MI Podiatrist Discusses Orthotics
 
"Orthotics may be used to help reduce foot pain and prevent, or minimize, the discomfort of foot deformities," said Jodie Sengstock, DPM, director of professional relations for the Michigan Podiatric Medical Association (MPMA). "Patients with arthritis, fat pad atrophy, and heel and arch pain are often pain-free with the use of orthotics."

"Other issues such as balance and leg or lower back pain can also be treated with the proper orthotics. An MPMA podiatrist would know if an orthotic is the right solution for an individual, or if a more comprehensive treatment plan is needed," says Dr. Sengstock.

The New Jersey Podiatric Physicians and Surgeons Group, LLC (NJPPSG) has announced the addition of Michael Banh, DPM to its group of physicians and surgeons. Dr. Banh will be joining Dan Phan, DPM, MBA, MPH as a partner in their new office in the Lawrenceville/Princeton area (Lawrenceville Foot Care, LLC).

He earned his degree in Doctor of Podiatric Medicine from the Kent State University College of Podiatric Medicine and completed his three-year surgical residency as chief resident at Southwestern Vermont Medical Center, Bennington, VT where he specialized in podiatric medicine and surgery with the added credential in reconstructive rearfoot/ankle surgery. He is is board certified by the American Board of Podiatric Medicine.

Atlantis Symposium Covers Wide Range of Topics
 
The 2020 Atlantis Foot and Ankle Symposium attracted close to 100 registrants, fellows, and residents over the January 17-19 weekend. The meeting was co-chaired by Drs. Charles Zelen, Director, Professional Education Research Institute (PERI), and Allen M. Jacobs. It featured lectures by the co-chairs as well as by Drs. John Visser; Lawrence DiDomenico; Paul Glat, and Michael Trepal. The New York College of Podiatric Medicine (NYCPM) served as the academic sponsor of the meeting.

The wide-ranging lectures allowed attendees intense exposure to presentations on many subjects. Saturday featured a morning focus on bunion surgery, and an afternoon concentration on rearfoot surgery. Sunday morning’s lectures mostly focused on diabetes and the Charcot foot.

OH Podiatrist Uses Wound Care Imaging Device
 
University Hospitals' (UH) Ahuja Medical Center is the first clinical setting in Ohio using special technology to identify harmful bacteria, leading to healing for patients suffering from chronic wounds. Windy Cole, DPM, Medical Director of the UH Ahuja Wound Care Center, conducted research studies utilizing the MolecuLight i:X®.

“A violet light is shone onto the area of the wound, and it excites bacteria on the surface,” Dr. Cole explained. “In real-time, different types of bacteria fluoresce different colors, so we can tell if and where tissues have certain bacteria levels. The i:X device can detect bacteria at loads that delay wound healing and go undetected with standard of care wound assessment. This means we’re now able to see what’s happening on and below the surface of these wounds, beyond the naked eye and before problems arise.”

Ways to Rein in Your Practice’s Hidden Billing Costs - Part 2
 
Where do hidden billing costs typically exist—and how can practice leaders more effectively rein them in? Here are some common blind spots.
 
2. Claim status inquiries. Manual claim status inquiries are the costliest of any administrative billing expense tracked by the Council for Affordable Quality Healthcare (CAQH). Why? Manual inquiries require 14 minutes of staff time, on average, to complete, according to the 2018 CAQH Index. Electronic claim status inquiries, on the other hand, take just five minutes to complete. As a result, the savings opportunity in moving from manual to electronic status inquiries totals $2.6 billion among medical providers.

In response to Martin Pressman, DPM’s comments on my Article “What Kind of Podiatrist Do Today's Residents Want to Be?”, I can’t help but agree that the original survey response was small at 58 out of 1,000 invited. I have more information now that 127 responses have come in. The percentages have stayed about the same. 88 out of 127 or 70% intend to be advanced foot and ankle surgeons, while 38 out of 127 or 30% intend to be general practice podiatrists. If the dual track 3rd Year were offered, 95 (residents) or 75% would choose the advance foot and ankle surgery track and 30 (residents) or 25% would choose the general practice podiatry track.
 
These additional data strengthen the case that as many as 25% of podiatric residents are being given training that they don’t wish to or intend to use, including working on cases that could have...

In my opinion, we have made the treatment of retrocalcaneal pathology far too complicated. It appears as though the performance of such surgeries has been directed by corporate-driven theoretical arguments rather than practical considerations. Whether Haglund’s deformity or insertional tendinosis, many patients may be successfully managed without the need to detach a major portion of the Achilles tendon, the use of orthobiologic materials to “enhance healing”, suture bridges, or anchors. When retrocalcaneal surgery results in complications, the undesired outcomes are often major in nature, resulting in prolonged recovery and disability. 
 
Careful and thoughtful surgical technique frequently allows the surgical interventions to be performed without the utilization of foreign materials with their added risks. When conservative palliative/accommodative therapy fails, keep it simple. Advise the patient that prolonged recovery and persistent edema may occur. Retrocalcaneal surgery is on my list of “do only if you have to” surgeries. Based upon my observations as an expert witness in retrocalcaneal surgical cases, I believe such complications are likely more common than generally appreciated, and can be of great significance, at times devastating. "Tantum  dilgentissame praecavete" should rule the day in the back of the heel.

Shoe surgery before foot surgery! I’m a lifelong Haglund’s sufferer, with supinator tendencies (cavo-varus, with plantarflexed first ray on right foot). I am lefty dominant, with shorter left side LLD (<1”). I played competitive sports until 65 and found viable footwear options. The first recommendation is longer shoe size. It makes a big difference. Double socks help reduce shear forces.

The excessive wear pattern of the posterior-lateral inside counter of the shoe is easy to see. Shoe-surgery consists of cutting a 1” hole in the heel counter centered at the point of maximum tenderness. The space can be left open or stretch Spandex material inserted. When not in closed-back footwear, open-back clogs provide desired relief.
