NY Podiatrist Uses Prolotherapy to Treat Achilles Tendon Disorder

Surgery left Fay Mihos with an Achilles tendon that was not completely attached to her bone. By her third pregnancy, she could hardly walk. When Dr. Robert Kornfeld first met Fay, her Achilles tendon was very, very swollen. "As a podiatrist, I’m going to focus on the foot. But I also focus on the mechanisms of the immune system that are involved in healing," said Kornfeld.

Kornfeld says anti-inflammatory drugs may reduce pain, but they also inhibit the body’s ability to heal. So he prefers natural therapies. says he supports the body’s healing process with diet, supplements, and therapies like MicroVas. It contracts leg muscles so they pump blood into the injured area. Another tool is called prolotherapy. It’s an injection of minerals to stimulate healing. Fay came in desperate for relief, but instead of just relief, she says she was healed. And a sonogram of her Achilles tendon proves it.

Dr. Ana Urukalo was in the KXAN studio on Saturday morning to talk about preparing your feet for running season. Urukalo, a podiatrist at Austin Diagnostic Clinic, says it's important to ensure you have a running shoe with a proper fit.

Make sure you properly measure your foot length, width, and arch. Allow extra room at the toe. Look for a secure fit in the mid-sole. Mike sure there is no slipping at the heel. And try different lacing techniques.

RE: Replacing Implant (Don Shrum, DPM)

The implant is unstable. The screws are broken. The patient is 74 years old. My suggestion is to try an articulated AZ Brace, as this way, the patient can still be active. Surgery involving replacement is a Pandora's box.

Aside from the broken screws at the tibi-fibular syndesmosis, the  large bone cyst in the fibula, and what almost looks like an avulsion fracture at the inferior aspect of the fibula, I suggest that you do nothing surgically to this 74 year old man. Treat him conservatively; otherwise you risk amputation and death in this age group.

RE: Fixation vs Nonfixation of Osteotomies

The concept of performing osseous surgery through a minimal incision technique makes sense in certain situations. The availability of cannulated screw systems lend themselves well to percutaneous fixation. I believe that procedures that we as podiatric/orthopedic surgeons perform routinely can be handled in such a fashion in capable hands. Capable hands means those who have had formal traditional surgical training and experience with a working knowledge of the anatomy and also the physiological aspects of bone healing. 

As I see it, the disparity and criticisms that have arisen over the years from our podiatric and orthopedic colleagues are aimed at those minimal incision surgeons who never had formal traditional surgical training. In years past, those untrained physicians saw their "entry" into podiatric surgical care through a minimal incision approach.

I suppose they felt they couldn't get into much trouble if they didn't open the foot. The notion that "bones will seek their own level" is antiquated thinking. Today, our patients deserve the highest level of care, including technology that can provide reproducible results. Minimal incision techniques, combined with sound surgical and biomechanical principles, will provide those results.  Open and closed techniques can live in the same house as long as their foundation is built not only with rigid internal fixation, but with common sense.

When a licensed health practitioner has an "impairment" - whether drug addiction, alcoholism, or mental health problem, it is a serious matter. Typically, the State Boards look at this condition as a reason to suspend a licensee.

When a doctor, nurse, etc., sees another doctor for professional help (i.e., psychiatrist), most states REQUIRE that the licensee be reported to the respective state "Health Practitioner Monitoring Program." (They have various names). Most states have a Health Practitioner Monitoring program. These are usually entities hired by the Boards to help...

I just wish I had that when I was preparing for the exam.

Dr. Bryan Markinson presented a number of dermatology cases, including a case of a patient with a melanoma on the plantar aspect of the foot. I strongly echo Dr. Levy's call for our profession to "focus seriously" on the relationship between podiatric medicine, cardiovascular disease, and other associated medical problems. Of course, among the top culprits are the metabolic disorders of obesity, metabolic syndrome, and diabetes. These disorders are of epidemic proportions, and we have an invaluable role to play. It seems we still need to figure out exactly what that role is.

Dr. Levy's post mentions the example of treating foot pain for exercise participation. While that is certainly important, I would also say that we need to encourage...
