U.S. Air Force Podiatrist Honored
U.S. Air Force Capt. Sheryse Mobley, DPM graduated from Ellison High School and is now successful in her career with the military. Dr. Mobley was honored recently as the only active-duty podiatric surgeon in Alaska. “She provides lower extremity surgical care to all active duty, dependents, and eligible veterans housed in the entire state of Alaska,” according to the Joint Base Elmendorf-Richardson’s 673d Medical Group, which highlighted its healthcare services recently.

Dr. Mobley earn a doctorate in podiatric medicine from the Kent State University College of Podiatric Medicine and was commissioned into the Air Force before going on to complete a three-year residency in Baltimore Veterans Affairs and Sinai Hospital of Baltimore at the Rubin Institute for Advanced Orthopedics. And, if that isn’t enough, the Ellison grad was honored by the Killeen Independent School District Alumni Association at the annual award ceremony in March 2023 as one of five 2023 Distinguished Alumni.

FAPA 2024 “Seminar in the Sun”
The FAPA Fraternal Association (FAPA) held its 28th annual scientific seminar Feb. 19–21. The venue for this year’s event was the Iberostar Rose Hall Resort and Spa in Montego Bay Jamaica. Attendees from all corners of the country and Canada were treated to stimulating lectures on Biomechanics/Podopediatrics, Wound Care, and Dermatology for 16 CME contact hours.

8 Billing Tips for Practices
The implementation of these billing tips can be a transformative step towards improving the financial health of your primary care practice.
1. Correct data entry and demographics at check-in
This seems elementary but surprisingly many practices don’t properly train their front desk about the importance of this first, crucial part of the billing cycle. Accurate data entry during patient check-in is critical. Gathering complete insurance and demographic information helps ensure proper billing and reduces potential claim denials. Make sure that you always collect a photo of the patient’s insurance card, and most importantly, get a photo of the back of the card. The back of the card is often more important for billing than the front.

Not only are there too many schools, but the curriculum has not kept up to the standards for today's podiatric physicians and surgeons to become successful in practice. Colleges are not teaching the courses necessary for podiatrists to sit for the USMLE board exams, keeping the profession under the hallux of others. I suspect the first podiatric medicine college offering a better curriculum will get far more applicants. Let the race begin.

According to an article published on March 27, 2023, in collegiategateway.com, "the latest figures put overall medical school enrollment at 96,520, an almost 18% increase over the past decade. This surge has been made possible by increasing class sizes, the opening of more medical schools, and government intervention to add Medicare-supported graduate medical education positions, and is a welcome response to the projected shortage of physicians in the coming years."
While those statistics apply only to MD schools, on the March 21, 2023 informational article on tiberhealth.com, "There are very few podiatry schools. According to the American Association of Colleges of Podiatric Medicine (AACPM), there are only 11 DPM programs in the U.S. as of 2023. As a result, the overall number of applicants is lower than those who apply to MD programs. The AACPM reports that there were just 910 applicants to DPM programs in 2021, a tiny fraction of the 62,443 people who applied to MD programs that year."
So, it still begs the question to ask what is our profession, the APMA, and all of the other podiatric professional organizations doing to increase awareness to the public, colleges, and universities on the career of podiatric medicine and surgery in order to increase the number of applicants?

If there is one thing that schools are good at, it's taking money. With the federal government fully subsidizing all education at ANY cost, there is no incentive for schools to consolidate. And if you already have an MD or DO school set up, why not cast a larger net to get more money?
Perhaps the conversation with applicants goes something like this, "Oh, I'm sorry, you didn't have good enough grades to get into our medical program. Why not apply to our DPM program? You can be a foot and ankle surgeon! By the way, that will be 60,000 dollars a year for the next 4 years."
Who wouldn't start a program? 90% of the classes are the same. You only need to hire one or two podiatry professors and use the existing machinery already set up for your med students, and your med school now makes an extra million a year. Until CPME denies new schools from popping up, the number will continue to grow. Would that be a restraint of trade violation? Probably. So we will keep adding more schools as numbers of applicants dwindle to a point it's no longer sustainable, and podiatry gets absorbed by the allopathic and osteopathic professions.

This would be a great time, if Congressman Brad Wenstrup, DPM would respond, showing the value of podiatric physicians on the national level. It could show the American Medical Association that letters similar to what they sent to Mississippi will be considered anti-trust and a restraint of trade, and may trigger a congressional hearing if their actions continue. A letter from the president of APMA is nice, but I’d bet a letter from a congressman would be better.

Dr. Roth is correct, both he and most of our colleagues can diagnose and treat onychomycosis without the need for expensive lab testing. There is a practice management perspective on this. In our geographic area, some insurance companies will not pay for a topical and/or oral antifungal agent unless there is lab proof that the patient has mycotic toenails. Many years ago, our state podiatry society and possibly even the APMA fought this battle and convinced many insurance companies that a doctor's clinical assessment is good enough without the need for lab testing.
Unfortunately, we are seeing cases cropping up where an insurance company will require prior authorization and will reject the prescription unless they see documented lab proof that the patient has fungal nails. A recent example is where an insurance company demanded lab proof to approve an RX of oral terbinafine. McKesson charges $7.98 for a bottle. Even with the add-on to have a pharmacist make it up, there is no way that the insurance company is making a profit by making us order a lab test.

While I can neither support nor refute Dr. Roth’s claims to a 98% success rate treating onychomycosis without a culture, I do take offense to Dr. Jacobs’ short, off-the-cuff condemnation of those of us who do not accept insurance. A lot was said in those couple of sentences. First of all, in a profession with a long history of struggles (which continue to this day), the last thing we need is someone coming along and dividing the profession into “them” and “us”. This same thing happened back when...
