NJPMS Executive Director Retires After 23 Years
With 23 years at the helm of the New Jersey Podiatric Medical Society (NJPMS), Executive Director Herman Hammerschmidt retired on February 28, 2022. During Mr. Hammerschmidt’s tenure, NJPMS became a leading economic, political, and professional Society. The Society has enjoyed 20 years without a dues increase and has provided scholarships in excess of $180,000 through its Jonathan Robertozzi Memorial Foundation.

At the February 2022 meeting, The NJPMS Board extended a heartfelt congratulations, warm appreciation, and best wishes both verbally and written on a plaque presented to Mr. Hammerschmidt.

LA Podiatrist Discusses Diabetic Foot Complications
Serious foot conditions can be part of diabetes. For instance, nerve damage—or neuropathy—is a loss of sensation that can occur due to uncontrolled diabetes, says Irene Evuleocha, DPM, a double board-certified podiatrist practicing at Ochsner Health Center in Baton Rouge, Louisiana. "[Neuropathy] can then result in calluses, which can lead to diabetic ulcers," she explains. In severe cases, this can also cause foot infections that may even lead to amputations if not managed in a timely manner.

Other foot problems caused by diabetes can include hammertoe deformities due to an imbalance with tendon and nerve muscle, poor circulation that may cause irreversible damage such as gangrene, and skin disorders like cracked skin or fungal infections due to an immunocompromised state, says Dr. Evuleocha.

FAPA 2022 Seminar Was "Sun"sational
The 26th Annual FAPA “Seminar in the Sun” took place from February 19-22 at the beautiful all inclusive Iberostar Maya Resort in the Riviera Maya, Mexico. FAPA’s scientific chair Gary Stones, DPM was pleased to feature educational lectures from past APMA president Matthew Garoufalis, DPM, Podiatry Management editor Barry Block, DPM, JD, and dermatopathologist Steve McClain, MD. Special memorial tributes were made to FAPA's former educational director, Al Kanegis, DPM .

Sev Hrywnak, DPM, MD, presented two lectures at the Vascular Surgeons meeting in Chicago, Illinois February 5-6, 2022. The first lecture was “DNA Methylation and Wound Care “and the second lecture was “Epigenetic Regulation of Cellular Function in Diabetic Ulcerations”.

“There have been many advances in the therapeutic approach to wound care and it starts with getting an understanding of metabolic cellular pathways,” stated Dr. Hrywnak. “Proper wound care is not just what you place on a wound but also obtaining the metabolic status of the patient through elaborate lab analysis and treating accordingly,” Hrywnak added.

Coding Mistakes That Cause Practices to Lose Revenue

3. Inaccurate Capturing of Patient Status
The confusion between whether a patient is new or established, which should usually be established at the front desk, can lead to lower payments if the patient’s status is not properly captured.

Query: Removal of Syndesmotic Screws
I have a patient who suffered a Maisonneuve fracture about six months ago. This required surgical repair. I placed two syndesmotic screws and the patient healed uneventfully. He has returned with limitation with range of motion at the ankle and he would like the screws removed. I took a new x-ray and it showed that one syndesmotic screw has broken and the other screw is intact. Neither screw has migrated.
I am going to remove both of the screws. Of course, I will not be removing the broken portion that is buried in the tibia as it is not causing any symptoms. My question is regarding the coding of the removal of these two screws. With the new guidelines, how does it affect the coding of this situation?

Response: The proximal screw is structurally broken and no longer serves its intended purpose. Therefore the guidelines define this as a foreign body. The distal screw an implant that has not moved, is not structurally broken is still serving its intended purpose, and does not present as a hazard. According to the above guidelines, that qualifies it as a foreign body for coding purposes. I am not saying that I like the 2022 guidelines, but that is what they are instructing us to do.
