The American Podiatric Medical Writers Association (APMWA) has announced its 39th Annual Student Writing Competition.

All papers MUST be non-technical in nature and original. (NOT AI generated) Appropriate subjects include practice management, ethics, law, education, healthcare, or any topic that would be suitable for a lay publication. Entries shou;d be submitted as a Word (.docx) file.

There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.

CO Podiatrist Discusses Rhabdomyolysis in Hikers
Jared Menchin, DPM, a Colorado-based podiatrist, says rhabdomyolysis is not common. “There are approximately 26,000 cases of rhabdomyolysis reported in the U.S. per year,” he says. Comparatively, there are about 41 million cases of the flu every year, with up to 52,000 resulting deaths. With several incidences of rhabdomyolysis taking place on long trails, it’s clear there’s a potential connection between backpacking and the condition. Dr. Menchin explains that the condition materializes at different points for different people.

Rhabdomyolysis can also be much more difficult to detect than your standard flu. In some cases, it’s asymptomatic. Dr. Menchin says. Hikers should look out for muscle pain, weakness, swelling, dark, tea-colored urine, and general fatigue. “If you experience signs or symptoms of rhabdomyolysis, it is crucial to seek immediate medical attention,” Menchin says. “Early diagnosis and treatment can help prevent complications and improve outcomes.”

Unnecessary and Negligent Calcaneal Osteotomy - Oregon
Case Summary: Plaintiff, female age 41, employed as administrative personnel. Plaintiff was a patient of the Defendant's. Plaintiff contended the Defendant performed a calcaneal osteotomy that was unnecessary and negligently recommended. The surgery failed to heal, and Plaintiff alleged the Defendant was negligent for failing to discover malalignment and non-union of the osteotomy to correct it in time. Plaintiff has had three additional revision surgeries to try and correct the problem.
Result: Plaintiff verdict for $3,000,000 (No new trial presently pending).

Query: Plaque-Like Calluses on Weight-Bearing Surfaces
A 19 year-old Hispanic female patient presented with plaque-like calluses on the weight-bearing surfaces (balls and heels) of BOTH feet which started a month after starting a new job at a memory care facility. Her new job consists of helping folks with bedtime routines, (wears/changes gloves routinely), and wiping/cleaning residents and repositioning them as needed. Shortly after the skin changes on her feet occurred, her fingernails started to become discolored, pitted, dystrophic and thickened with dark longitudinal streaking, starting with the right thumb nail. Her toenails look fairly normal with no obvious signs of fungal infection.

Plaque-Like Calluses on Weight-Bearing Surfaces |
This seems to be auto-immune driven, however, given the sudden onset with the new job and heavy use of gloves, I have some concern for infection, either fungal or bacterial. We have started her on antifungals to see if this helps. A dermatologist has suggested hereditary palmoplantar keratoderma, recommending symptomatic treatment with inserts and urea cream. A biopsy is being considered, but first utilizing supportive inserts to reduce stress/weight on the weight-bearing surfaces. Additionally, she will start applying urea cream to one foot, triamcinolone to the other, and see what results we get. Of interest, the patient also has a history of loop bowel in addition to scoliosis and eczema. No skin changes to the knees, back, neck, etc., although she has a very dry scaly scalp. Psoriasis is a consideration. Input and suggestions are appreciated.

Query: ANTHEM Blue Cross Payments for Orthotic Casting
I would like to find out from my colleagues around the country if they are seeing the following:
In the October, November, and December issues of the California Podiatric News Magazine (CPMA) , there was a section on page 21 about Anthem Blue Cross Best Billing Practices for orthotics. In the second paragraph, it discusses using the code S0395 for the casting code. According to CPMA and their legal section, Anthem signed off agreeing to this.
However, we have been doing that for a number of months and while Anthem initially does pay the code, they then come back a few weeks later demanding a refund stating it is inclusive in L3000. They then, if not reimbursed immediately, begin offsetting other payments and deducting the amount they paid from future payments until they have been fully reimbursed. We have forwarded examples of the payments and then the recoupment requests but have not heard anything. Has anyone else been having these issues?

I'm glad to see someone else is interested in seeing the DPM's role go from cutting nails to overseeing the management and treatment of nail pathology. I've opined in this forum a few times now that we should not be spending our time on the physical practice of cutting nails. About 30 years ago, I spoke with the Executive Director of our state society (PPMA), Michael Davis, about the possibility of having podiatric assistants cutting nails and still being able to bill for that. At that time, he told me that podiatrists were not legally able to supervise such activity in Pennsylvania, let alone bill insurance for it.
When I asked how difficult would it be to change that, he said that it would probably have to involve state legislative changes, which would be very difficult. I did speak with a Board member also, a DPM, and he felt that there would be very strong resistance from the podiatric community. Most DPMs were very content to ...
