IA Podiatrist Discusses Options for Treating Heel Pain

Mahaska Health Partnership podiatrist Dr. Mark Beers said one of the most common complaints from his patients is heel pain, often diagnosed as plantar fasciitis. “Plantar fasciitis causes stabbing pain that usually occurs with your first steps in the morning,”  Beers, who has been a practicing podiatrist in Oskaloosa for more than 20 years, explained. “It often takes a while for your foot to limber up and the pain to decrease. However the pain often returns after extended walking or long periods of standing.”

Dr. Mark Beers

Dr. Beers said that more than 90 percent of patients with plantar fasciitis don’t require surgery. “Initially, plantar fasciitis treatment consists of anti-inflammatory medications, heel stretching exercises, night splints and/or shoe inserts.” If these treatments fail,  Beers said a cast boot for three to six weeks is often successful in reducing pain and swelling. Steroid injections are also used to relieve pain. In a few patients, surgery is required to release the inflamed fascia.

TN Podiatrist's License Revoked for Practicing With a Suspended License

The Tennessee Podiatric Medical License of Michael W. Lawrence, DPM has been revoked. Lawrence's license was suspened on February 6, 2013 for dispensing, prescribing or otherwise distributing any controlled substance or any other drug not in the course of professional practice, or not in good faith to relieve pain and suffering, or not to cure an ailment, physical infirmity or disease. Lawrence examined and treated patients at the Volunteers in Medicine Clinic in Chattanooga, Tennessee while his license was suspended. Practicing podiatry while a license is suspended, whether for compensation or not, is a violation of the Tennessee Podiatric Medical Practice Act.

MEETING NEWS
AAFAS Winter Cadaver Conference Draws Rave Reviews

The Academy of Ambulatory Foot & Ankle Surgery (AAFAS) held its Minimal Invasive Foot Surgery (MIFS) Winter Cadaver Conference at The Louisiana State University Medical Center in New Orleans to rave reviews. Hot topics included minimally invasive nerve decompression, subtalar stenting, forefoot osteotomies of metatarsals, digits, bunions and bunionettes and the soft tissue MIFS treatment of diabetic wounds by famed orthopedic surgeon Dr. J. Monroe LaBorde.

Attendees at AAFAS Winter Cadaver Seminar

The highlight was the full day of cadaver dissections presented by world leaders in MIFS. In attendance were many new faces including high powered open surgeons looking to add MIFS to their tool boxes to become what is being called “hybrid” surgeons. There were also adjacent lectures on external fixation bandaging, the biomechanics of MIFS, MIFS Instrumentation, and a historical overview of the growth of this podiatry driven school of surgery into what is fast becoming a respected and growing International and multidisciplinary field of medicine.

CODINGLINE CORNER
Query: Tibialis Anterior Repair Coding

I performed a tibialis anterior repair with excision of damaged tendon, slide graft, and reattachment to the cuneiform. What is the appropriate CPT coding for this procedure?

Carl Ganio, DPM, Vero Beach, FL

Response: A tendon repair is a tendon repair no matter what steps are required to repair it. I suggest CPT 27665 (repair of extensor tendon of the leg).

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Technology May Put Private Practice Into Extinction
From: Joseph Borreggine, DPM

I have a concern that with the upcoming advent of ICD-10CM and the current requirement in 2014 of Stage II meaningful use attestation that technology will inherently put the independent practitioner out of business sooner than later.

Maybe this was CMS's intention all along. The "uncontrollable expense variable" of a private practitioner has long been the problem in the escalating healthcare expenses. But, with tighter and tighter regulation, more...

Editor's note: Dr. Borreggine's extended-length letter can be read here.

RESPONSES/COMMENTS (MEDICAL/LEGAL)
From: Olga Luepschen, DPM

This discussion brings up an interesting point. What percentage of our colleagues carry $250,000/750,000 vs 1 million/3 million coverage? I was under the impression that $250,000/750,000 coverage was adequate.

Olga Luepschen, DPM, Sebring, FL, Feetdoctor10@hotmail.com

Editor's Comment: This will be the topic of next week's PM News Quick Poll question. As previously stated, we believe that $250,000/750,000 coverage is inadequate.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1A
From: Tip Sullivan, DPM

PM News is my rock. It is usually the first thing I look at when I get to the office every morning. Having practiced in a fairly underserved area for 25 years, PM News is where I get my relevant podiatry information. It has kept me connected with my colleagues in times when I have felt very isolated. Sometimes I do not like some of the views expressed, but one thing that you have to say about Barry Block is that he will do his best to put all sides of an issue out there –even when he personally may have strong feelings in favor of one or the other. His dedication to all of us is tireless! Think about it---5,000 issues! 5,000 days! How many articles? How many topics? How he does it is miraculous! Thank you Barry Block!

RESPONSES/COMMENTS (NEWS STORIES) - PART 1B
From: Alan Sherman, DPM

Congratulations, Barry, for creating and then so carefully nurturing a totally new institution in podiatry practice. Its value is beyond reproach. Reading PM News and contributing to the conversation that it fosters has been adopted as habit by the majority of our colleagues in podiatry. Due to your efforts, podiatry is unique among medical specialties in its connected-ness, and in the age of networking, we all know what value those connections create. Due to PM News,  podiatrists are better informed and speak more with one voice. Publishing PM News and editing Podiatry Management will be the legacy that you leave behind, making your contribution to the profession among the greatest our young profession has known. Thank you.
