PODIATRISTS IN THE NEWS
MA Podiatrist Discusses the Use of PRP Therapy

Platelet-rich plasma therapy (PRP) aims—and claims—to supercharge the body’s natural recovery processes, whether needed for a strained muscle or a scalp sparse from hair loss. With an injury, the platelet is what “creates a clot and sends out growth factors and a whole host of other chemicals to start this very beautiful and predictable chain of events—the inflammatory cycle itself,” explains Michelle Morse, DPM, a foot and ankle surgeon in Massachusetts. “So the idea [with PRP] is: Can we make that very potent, our body’s own natural way of healing?”

Dr. Michelle Morse

“Prior to me knowing much about PRP, I just viewed it as an expensive Hail Mary right before you go to surgery,” says Dr. Morse, a foot and ankle surgeon. But then she joined a new practice and began to integrate PRP into her offerings. “Now I am a full-fledged believer in our body’s own power to heal.” Dr. Morse says clinicians should be injecting with the guidance of an ultrasound “so you know exactly where you’re putting it.”

INTERNATIONAL PODIATRISTS IN THE NEWS
Aussie Podiatrist Discusses Women's Foot Health

For women, being pregnant can bring more change to the feet. “Not only do you get the effects of relaxin hormone, which enables you to give birth,” says Hylton Menz, Professor of Podiatry at La Trobe University’s School of Allied health, Human Services and Sport “that actually acts on all the different levels in your body, including your feet. There is a tendency for your feet to spread, so to become a bit wider and for the arch to get lower as pregnancy develops.”

Dr. Hylton Menz

At any age, podiatrists say, give your nails a break from nail polish from time to time: “It does actually mask things like, if you’ve got say, onychomycosis, which is like a fungal infection of your nails,” says Dr. Menz. Speaking of which, opt to keep your cuticles intact; they function as a protective seal, just as your nails protect your digits.

PRACTICE MANAGEMENT TIP OF THE DAY
7 Ways to Avoid a Malpractice Case Before it Starts

Here are seven practical, systems-minded ways practice leaders can lower malpractice risk without turning every visit into defensive medicine.

4) Upgrade documentation from “what happened” to “why this plan made sense”

In litigation, the chart is the story. If it does not show clinical reasoning, differential diagnosis, safety-net instructions and follow-up responsibility, the defense gets more difficult even when care was appropriate. Write for a skeptical reader: Include what you considered, what you ruled out and why, what you told the patient to watch for and what the plan was if symptoms changed. The Doctors Company’s risk guidance is blunt about documentation as a core defensibility issue; its clinician checklist on litigation prevention is a useful framework for practice audits.

FINANCIAL TIP OF THE DAY
6 Common Tax Mistakes to Avoid This Tax Season

To help you avoid a shock on filing day, here are six common ways you could be paying more income taxes than necessary.

6A.  You overlooked state-specific tax credits and deductions

While not every state has its own tax rules (and some states have no income tax at all), ignoring state-level credits and deductions is one of the easiest ways to overpay your year-end income tax bill.

Taking advantage of every tax break available to you could help save on state income taxes. To ensure you aren't leaving state money on the table, consider these tax resources and strategies:
  • Stay current on state tax changes. Review applicable rules and any upcoming local changes to better prepare for your state income return.
  • Look for how retirement taxes work in every state. If you're retired, how your state treats Social Security or pension income is vital — and could help you save on your next income tax bill.
  • Check out the best low-tax states for middle-class families. Is your state a high-tax state? For some, a move across state lines might be the most effective tax strategy of all.

SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM
Making the Most of Cadaver Lab

Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:

6.  Seek Feedback From Teaching Faculty

I love when we have sponsored labs with faculty present who can provide real-time feedback, tips, and pearls. Take advantage of that. Ask them what you could be doing better—whether it’s dropping your hand position, getting closer to the skin, or understanding where surgeons commonly struggle in the OR.

The cadaver lab is one of the best settings to receive feedback without pressure, so make the most of the expertise that’s available to you.

CODING & BILLING Q&As FROM CODINGHELPLINE.COM
Query:  Wound Care Code

I have been using this code for wound care: 97597 = Debridement (e.g., high pressure waterjet with/without suction, sharp selective debridement with scissors, scalpel and forceps), open wound (including topical application(s), wound assessment, use of whirlpool when performed and instruction(s) for ongoing care), first 20 sq cm or less.

Recently it gets rejected as: PAYER RESPONSE: NGS EDIT SUBMITTED THERAPY
PROCEDURE CODE IS MISSING REQUIRED MODIFIER GP, GO, OR GN.

GP - Services delivered under a Physical Therapy plan of care
GO - Services delivered under an Occupational Therapy plan
GN - Services delivered under a Speech Therapy plan

Has the code changed? GP seems closest but then do we need a physical therapy plan of care on file?

Response from Codinghelpline.comThe denial is due to a missing therapy modifier (GP, GO, or GN), not a code change. National Government Services (NGS) LCD L33631 has not been revised since...

Editor's note: Dr. Freedman's extended-length response can be read here.

RESPONSES/COMMENTS (SCOPE OF PRACTICE)
RE: Advanced Placement for DPMs to Obtain a Nurse Practitioner Degree
From: Daniel Chaskin, DPM

Instead of trying to compare a podiatrist's training to a physician's training, why not compare a podiatrist’s training to a nurse practitioner’s training? If a nurse practitioner can treat the entire lower extremity, then so should a podiatrist. At a minimum, nursing schools should allow advanced placement for podiatrists to obtainable an NP degree so the podiatrist can be licensed to treat the entire lower extremity.
