CA Podiatrist Named New Dean at WesternU
 
Western University of Health Sciences has named V. Kathleen Satterfield, DPM, as Dean of the College of Podiatric Medicine, effective May 1, 2019. Dr. Satterfield joined the College of Podiatric Medicine (CPM) in 2010 and currently serves as Professor and Associate Dean for Pre-Clinical Education and Outcomes Assessment.

Satterfield earned her Doctor of Podiatric Medicine degree from the Des Moines University College of Podiatric Medicine and Surgery. Satterfield will take over for Lester J. Jones, DPM, MSEd, who has served as CPM’s interim dean since July 2017. Dr. Jones will retire in April 2019 after serving CPM for more than 13 years as a consultant, professor, executive associate dean, and interim dean.

GA Podiatrist is New AENS Fellow
 
Stephen L Barrett, DPM, fellowship director for the Association of Extremity Nerve Surgeons (AENS) and CEO of US Neuropathy Centers (USNC), and Sequioa DuCasse, DPM, COO of USNC, have welcomed Porscha Jacque Bailey, DPM as the new 2019-2021 Fellow in Lower Extremity Peripheral Nerve Surgery.

Dr. Bailey, a graduate of the Barry Universty School of Podiatric Medicine, is currently the chief resident at Westchester General Hospital in Miami, FL; a three-year program with the added credential in reconstructive rearfoot/ankle surgery, which she will complete in June 2019. Dr. Bailey will serve as the 4th Nerve Fellow in the program, which is becoming a 2-year fellowship.

GA Podiatrist to Perform First Total Ankle Replacement at Northside Hospital 
 
Ryan Fitzgerald, DPM of Village Podiatry Centers of Canton will be conducting the first Total Ankle Replacement Surgery at Northside Hospital Cherokee on Friday, March 1st. Dr. Fitzgerald will be using the MoveWright™ Total Ankle Replacement System and Prophesy pre-operative navigation system, which uses the patient’s CT scan imagery to create 3D printed, patient-specific instrumentation. This, in turn, helps perform a complex procedure in less time and with greater accuracy as compared to more traditional treatment methods.

Key Steps to Mitigating Your Cyber Risk - Part 4
 
Continuing cyber attacks on healthcare providers emphasize the need for medical practices of all sizes to make cyber security an essential part of their business. Here are some steps to strengthen your practice’s cyber security position. 
 
4. Mobile devices - Many practices allow employees to bring their personal mobile devices to use at work (bring your own device, or BYOD). While BYOD is convenient, it also raises security issues, as personal devices can have outdated operating systems, lack minimal security controls such as password protection or encryption, and may be easily lost or stolen. 
 
Adopting a well-defined BYOD policy helps lessen the security risks. A BYOD policy should address which devices may be used on your network and require the use of passwords or biometric authentication, regular updates of the operating system and installed apps, installation of anti-malware and antivirus software, and a remote wiping program to delete data in the event the device is lost or stolen.

Query: Multiple Tarsometatarsal Fusions
 
The surgeon is performing a tarsometatarsal fusion of 1, 2 and 3 rays. Do we bill code CPT 28730 x 3, or CPT 28740 x 3? The Medicare fee schedule has a lower rate of reimbursement for the multiple joint surgeries vs the single joint, which we don’t understand.

Response: CPT 28730 – Arthrodesis, midtarsal or tarsometatarsal, multiple or transverse
CPT 28735 – Arthrodesis, midtarsal or tarsometatarsal, multiple or transverse; with osteotomy (e.g., flatfoot correction)
CPT 28740 – Arthrodesis, midtarsal or tarsometatarsal, single joint
 
When fusing the three joints you listed, the only options are one unit of CPT 28730 or one unit of CPT 28735, with selection between the two based on whether or not osteotomy was performed. If it was just straight fusions without any osteotomy (often seen with trauma), CPT 28730 should be used. If there was an osteotomy performed to allow for shortening, lengthening, or angular correction, CPT 28735 should be used. The fee schedule or RVUs should not play a role in code selection in this case.

Pure transposition is transposition, regardless of the length of the dorsal arm. Extending the dorsal arm, as originally described in German literature (the offset "V" osteotomy) and elucidated by Vogler, does not increase the amount of transposition available inherently. Geometry is geometry. Extending the dorsal arm does allow for the provision of more fixation, such that one might transpose the 1st metatarsal to a greater degree with the security of additional fixation to compensate for decreased “bone to bone“ surface area contact. Additionally, elongation of the dorsal arm provides a greater opportunity to rotate an otherwise purely transpositional osteotomy, thus further reducing the intermetatarsal angle at the expense of decreased bone area contact.
 
Every osteotomy affords a maximal correction (lineal, transpositional, angulational, rotational as examples). When such osteotomies are "modified", there is frequently some reduction in the benefits for which the original osteotomy was described. Of course, the entire question of distal osteotomy is silly. Perhaps the good doctor is unaware that to do anything but a Lapidus procedure is inappropriate and in this particular, associated with an 87% failure rate.

The divisions described in these two topics represent divisions within our profession and between our profession and other healthcare providers and payers of healthcare. To me, they clearly represent the same issues. Podiatry has these issues because it has run its course, in a way, just like chiropody ran its course. As podiatry matured from chiropody, it appears podiatry has matured itself into the fabric of mainstream medicine. So is it necessary to have another global discipline separate from allopathic or osteopathic medicine? I don’t think so.
 
Chiropodists and podiatrists should be proud of what we accomplished. We all in our timebrought awareness of the importance of the healthcare of the foot to the public, but also to other healthcare providers. If the fight of the advanced foot and ankle surgeons is with orthopedic foot and ankle surgeons, the general podiatrists with other medical specialties and ancillary providers, and all podiatrists with third-party payers, don’t you think it’s time to re-evaluate our profession and move forward to the inevitable and correct end? For those who want to compare podiatry to dentistry, it’s like comparing oranges to apples.
