PODIATRISTS IN THE NEWS - PART 1
Topical Antifungals Should Follow Oral Treatments:  MD Podiatrist

If your toenails have started to discolor or are becoming thicker and more brittle, chances are fungus is to blame. "While nails clear up in time by taking antifungal pills, your risk of a reoccurrence is high so I recommend a topical cream to help prevent it," says Timothy Swartz, DPM, chief of podiatry of Kaiser Permanente Mid-Atlantic region.

Dr. Timothy Swartz

Keeping your foot dry—and changing out of sweaty socks—can help prevent the fungus, which thrives in warm, moist environments and can invade your skin through tiny cuts or the small separation between the nail and nail bed, from rearing its ugly head in the first place.

PODIATRISTS IN THE NEWS - PART 2
Morton's Toe Increases Chance of Bunions, Hammertoes: CT Podiatrist

If your second toe is longer than your first toe (that's the case for an estimated 20 to 30% of Americans), you're at increased risk for bunions, hammertoes (ones that are bent downward), and even back problems due to how you distribute pressure throughout your body, says Robert F. Weiss, DPM, director of the Foot & Ankle Institute at Stamford Hospital in Connecticut.
Dr. Robert Weiss

"All the weight should push off your first toe, but when the second one is longer, it rolls and flattens, causing all kind of foot problems," he explains. If you do have this condition (called Morton's toe), talk to your podiatrist about the best kind of footwear for the shape of your feet, since ill-fitting shoes make the condition worse.

PODIATRISTS AND THE LAW
FL Podiatrist on Trial for Molestation Charges

A Weston podiatrist accused of molesting his 15-year-old babysitter in 2012 appeared in court Tuesday for the first day of his trial. The victim, now 19, was the state's first witness. She cried while speaking about the night of the incident four years ago. Dr. Augustine Bollo is accused of molesting the teen and then offering to pay her to keep quiet. The incident allegedly happened at his home in Weston, where she was hired part-time to babysit his four children. If convicted, he could face up to 15 years in prison.

RESPONSES/COMMENTS (ETHICS) - PART 1A
From: Tip Sullivan, DPM

Where I live, if you were to bill this as a service, you would be at risk for practicing outside of your scope. Many years ago this issue came up. As the president of our state podiatry association I wrote a letter to the attorney for our state board of medical licensure. The question was that if I were ACLS-certified and a patient had an MI in my office, could I as a podiatrist perform ACLS and administer the drugs required. The answer was NO! The reason was that I was a limited licensed practitioner and was not licensed to provide ACLS. It had nothing to do with qualifications or the ACLS certification. In this case, it seems as though you did not actually physically/medically do anything except be the good Samaritan that I am sure you are and help identify your patient's need.

RESPONSES/COMMENTS (ETHICS) - PART 1B
From: Howard R. Fox, DPM

Back in 2008, there was a similar incident on Staten Island that landed on the front page of the local paper. The press made it sound as if the cruel and heartless medical office only cared about collecting their co-pay, not the welfare of the patient. I personally know this was not the case, but the press sensationalized it because the patient called the press to get attention.

It might not be good for business to have a fire truck or ambulance flashing their lights in front of your office, but it’s much worse to get such horrible press that’s written to sell newspapers rather than inform the public of a non-news event. My advice is take the loss, write it off, and go visit your patient in the hospital to see how he’s doing.

Howard R. Fox, DPM, Staten Island, NY

RESPONSES/COMMENTS (ETHICS) - PART 1C
From: Jeffrey Kass, DPM

I opine that you should bill. You exemplified everything it means to be a doctor, and you did evaluate your patient and managed him appropriately. You can elect to put the diagnosis the patient presented with or filled out on your form and bill out an evaluation and management code. I want to congratulate you on your handling of the scenerio. You, sir or madam, are a fine doctor.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Larry Cohen, DPM

Regarding podiatrists joining orthopedic groups, I have to agree with Dr. Hall. There is both money and opportunity for the podiatrist. However from the orthopedic groups' perspective, they are getting someone to do their foot and ankle surgery at probably a fraction of the cost of hiring a fellowship trained foot and ankle orthopedist. Don't think for a minute that they are being altruistic.

RESPONSES/COMMENTS (NEWS STORIES)
From: Todd Lewis, DPM

The problem with awareness of our profession whether abroad or here in the USA, and what we can offer patients varies from state to state, city to city, private practice to private practice, and physician/practitioner to physician/practitioner. More importantly, the limiting factor is what a particular practice wants? What are their short- and long-term goals? Are they lost in a practice that doesn't want change. Do the podiatrists do research, go to several conferences regardless of CME credits, including but not limited to podiatry (i.e. vascular conferences), introduce new modalities/therapies, (i.e. Zimmer shock wave therapy, MLS laser therapy, TCC-EZ, PRP injections, Supartz injections, amniotic stem cell dressings and injections, just to name a few).

In addition, most private practices lack basic fundamental business knowledge because it's not implemented in medical school and that's a huge problem. Also, the correct codes and modifiers utilized when billing is another huge problem. It significantly affects how you get reimbursed/paid! My recommendation is hire a sound marketing agent to represent your practice, advertise, send out monthly newsletters, attend community fairs, join Top Practices, network, and join local chambers of commerce.

Todd Lewis, DPM, Wichita, KS
