Nicotine Pouches Pose Circulatory Threat to Feet: CT Podiatrist
 
It’s becoming apparent that even tobacco-free nicotine pouches—the latest and admittedly least-researched nicotine product on the market—cause gum issues and GI upset, for starters. But there’s a lesser-known common denominator among all these addictive items: circulation issues. Any form of nicotine exposure can damage your blood vessels, leaving them irritated or constricted, Jeffrey M. DeLott, DPM, a podiatric surgeon at Hartford HealthCare’s Connecticut Orthopaedic Institute says. This is especially problematic for your feet, which are far from your heart and thus prone to poorer blood flow as is, he explains.

That dip in blood supply could up your risk for Raynaud’s syndrome, a condition in which the tiny vessels in your toes (and fingers) spasm and shrink up in response to any cold exposure, Dr. DeLott says. It can show up as skin discoloration, numbness, and stiffness at first, and then pain and soreness as the vessels open back up and blood rushes back. (If you already have Raynaud’s, research also suggests smoking can trigger or worsen your symptoms.) Over time, a consistent flow of nicotine could also spark more severe circulation issues like Buerger’s disease.

Podiatrists Discuss Integrating Research and AI
 
Ben Pearl, DPM hosted a webinar on integrating research and AI. Panelists included Warren Joseph, DPM, David Armstrong, DPM, Patrick Deheer, DPM, and Jeff Robbins, DPM. Dr. Joseph commented that a big push for research and publishing came historically from Harvey Lemont, DPM, who was the chairman of the department of medicine at PCPM. The process of developing research and going through an IRB was reviewed.

RE: The Passing of Joseph John Rusin, DPM, MD
 
Joseph John Rusin, DPM, MD of Maumee, OH, passed away at home on Sunday, February 16, 2025. Joseph was a graduate of the Illinois College of Podiatric Medicine, and the Chicago Medical School.

After completing his 5-year orthopedic residency at the University of Toledo Medical Center, he completed 3 fellowships. They included Foot and Ankle in Oakland, CA, Spine at Northwestern University in Chicago, IL, and the  AO fellowship in Trauma in St. Galen, Switzerland. Before his fellowships, he practiced in Mequon, WI, and was a director of a podiatric residency program in Milwaukee, WI.

Although I agree that the future of the profession is plenary licensure, I’m confused as to why there would be a push to obtain a degree as an osteopath with a podiatric sub-specialty, as opposed to obtaining a degree as an allopath with a podiatric sub-specialty? There are a certain finite number of medical theories out there, including allopathic, osteopathic, homeopathic, chiropractic, native American Indian pan-theistic naturopathy, witch doctors, Eastern Indian Ayurvedic medicine, and a few others.
 
Allopathic medical theory has as its basis the idea of pathology from disease state: bacteria, virus, spirochete, genetic dyscrasia, prion, etc. Unless I missed something critical in medical school, that’s the disease model we in podiatry follow as well. As such, podiatric medicine IS allopathic medicine. As we don’t follow the osteopathic theory of medicine, why would we obtain degrees as osteopaths, practice as allopaths, and so muddy the waters? I’m finding this very curious.

There has got to be some sort of educational program so non-surgical podiatrists can increase their scope of practice to treat the ankle as well as the rest of the body. Nursing schools should offer advanced placement for DPMs who wish to medically treat the ankle and above, as well as systemic diseases in certain states. This way, a DPM could qualify to get a nursing degree as a path to obtaining a license for treating the ankle as well as systemic diseases.
 
Once getting a nursing degree, they could then opt to get a nurse practitioner degree. Is it possible Touro might consider offering advanced placement for DPMs to obtain a nursing degree as a path to increasing scope?

Stephen Peslar is correct that many of the chiropodist graduates have left the profession, and the actual DPM podiatrist population is dwindling because of the idiotic 1993 legislation, stopping any DPM podiatrists coming into Ontario from practicing their full scope. This was a power grab by the medical establishment and an extremely poorly thought-out attempt by the government of the day to bring in chiropody practitioners to work in nursing homes and hospital clinics on a salary. The right to establish themselves as private practitioners and make positive progress in Ontario was through a charter of rights challenge brought by the chiropodists in the late 1980s which was successful. The governing college for the profession has been fighting tooth and nail for many years to establish podiatry as a properly recognized profession and unify us and increase our scope of practice to a full scope.
 
I personally had a wonderful and fulfilling career as a DPM podiatrist in Ontario until my retirement. There are many excellent, hardworking chiropodists and many excellent, well trained, skilled podiatrists in Ontario who have been pioneers in surgical procedures and...

Lynn Homisak’s article about what not to say hit home. Words Do Matter. I had a 78-year old female years ago who had lipomas on her ankles and she wanted them removed. I asked if they were painful and she said no, but she didn't like the way they looked. So I referred her out to an orthopedist. She returned to see me a few weeks later and she was mad at the other doctor because he said, "When we get to a certain age, we shouldn't care that much about how things look. I wanted to slug him."
 
I chuckled to myself and thought how I would say things differently but still get my point across. Now, I simply say, "You know, your birth certificate is getting kind of old." Patients usually laugh and they are not offended because I'm not calling them old. I'm calling their birth certificate old. Words do matter.
