-flops are bad for your feet.
Charles Morelli, DPM, Mamaroneck, NY
