American Academy of Podiatric Sports Medicine (AAPSM) Fellow in Training Andrew Yun, DPM from Menlo Park, California was selected to present his 2015 Graduate Student Fellow Poster on "Percutaneus Plating of Fibular Fractures" at the Joint Commission on Sports Medicine and Science Meeting held February 19-22 at the Peabody Hotel in Memphis, Tennessee. His poster was awarded first place by the committee. Dr. Yun completed his fellowship under Amol Saxena, DPM.

Sherlock Holmes Used Forensic Podiatry: UK Professor
Professor Wesley Vernon of Huddersfield University (which is fast becoming a center of excellence for forensic science) reckons there are less than a dozen trained forensic podiatrists in the country, but with increasing numbers of police forces using evidence from both footprints and the way people walk to solve crimes, Huddersfield has just launched the world’s first postgraduate degree in forensic podiatry.

Prof. Vernon has never lost his early love of puzzle solving and recently has been trawling through the Complete Sherlock Holmes for evidence of early forensic podiatry. “I’d read them as a a child, but had forgotten most of the stories, so when I got a Kindle, one of the first things I did was buy the entire lot,” he says. “In A Study in Scarlet, The Adventure of the Engineer’s Thumb, and The Adventure of the Solitary Cyclist, Holmes takes a stab at forensic podiatry. “He doesn’t always get it right. In A Study in Scarlet, he says you can calculate a man’s height from the length of his stride which isn’t actually true, but you do have to give Arthur Conan Doyle a lot of credit because, in many respects, he was way ahead of his time.

Bret Ribotsky: What major changes to Medicare will occur in the near future, and how should podiatrists prepare?
Harry Goldsmith: Medicare recently announced that alternative payment models will be going into effect in the next few years, impacting 50% of Medicare payments by the year 2018. We are talking about how Medicare payments will be made to providers; for example, through Accountable Care Organizations, medical home models, bundling of care based on episodes of care, etc. Ultimately, if implemented, the impact on healthcare providers will be profound. I believe this effort to “increase quality” in healthcare will result in significant upheaval in the delivery of healthcare services. Instead of benefiting, I believe Medicare patients will be the worse for it.

Forewarned is forearmed. DPMs should be reading everything they can on the CMS alternate payment models; they should be just as aware of any commercial copycat alternative payment models. Since much of the funding may shift from fee-for-service Medicare to these alternative payment models, podiatrists, if they currently are not part of ACO, should be looking into joining or at least looking into what it would take to be included. Since these payment models may be administered by hospitals and some large primary care organizations/groups, developing solid professional relationships could spell the difference of being in or out when specialists are invited to participate.

Testing Live Claims
Some providers are dual-coding dummy claims to test with payers, while others are sending live ICD-10 claims for real patients; it depends on the wishes of both the provider and the payer. University of Utah Health Care learned that using live ICD-10 claims gives a provider real-time data without using an ICD-10 era production environment, which most providers don't yet have. "That was the only way we could be sure the end result was what we really would see if we were live," says Connie Tohara, the provider's HIM director.

Query: Patients and Medicare Deductibles
A new patient was evaluated and treated for plantar fasciitis. She informed my front office that she had gone to her internist 10 days before and paid him $147 dollars, meeting her annual Medicare deductible. I billed Medicare for a new patient visit and a strapping. About one week later, I got an EOB from Medicare stating that the patient did not meet her deductible and is responsible for $105. She has no Medicare supplement. Obviously, the internist did not submit his claim until after I did, so I got stuck being owed the deductible. My question is, how do other offices handle this situation? Any advice would be greatly appreciated.

Response: One solution is to call the internists office, explain the situation, and try to work it out either between you two, or facilitating the overcharge refund to the patient, then getting it from the patient. That is the cleanest way to patch this up.
A second way is to have the patient call 1-800-Medicare in your office, and clearing it up with your input if necessary. Since you say you have a lot of problems with this, then I suggest that you avoid this in the future, either for specific patients or across the board, by holding your claims until other doctors eat the deductible for you.

A podiatrist CAN report on a Measure Group. CMS IS auditing doctors who submit PQRS data. Please make sure you have all of the necessary documentation in place should you be audited. Read the requirements for each measure in detail. You cannot have a performance rate of zero (0) for any of the measures in the group. That will invalidate your submission.
As far as registries go, when a registry is accredited for reporting, it is accredited for individual measures or measure groups. In addition, it may not be accredited for all individual measures. The registry you spoke to may not be processing PQRS reporting for Measure Groups.

In my area, a physiatrist/pain specialist performs prolotherapy for chronic Achilles tendinosis. He bills the CPT code for percutaneous tenotomy and gets paid by the insurance company. Two patients have shown me their EOB and op reports. He gets good outcomes with one injection procedure!

Every state's laws, as they pertain to what you can and cannot do for a patient, are different. In New York, pharmacies can dispense diabetic shoes. My father, who is a registered pharmacist, took a short course and became certified.
This leads to the next issue. What knowledge does a person really have to have in order to measure for and dispense shoes? As one of my esteemed professors said to me recently as he was chewing me out, "Elliot, you trained for nine years in order to do what a high school kid can do working in a sneaker store." He is sadly correct.

The U.S. Department of Health and Human Services advises diabetics to have an annual preventive foot examination. So does the American Diabetes Association. So does the APMA. So does virtually every diabetes-related public health agency. Yet, according to PM News' recent survey, if a diabetic patient does not require being seen regularly for some ongoing problem, apparently less than 40% of responding podiatrists will recommend and appoint them for an annual comprehensive diabetic foot examination.
Aside from throwing away the revenue that would be generated by explaining to diabetics why they need this annual exam and appointing them, doesn't not appointing them at least border on not recommending and providing what is generally considered to be a medically necessary service?
