Anthem Agrees to Fully Rescind Proposed Modifier 25 Policy
This past fall, Anthem Blue Cross notified physicians in several states (including California) that effective January 1, 2018, it would reduce reimbursement of evaluation and management (E&M) services billed with modifier 25 by 50%. The California Podiatric Medical Association (CPMA) immediately sprang into action and coordinated with the American Podiatric Medical Association (APMA), American Medical Association (AMA), California Medical Association (CMA), and the American Association of Dermatologists (AAD), along with many other state and specialty organizations, to push back on the proposed change.

Spearheaded by CPMA Health Policy Chair Franklin Kase, DPM, the hard push back initially resulted in Anthem walking back the policy and agreeing to a 25% reduction instead of the original 50% and with a March 1 implementation date rather than January 1. Believing the 25% reduction to still be unjust and onerous, CPMA continued a full-court press for the complete and total retraction of the policy. Yesterday, Dr. Kase announced that Anthem has agreed to fully rescind the new modifier 25 policy, saying, “This is In the name of Harry Goldsmith, DPM, who contributed greatly to this effort."

NY Podiatrist Established Scholarship for NYCPM 4th Year Students
After attending the Alumni Relations Association meeting at the New York Podiatric Clinical Conference in January, E. Giannin Perez, DPM, Class of 2011, was inspired to create a scholarship for fourth-year students going on externships. “During the meeting, there was talk about how we could support our students and I thought of what could have helped me when I was a student,” she said.

“Immediately, I remembered financially how difficult it was to expense my externships abroad – flights, room and board, rental cars, etc. Therefore, I decided to create a $1,000 scholarship to facilitate a student on their travels while they are on externships.” The new Dr. E. Giannin Perez Scholarship Fund will give $1,000 annually to a student with a GPA of 3.0 or above who is going on externships out of state.

CA Podiatrist Offers Tips on Treatment for Snowboarder’s Ankle
Snowboarder’s ankle (so named because the injury occurs 15 times more in snowboarders than the general population) is the common term for a fracture of the lateral process of the talus (LPT), which often occurs during a high-impact event, such as a car collision or a hard fall from high up. A talus fracture that does not heal properly can lead to serious life-long complications, including impairment of foot function, early onset of arthritis, and chronic pain,” says podiatric foot and ankle specialist Thomas J. Elardo, DPM, a board certified podiatric physician and surgeon in private practice in San Jose and Los Gatos, CA.

“Once identified as snowboarder’s ankle, treatment of the fracture will depend on how large and how displaced the broken fragments are. A small fracture that is in normal alignment can usually be treated without surgery. This typically means about 4 to 6 weeks of having the foot and ankle in a cast and on crutches with no weight-bearing on the leg. However, large and displaced fractures are typically treated with surgery. Sometimes this is highly problematic as the multiple fragments of bone can be like putting together the pieces of a difficult jigsaw puzzle," says Dr. Elardo.

MA Podiatrists Collect Socks for Homeless
Local podiatrists Dr. Angela Barnes and James J. DiResta partnered again with Anna Jaques Hospital for the Sox for Socks program and collected more than 500 pairs of socks in February for homeless people. Sox for Socks, named after the program’s largest supporter — the Boston Red Sox, collects socks for homeless men, women, and children through Boston Health Care for the Homeless.

Dr. Barnes said she believes that “one’s general health is intrinsically linked to our foot health, and protecting feet all year long is vital to our overall health. Winter in New England produces many cold days that upon overexposure to the cold and damp can result in frostbite and loss of limb for those less fortunate,” she said. “Providing socks from our drive is just one of many opportunities for all of us to help.”

CMS to Study MIPS Burdens on Clinicians
The CMS is inviting clinicians to sign up for a study to help it determine how burdensome it is to report under the Merit-based Incentive Payment System (MIPS) created by MACRA. The agency is specifically looking into clinicians' workflow under MIPS and if the system poses hardships on them. Interested clinicians can sign up for the study through March 23.
A limited number of clinicians who are not eligible for MIPS in 2018 will also be included in the study to compare their workflows. Depending on the results, researchers will make recommendations to eliminate the burdens and improve quality data capture, reporting, and enhance clinical care, a CMS spokeswoman said.

How to Combat Negative Reviews - Part 2
Concierge patients "love" their doctors. We survey patients annually and it's always the same. Just the very nature of having a patient commit to their physician and invest in their health with an annual membership fee means they value and respect their physician. If they didn't, they would not pay to be a member. These patients feel partnered with their doctor and will often gladly provide support.
Most doctors don't have a select group of concierge patients, but surely they have deep, long-standing relationships with patients. Perhaps you were able to help a special patient through a unique health situation? Your patients are grateful for the support you offer. Don't be shy! Ask them to post a review of your practice on Google or Healthgrades.

RE: Lack of Podiatric Authors in Our Journals

Can anyone explain why the authors of most of the articles in our podiatric journals are now written by mostly non-DPMs, foreign doctors, and others not in the field of podiatry?
I remember the days when all I ever saw was solely DPMs authoring in these journals that represent this profession. I recognize none of the names of those who are published in these journals. Does this mean that we, as profession, are no longer interested in research and writing about what we do anymore?
Have we just left innovation and outcome studies in the hands of others who have nothing to do with podiatry, or are these journals now just a platform for a totally new and international audience specializing in foot and ankle medicine and surgery?

The EPA-approved label on Benz-All says weekly. It does not say, however, if Benz-All is rendered ineffective by bioburden, meaning it might or probably needs to be changed earlier. The EPA requires that hospital disinfectants kill certain benchmark microbes and Benz-All does that. Many disinfectants, however, are tested for further levels of kill, and these are added to the label as "label dressing" since no disinfectant at this level kills everything. In other words, it's designed to improve sales. Keep in mind that there are many organisms it does not kill, but that is true of every disinfectant, no matter the brand. Only sterilization kills them all = use of an autoclave.
There are those that say disinfection is the okay-level of care for podiatry instruments unless you are performing invasive treatments. But any treatment can cause any level of invasion by instruments (in any type of care - podiatry or otherwise), even by accident, so any procedure must be performed under aseptic conditions Doesn't that call for sterilization of instruments in podiatry? Even in the offices? I am always shocked when I go into a podiatry office and there is no autoclave! But it happens way too often! In a survey of podiatry offices in my area, 4 out of 6 offices did not have an autoclave.

I for one would not want to be the doctor who gets inspected and has to then prove how much of this product I bought to match the patient load and requirements of changing it on a 2 to 4 week basis depending on whom you listen to.
In addition, I would then have to show proper paperwork as to the change in schedule and process of how instruments were utilized in the setting. Gas or steam sterilization seems to me a more definite way to avoid this quagmire with any board of any state.
