Give Your Toes More Space NY Podiatrist
Women’s feet tend to get bigger in midlife, especially after pregnancy, says Brad Schaeffer, DPM, a New York City foot surgeon at Sole Podiatry NYC. He adds that most women he sees are wearing the wrong shoe size.
|
Dr. Brad Schaeffer |
“This can have a ripple effect on your body and affect your knees, hips, and lower back,” he says. Seek out a shoe store or a podiatrist to get properly measured.

INTERNATIONAL PODIATRISTS AND SPORTS MEDICINE |
UK Podiatrist Discusses Nail Injuries in Roller Derby
"Since roller derby is a growing sport, podiatrists like me will likely see more foot injuries from players, including bruising under the toenails and lost nails. Lost toenails are a common injury among these athletes due to the frequent quick turns and stops required during events," says Dr. Benjamin Bullen, Lecturer in Podiatric Medicine, University of Galway.
|
Dr. Benjamin Bullen |
"It is not at all uncommon for people to have one foot longer than the other, which will increase pressure in the roller skate, particularly on the big toenail. Silicone toe protectors can be worn to reduce this pressure; however, they may also reduce the amount of available room in the toebox – front of the shoe. Appropriate roller skate fit is essential to prevent such nail injuries and, much like other sports, shoes that are too short or too long may lead to recurring nail issues, such as “runner’s toe” – also known as a subungual hematoma – a painful, blackened toenail caused by repeated trauma," says Dr. Bullen.

RESPONSES/COMMENTS (CODING & BILLING) - PART 1A |
From: Donald R Blum, DPM, JD
I think more information for this query, 28300 and 28730, is necessary. What were the ICD-10 codes used for the different procedures? Were the correct modifier(s) used? Finally, what happened after the appeal was made with an operative note?
Donald R Blum, DPM, JD, Dallas, TX

|
RESPONSES/COMMENTS (CODING AND BILLING) - PART 1B |
From: Steven J. Kaniadakis, DPM
In my experience, certain coding "experts" conveniently leave out the diagnosis (Dx) code that is supposed to directly correspond to the correct AMA CPT code. This may address the issue why each CPT code is being used for a different diagnosis code, and the CPT code is for a procedure performed to treat and repair a different condition.
If there's a different diagnosis code associated with the particular CPT code(s), each different diagnosis code must be placed in the very first position of the four diagnosis blocks. Then it might be legitimate in your case.
Therefore, I maintain there remain certain experts who use what is or what is not included in the CPT procedures as "bundles or packages" of CPT codes; however, they conveniently leave out the important diagnosis code, which is directly being treated in the first position of the submitted form.
Steven J. Kaniadakis, DPM, Saint Petersburg, FL

RESPONSES/COMMENTS (NON-CLINICAL) |
From: H. David Gottlieb, DPM
Change is constant in life and in podiatry. The APMA used to be the APA, and before that, NAP. The degree used to be DSC (and PodD) before DPM. Podiatry schools in the early '80s had as their mission "to prepare graduates for practice.'' Now, their mission is to "prepare graduates for residency." Many states did not require residencies for a license. Now most do, and I expect very few, if any, require only a one-year residency.
Providers on this forum complain incessantly that the APMA is not addressing the current "crisis"' facing our profession… that they need to do something to be more aggressive… that the current staff needs to "wake up''. So change is being made. First, someone with an MBA is hired to lead the organization - someone with a fresh perspective who can evaluate APMA as a business, which it is. After assessing the business of APMA and determining what "needs" need to be met in order to be successful, appropriate changes are being made to accomplish those goals. APMA is no longer doing business as usual.
So, as I see it, APMA is first being criticized for not making changes to address the current crisis but when APMA starts to make changes to meet the challenges facing this profession, TODAY they get criticized. Give me a break. Change must be made. Change IS being made.
H. David Gottlieb, DPM, Baltimore, MD
