UK Podiatrist Discusses Causes of Smelly Feet
Smelly socks, sandals and shoes, and foul-smelling feet are all the result of bacteria breeding on sweat and building up in the foot area. Consultant podiatrist Matthew Fitzpatrick, spokesman for the College of Podiatry, says no one has inherently smelly feet and that it all comes down to poor hygiene.

Not everyone is going to use an insert along with their orthopedic shoes. However, if you know you will, podiatrist Bruce Pinker, DPM, founder of Progressive Foot Care, says you need to keep that in mind when you’re shopping for orthopedic shoes. Because the point of orthopedic shoes is to provide support to your whole foot, podiatrists don’t recommend wearing high heels—which put a lot of strain on the front and arch of your foot. “Women should avoid shoes that have heels higher than two and a half inches,” says Dr. Pinker.

I agree with Dr. Steinberg. From what I have observed and have been told, the curriculum in our schools has not kept up to the standards for today's podiatric physicians and surgeons to become successful in practice. A glaring, practical example applies to information that pertains to a podiatrist's personal financial success. It is my understanding that there is little if any classroom content on practice management and "The Business of Podiatry."
I don't find this surprising. Having been a podiatrist for 66 years, I have observed a long-time reluctance of our schools to include practice management and podiatric medical economics educational curricula. For example, right now, our entire healthcare reimbursement system is dramatically changing from a fee-for-service model to a value-based care model. In fact, in a recent issue of APMA News, there was a very informative article advocating podiatric practices transition from fee-for-service to value-based care.
I doubt whether current podiatry students are being informed that their future insurance reimbursement for spending an hour of preventive services, providing chronic care management (CCM) for a patient who suffers from lower extremity conditions such as diabetic peripheral neuropathy and peripheral artery disease. This already surpasses what they can earn in an hour for providing most surgical procedures that include post-operative care. Knowing this might influence a student to not become financially dependent upon providing major surgical procedures, especially if they are elective. Or, maybe even go, “Direct Care” and not accept anything other than cash payment. Who would deny that, forewarned is forearmed? Our students deserve to be better prepared for the future.

Thanks Dr. Jacobs for your comments. With my concierge practice, I take the time to treat each patient personally and follow them up very carefully. When I say follow-up for fungus nail treatments, I include a 3 month follow-up as well as a 12 month at no charge. Since the patients paid significant funds, we have a high number that return for their follow-up appt. We take photos 100% of the time at each visit during the treatment and with follow-ups. I have spent over 2 decades trying to cure fungus toenails, and I can say with great pride that I have achieved that goal. I have successfully treated thousands of patents now. When I say cured, I mean cured. I use a 4-step process.
The first step is using a laser or lasers which I have developed a proprietary protocol for. It took me years to figure out what settings and times to use. The second step is meticulous...

Dr. Ivar Roth may indeed have subjective success in treating onychomycosis without laboratory testing; however, this is certainly not the case universally. Essentially, all dermatology and podiatry peer-reviewed literature on this topic have demonstrated that only ~50% of dystrophic nails are, in fact, onychomycosis. Visual assessment by a physician, while an essential part of a patient encounter, is unreliable at discerning an accurate etiology of the nail changes observed, as demonstrated by multiple referenced studies. The level of precision required to render an accurate and proper diagnosis of onychomycosis is best obtained through laboratory confirmation following appropriate clinical evaluation.
Onychodystrophy can have varying etiologies both infectious (fungal and bacterial) and non-infectious (microtrauma, psoriasis, lichen planus, melanonychia, etc.), which is the precise reason why laboratory confirmation prior to definitive pharmaceutical therapy is recommended. Additionally, terbinafine resistance, which requires laboratory evaluation to identify, is an omnipresent issue with a current rate >5% in those with confirmed onychomycosis in the U.S., although the rates overseas are far higher and increasing. It is for these stated reasons that laboratory confirmation, no matter which laboratory or testing methodology is chosen, is recommended by the CDC, American Academy of Dermatology, British Association of Dermatologists, and others. Our recently published paper with 96,293 samples supports and reinforces these recommendations. It goes without saying that the accurate characterization of nail pathology utilizing evidence-based medicine, no matter the etiology, allows for individualized treatment plans that ultimately result in improved patient outcomes and overall satisfaction.

I was somewhat taken aback to read that almost half of the profession is paying over $450/month for EHR services. In every journal I read, at every meeting I go to, and in personal correspondence, this topic is never discussed. It might be interesting to begin a discussion concerning the various ramifications of EHR cost and implementations.
Have any readers been through the process of your program being sold without your knowledge until you get a call or email stating that your program has been sold to another EHR provider? Then, you must continue with the new provider or find a different one. This topic, and those related to it, should be on every podiatry program in the country. We might begin here. If you discuss a particular EMR provider, it’s probably best to delete the name.
