If you already deal with foot pain, then it’s particularly important to wear indoor shoes, says Bobby Pourziaee, DPM, “The High Heel Doctor” and a board-certified foot and ankle surgeon. “Supportive footwear reduces repetitive strain on the plantar fascia, distributes pressure evenly, minimizes arch collapse, and decreases muscle overwork,” Dr. Pourziaee says.

OH Podiatrist Joins Southeastern Medical Center Wound Care Center
OhioHealth Southeastern Medical Center has expanded access to specialized wound care services in Cambridge with the relocation of its wound care center and the addition of hyperbaric oxygen therapy, an advanced treatment for patients with complex, non-healing wounds. The expanded wound care program also includes new clinical expertise with the addition of Zack Hill, DPM, a podiatrist and podiatric surgeon who specializes in foot and lower-extremity conditions commonly associated with chronic wounds.

7. We Know You’re Tired
By the time Monday night cadaver lab rolls around, it’s usually after a long day of surgery or clinic—sometimes following a weekend conference and a late flight home. We’re all tired. It’s late, the room is cold, and, well… we’re literally working with cadavers.
An attending was once asked how he prevented burnout in residency. His answer: “Work harder. Nobody cares.” Harsh—and not exactly my vibe—but in this case, you kind of get it. Cadaver lab is one of those “suck it up” moments. You’re exhausted, but it’s an opportunity to learn that you’ll never get back. So, show up, stay engaged, and get something out of it.

My associate is on call for floor admissions from ER. What is the code for new and established patients that will go to the OR in 2-3 days. What if they are operated on in less than 24 hours? Can we bill for the E/M - or is it part of the surgical code? My codes: CPT 99222 initial 99232 established.

Response from Codinghelpline.com: CPT 99222 or 99232 are not necessarily the right codes for a diabetic patient who has an acute complicated problem that requires higher level care. Using the AMA grid, look at all levels of coding when the patient is seen initially and the same goes for the subsequent visits. As for "consults", you do not do consults anymore you are receiving a "referral". This is especially the case if the patient is with Medicare and with plans that follow Medicare. It is either initial hospital inpatient or observation care or subsequent hospital inpatient or observation care. Next concern - you asked was will you get paid? We can't assume a payer's role, but if they follow different specialty with NPI and different tax ID, you should get reimbursed for your services. If you are in the same group tax ID, then you would most likely not get reimbursed.

RE: Pathologization and Medical Care

Pathologize. The act of declaring a normal variation in anatomy or function pathological, that is, a disease. Pathologization is utilized to justify the need for intervention in order to control or correct the declared abnormality. The result is unnecessary care, the burden of unnecessary costs, and the potential harm or sequelae which may be associated with such treatments. Not infrequently, pathologization evolves in the absence of supportive studies, or evidence that the declared “pathology” has been demonstrated to negatively impact on the health of an individual. The theoretical disorder translates into profit for those treating these declared pathologies. The need for treatment of such entities is rapidly adopted by a subset of practitioners absent any vigorous proof that treatment is required. Marginal becomes major. Insignificant becomes significant. Unnecessary becomes necessary.
Pediatric foot types are a paragon example. Parents are informed that their child has an abnormality based upon no published science. If such variations in anatomy are necessary to identify, there is no evidence that such declared abnormalities result in patient harm. Over-screening leads to over-diagnosis. Over-diagnosis results in over-treatment. Unneeded treatment. There is a misrepresentation made to parents suggesting present or future harm to their children. As a result, a misguided demand for such services is created. Financial incentives rather than evidence-based science drive the process. Like-minded practitioners gather and form a collaborative for mutual protection and group think, as well as rationalization.
Why are pediatric “foot types” not discussed in textbooks of pediatric orthopedics? Where are the published studies supporting these theoretical abnormalities requiring treatment? Who are the individuals allowing these unproven theories to be presented for CME credits at seminars?
