This marks the 5,000th issue of PM News. For the past 20 years, it has been our privilege to provide the podiatric community with a daily newsletter containing news, coding and billing information, as well as an interactive forum for a discussion of issues important to this profession. In short, we have attempted to keep everyone on the same page. We are proud to be the longest running periodical of this type serving any profession.

Two NYCPM Faculty Members Comment on Yankee Shortstop's Ankle
 
Can Derek Jeter, who will turn 40 in June, with his particular set of recent maladies, play an effective 150 games at his sport's most athletically demanding position? Sports Illustrated asked Drs. Rock Positano and Josh Dines. While it must be emphasized that neither Positano nor Dines has examined nor treated Jeter, both know baseball, and both know feet. Positano is a renowned foot specialist, who in the 1990s finally cured Joe DiMaggio of the heel pain that had plagued him late in his career, and ended it prematurely. That Positano is the founder and director of HSS' Joe DiMaggio Sports Medicine Foot and Ankle Center is one indication of the Yankee Clipper's gratitude. Dines is an orthopedic surgeon and an Adjunct Instructor at the New York College of Podiatric Medicine. He recently collaborated with Positano and two other physicians on a textbook called Foot and Ankle Sports Medicine.

The doctors agree that, as far as the integrity of Jeter's ankle, past events are not indicative of future results. That Jeter suffered a new crack in the joint last spring, when he resumed baseball activities less than five months after sustaining the initial injury, does not mean that he is in any particular danger of re-fracturing it in 2014. Positano and Dines have little doubt that the cascade of injuries Jeter suffered during his multiple aborted comeback attempts last year all stemmed from his still weakened ankle, and could also have resulted from his eagerness to return to the field. "The foot and the ankle are primarily shock absorbers, and protect everything above them," Positano said.

MD Podiatrist Comments Before FDA Advisory Panel on Iontophoresis Devices

A U.S. Food and Drug Administration (FDA) advisory panel has recommended easing regulatory restrictions for some iontophoresis devices. At a hearing that involved discussion, but not votes, on February 21, the FDA's Orthopedic and Rehabilitation Devices Panel of the Medical Devices Advisory Committee recommended that the FDA consider a less-restrictive category for iontophoresis devices used with tap water to treat hyperhidrosis, and for other local drug administration. However, the panel felt that the FDA should retain the current more restrictive classification for iontophoretic systemic drug delivery.

LeBow, DPM, in private podiatric practice in Baltimore, Maryland, said, "[With] a syringe, we all know what 2 cc of fluid is, the dosage that 2 cc represents, and where we're depositing it. With this device, we don't know how many ions are transferring into the skin or the potency or the depth of where they're being delivered."

ASFP Members Attend the Academy of Forensic Sciences Meeting

American Society of Forensic Podiatry (ASFP) President John DiMaggio, DPM, Secretary David Agoada, DPM, and Treasurer Bryan B. Kagan, DPM attended this year’s annual American Academy Forensic Sciences scientific meeting this past week in Seattle, Washington. Approximately 4,000 forensic professionals presented the most current information, research, and updates in forensic medicine.

More than 900 scientific papers, poster presentations, and workshops were presented. Approximately 166 exhibitors showcased cutting-edge forensic technology and forensic services. The American Society of Forensic Podiatry is committed to working with other forensic specialties to further the science of forensic medicine.

Why use Botox if your plan is to do a pan-talar arthrodesis? I think a Medshape IM rod ankle/STJ fusion is perfect for this patient. It is the only 4th generation IM rod on the market with Nitinol technology for continuous compression. You can do a TAL along with your fusions. The Botox is temporary and will mask true intra-operative contracture issues that need permanent release.

Those who believe that they can sneak by and live with lower limit coverage for professional liability insurance are playing a risky "board game" at the Las Vegas crap tables, with the exception that they don't even have a chance of winning the pot.  The Editor pointed out that the DPM is personally responsible for the excess jury verdict. Successful plaintiffs hire special investigators to locate and expropriate a defendant's properties for the settlement.
 
Sometimes, these investigators also get a share of the recovery, depending on the agreement they have with the plaintiff. For this reason, they are "highly motivated." It is illegal to try to hide proceeds from legal recovery. Doctors who think that they may have a risk of going above their coverage limit should consider hiring their own private attorney to help monitor and control the process. You have every right to do this, no matter what your carrier says.
 
My sense is that for the recent situation of a coverage being exceeded by the settlement, there was no separate attorney to represent the podiatrist involved. Had there been, the result may have been quite different. The Editor's comments are on target. YOU will be responsible to cover what is not covered by your carrier limits. This is not legal advice. I am not an attorney - all the more reason why you should hire one if you think you need one.

WOW - twenty years of getting out the best and most read specialty publication in any medical discipline is an accomplishment that is epic. Twenty years ago, life was different: · Justin Bieber was not born · David Armstrong had hair · and Barry Block started PM News. 
 
Twenty years later, Barry has been our “Jay Leno and 60 Minutes” all wrapped into one. He has served as editor, overseer, and conscious protector of our profession. I‘m sure I speak for most podiatrists in the world when I say thank you. I feel proud to know you and call you my friend. 
 
*An isogram is a term for a word without a repeating letter.

I agree with Dr. Connor's comments, but the footwear solution he offered was very vague: "Lessen the load on the knee by getting something underneath you that reduces the load.” 
 
I believe that in the past, podiatry has been overly focused on controlling inversion and eversion of the heel, and not on controlling heel and extremity loading. I hope we continue to move forward with our biomechanical solutions, even though we generally have been forced to accept the "foam solution" from the average standard footwear makers.
