Cushioned Shoes Protect Middle-Aged Runners: NY Podiatrist
 
Podiatrist Dr. John Connors said he had been seeing an increasing number of patients who had tried to adopt minimalist running shoes. “People say to me we weren’t designed to run with trainers,” Dr. Connors says. “But we were also not designed to run on concrete and macadam.” Connors says that when you run, you are hitting the ground with four times your body weight, and hard surfaces such as concrete cannot absorb the shock. So the force goes right back up your leg. “Minimalist shoes are putting more stress and strain on the Achilles tendon, and more stress on the bones of the foot and leg,” he says.

“I’ve been seeing a spike in Achilles injuries because of the eccentric load being placed on the tendon,” says Connors, who was physician to the U.S. team at the London Olympics. He believes middle-aged runners need to take special care, even if they are frequent exercisers, because the cartilage in their knees wears down through normal use. His solution? “Lessen the load on the knee by getting something underneath you that reduces the load.”

PA Podiatrist Receives Ilizarov Award of Excellence at IEFS
 
Patrick R. Burns, DPM, FACFAS was awarded the Ilizarov Award of Excellence at the 8th Annual International External Fixation Symposium (IEFS) sponsored by the University of Texas Health Science Center in San Antonio, Texas. Dr. Burns practices in Pittsburgh, PA, and is an Assistant Professor of Orthopedic Surgery and the Podiatric Surgical Residency Program Director, Foot and Ankle Division, at the University of Pittsburgh School of Medicine.

Burns' many years of service with expertise in external fixation and diabetic foot reconstruction to the Pittsburgh community, active research participation, numerous publications and book chapters, and resident education and training at the University of Pittsburgh School of Medicine have distinguished him as the honorary recipient of this year’s award. The 8th Annual IEFS attracted its largest crowd ever, with over 100 participants coming from across the United States and around the world. Next year’s 9th Annual External Fixation Symposium will take place on December 11-13, 2014.

Query: Responsibility for Malpractice Verdict Overage

A recent podiatric case was settled for over $600,000. The insured policy had a limit of $250,000 per incident. The balance of the settlement is $350.000 Who is responsible for the balance?

Editor's comment: PM News does not supply legal advice. Unfortunately, the podiatrist is personally responsible. This case underscores the need to have adequate malpractice coverage. Today, the minimum coverage for any podiatrist should be 1 million/3 million. The difference in premiums is insignifiant compared to the added risk. Opting for the lesser coverage is a matter of being "pennywise and pound foolish."

Here's a follow-up to the matter regarding BC/BS wanting their money back for a surgical procedure they deemed an "investigational or experimental procedure." I was under the impression after my conversation during a requested peer-to-peer interview with the DPM who initially reviewed the case that my claim should have "no problem being paid" if I just resubmitted a "corrected or addendum" operative report as to why a metallic implant had to be used during a joint salvage arthroplasty of the 1st MPJ utilizing CPT codes 28111 and 28126. 
 
I did just as he requested and the union insurance plan with BC/BS as its TPA stated that all claims paid past, present and future will not be paid and...

Darco® makes an  APBtm closed toe post-op shoe. Bledsoe's (Texas) also sells a fine cold weather boot cover (AL039000-B) for walking braces. It was my practice to make these shoes available to my patients in Alaska. My patients did not mind paying a few extra dollars, and we dispensed them from our place of service. Overall, these were better than open-toed shoes since they kept the post-op dressing relatively clean and less exposed to the elements.
 
Document the extra expense in a letter of medical necessity to the insurance company, and mention the decreased chance of post-op contamination, exposure, and infection (causing more money to the third-party payers). Ask your coders if a supply code should be used for these extras.
