NJ Podiatrist Makes Fundraising a Father & Sons Event

Temperatures were hovering just above freezing. The wind was light, but it still stung. The sun was obscured by light-gray clouds. All in all, it was a regular Saturday in February. Except 2,400 people stripped down to their bathing suits, and charged the Atlantic Ocean's pounding gray-and-white surf to raise money in the 15th Law Enforcement Polar Bear Plunge for Special Olympics New Jersey.

Dr. Hal Ornstein with son's Tyler (L) and Zack (R)

Among those participating in this fundraiser were podiatrist Dr. Hal Ornstein of Howell and his two son’s Tyler, 12 and Zack, 10, who raised $2,700. The plunge is Special Olympics New Jersey's single-largest fundraiser. As of late Saturday afternoon, an estimated $800,000 was raised, said Lynn Schindel, a Special Olympic New Jersey spokeswoman.

PODIATRISTS AND THE LAW

WA Podiatrist Charged In High-End Bicycle Thefts Takes His Own Life

A Longview, WA man who faced multiple charges for allegedly stealing a string of expensive bicycles throughout the Northwest killed himself Saturday morning. Jacob Bos, a 35-year-old podiatrist, had been admitted to the psychiatric hospital after attempting suicide twice while out on bail. He had pleaded not guilty in Cowlitz County Superior Court to 12 counts of possessing or selling stolen bikes.

Dr. Jacob Bos

Bos had been convicted of a felony in Ohio, where he used another man's identity to buy furniture on credit. DNA from that case was used to link him to the Seattle bike theft.

RE: Antibiotics for Dialysis Patients (Jim Clark, DPM)
From: Multiple Respondents

I always defer to the nephrologist regarding any medications (save for topicals). I've cared for a large number of renal impaired patients over the last 2 decades and find these specialists to be extremely familiar with their patients (they see them three times per week) and most helpful regarding any potential solutions to the myriad of problems these truly debilitated patients face. Teamwork is the approach, and your personal concern will be rewarded with numerous referrals.

Kenneth L. James, DPM, MPA, Fort Worth, TX, docjammin@sbcglobal.net

From years of experience I have always found that it best to run all prescriptions past the patients personal nephrologist and document it in the patients chart. In many cases you might not be able to reach the patients personal physician and they will place you on the phone with another doctor. Always document the name of the doctor, time of the call and the decision. Nephrologists will often have different ideas on what antibiotics and what doses to allow. It is best to leave a nephrology decision up the nephrologist even if you many not agree with the decision.

Elliot Udell, DPM, Hicksville, NY, Elliotu@aol.com

I recall a little pocket reference called The Sanford Guide for Antibiotics had a section about dosing renal patients because they usually require smaller doses than normal and on certain days in regard to their dialysis treatments. The pocket reference is printed and updated yearly.

RE: DPM’s Still Prescribing Lac-Hydrin?
From: Marc Greenberg, DPM

I always give the surveys done annually by several prominent podiatry organizations a look and there is usually something in every survey that surprises me. I would like the 14% of DPMs who said they prescribe Lac-Hydrin (a non-supporter of podiatry) in the Podiatry Management survey to please contact a company rep and see if they will detail your office, and ask why when they tell you no. If you are an employee of one of our VA hospitals, you get a pass on this unless your formulary has alternatives.

Marc Greenberg, DPM, Dayton, OH, tripperdpm@yahoo.com

RE: Tear of Peroneal Longus (Stuart W. Honick, DPM, PT)
From: Paul Kesselman DPM

Many years ago, I repaired a severe calcified peroneus longus tendon. A few years later, the patient’s symptoms on the contra-lateral foot were severe enough to warrant surgical repair. Use of Graft Jacket Max Force, a product of Wright Medical, significantly reduced the amount of surgical dissection (no need for auto-graft), and significantly reduced her return to weight bearing activities as compared to her initial surgery.

I now routinely use Graft Jacket Max Force, for tears and calcifications of the peroneus longus, peroneus brevis tendons, Achilles, anterior and posterior tibial tendons and for augmentation during STATT and Jones teno-suspension procedures. Use of this material is quite simple, only requiring some re-hydration at the time of surgery.

Perhaps others can provide insight on other new products which have recently come on the market, which may also be applicable to this clinical scenario.

RE: ACFAS to Poll Members on Membership Policy
From: Gene Mirkin, DPM, Bruce G Blank, DPM

Way to go, PM News! You helped bring this issue to our attention. Now, colleagues, vote to keep us united.

Gene Mirkin, DPM, Silver Spring, MD, gsmirkin@comcast.net

I too would like to see that ACFAS members who resigned since the dual membership policy change be included in the poll. It would be a shame not to include former members who resigned over this policy not be represented in the poll since they would likely still be members if not for the policy change.

Bruce G Blank, DPM, Martins Ferry, OH, bruceblan@aol.com
