NYCPM Partners with Community to Provide for the Homeless
 
For the last several years, New York College of Podiatric Medicine (NYCPM) and the Foot Centers of New York partnered with Rescue Alliance, City Relief, and the Salvation Army to provide Podiatric Medical care to those in need. Anthony Iorio, DPM, Chairman of Community Medicine at the College and several student volunteered in this great event on Saturday February 24th in New York City. They provided counseling and much needed medical support to the most vulnerable members in our city.

Don’t Walk By started in 2008 is an annual winter outreach event. This outreach encourages New Yorkers to put into practice the parable of the Good Samaritan. Over the past decade, Don’t Walk By volunteers canvassed the streets of Manhattan and invited the homeless back to a host site... The Salvation Army on West 14th Street in NYC. While at the host site, guests are offered a hot meal, clothing, basic medical care, and connection to ongoing support.

CT Public Health Committee Considers Expansion of Podiatric Surgery
 
The Connecticut Public Health Committee heard testimony Monday regarding whether it should move forward on a bill to allow podiatrists to perform certain surgeries. HB 5196 would revise the scope of practice for podiatrists in the state to perform a broader range of surgical procedures. Proponents of the bill said that allowing podiatrists to perform these procedures would increase access for patients, especially for patients on Medicaid.

Kristin Winters, DPM, President of the Connecticut Podiatric Medical Assoiation (CPMA), spoke in favor of the bill and also called for full foot amputation to be added into the bill. “Our current amputation law is a proven obstacle to delivering complete care for our patents,” she said. Winters also pointed out that the bill would keep new podiatric doctors in the state. "This bill will also attract our residents to stay here. We train them, we want them to stay and practice in the state of Connecticut, and help deliver this care for patients," Dr. Winters said.

Never Ignore Painful Cracked Heels: TX Podiatrist 
 
Cracked heels, or heel fissures, occur when the dry, thick skin on or around the sides of your heel cause little splits. When this happens, skin on the bottom of the heel can develop into calluses, which occur from repeated friction or pressure to the same area, explains Suganthi Kandasamy, DPM, a podiatrist and an assistant professor in the department of orthopaedic surgery at the UT Southwestern Medical Center in Dallas.

“These calluses can build up and become thicker over time,” says Dr. Kandasamy. “As a person walks and places more pressure on their heels, these calluses can develop into cracks.” Painful cracked heels can vary from person to person, says Kandasamy. Some people may have soreness while others may describe their pain to be sharp, making it hard to bear weight on their feet, she adds.

LACPMA’s PRESENT Treasure Hunt Returns
 
Los Angeles County Podiatric Medical Association’s (LACPMA) annual conference, PRESENT Treasure Hunt, returned this weekend February 24-25 with podiatrists attending from several Western states including Hawaii! The annual Medicare and DME update lectures presented by Gabriel Halperin, DPM and Paul Kesselman, DPM, respectively, were a couple of the highlights of the combined scientific and practice management curriculum presented throughout the conference.

8 Billing Tips for Practices
 
The implementation of these billing tips can be a transformative step towards improving the financial health of your primary care practice. 
 
3. Accurate coding of symptoms vs. diagnoses
 
Use the appropriate diagnosis codes for billing, avoiding the use of symptoms as primary codes. Insurance companies typically require specific diagnoses for proper reimbursement. Primary care providers or their billers often make the mistake of coding for the symptom vs. the underlying diagnosis. Take for example, a patient who comes in seeking help for a sore throat. If the diagnosis is streptococcal pharyngitis, then the coding needs to be for streptococcal pharyngitis (bacterial or acute) in order for the insurance payer to accept the claim.

One aspect of the current surfeit of schools of podiatric medicine we should consider is how an average applicant pool being diluted by so many schools weakens the whole. I can only speak of Temple, as I am good friends with someone who instructs there and reveals that the number of applicants and the number of admitted students is too low to sustain a tuition-driven institution.
 
I understand that at some point Temple University dropped the hammer and let the school of podiatry know that it has to start being a neutral entity and not be financially subsidized to keep it afloat. I know of no established time frame, but if Temple may be contemplating the long-term fate of the Philadelphia school (my alma mater) I can only imagine that...

Dr. Scherer's article, which I have not yet read, as summarized in PM News, reported a 59.4% positivity rate for clinically diagnosed onychomycosis. In this very large sample of cases, I am sure there are variables that were very difficult to control. For example, specimen collection techniques are so varied that I am certain this was a factor in the end results. However, I fully understand and support wholeheartedly the point of the study...it's NOT all onychomycosis.
 
Laboratories publicly promising that they will find it and the expectations from practitioners that they must find it has not helped our collective...

RESPONSES/COMMENTS (PUBLISHED PODIATRISTS) - PART 1 B

I would like to state my findings and observations concerning the comments by the esteemed doctors Scherer and Bakotic. They wrote that 50% of dystrophic nails are, in fact, onychomycosis and that onychodystrophy can have varying etiologies, both infectious (fungal and bacterial) and non-infectious (microtrauma, psoriasis, lichen planus, melanonychia, etc.). They state that is the precise reason why laboratory confirmation prior to definitive pharmaceutical therapy is recommended.
 
Unfortunately, while what they state is accepted dogma in the academic world, the reality is I have found that by using my treatment for “ugly” nails, whether fungal or onychodystropic, the fact remains that they almost all can be cured to perfectly normal nails. This is why I stated the testing does not matter anymore and is a waste of time and money.
 
I will offer as proof photos of my patients' nails to PM News so everyone can see for themselves. I will send in dystropic 2nd and 5th toenails that everyone says is from micro-trauma and very deformed nails that until now most docs would say are impossible to treat. I hope our profession takes note and gets behind the idea that what was once dogma concerning “ugly” nails is no longer true.
