NYCPM's General Anatomy Exams Go Digital
The general anatomy exam given to first-year students this January represented another leap forward into the digital domain by NYCPM. The exam has been given in two parts: a written half and the practical/dissections half (in which the answers were also written). The latter was divided into 25 stations that the students had to rotate through. That had to happen four times to accommodate all of the students, meaning that the sequestered students waiting to take the practical portion of the exam could wait for up to three hours after taking the written part of the exam.

To alleviate these problems, a Pre-Clinical Sciences team – Eileen Chusid, PhD, Dean of Pre-Clinical Sciences and Associate Professor; Joy Reidenberg, PhD, Professor and Director of Anatomy; Maria C. Moschella PhD, Assistant Professor; and Annette Kirchgessner, PhD, Associate Professor -- tackled revising the exam and making it digital. A mock practical was tested on the TAs, with success! The result? Students didn’t have to wait between parts of the exam, and scored better this year with the new format; a full day of grading was saved besides.

Loss of Hair on Feet Can Mean PAD: OH Podiatrist
Your feet can clue you in to your overall health. For example, if the hair on your toes suddenly disappears and the skin on your feet gets thinner or shiner, peripheral arterial disease (PAD)—poor circulation caused by a buildup of plaque in the leg arteries—may be to blame, says Amanda Meszaros, DPM, a podiatrist in Oberlin, OH. PAD is a major red flag for heart problems or a stroke since clogged arteries in the legs are usually associated with blockages elsewhere in the body.

Also look out for extremely dry skin and foot ulcers that don't heal; they may be triggered by undiagnosed diabetes since high blood glucose levels decrease sweat and oil production, points out Meszaros.

OTC Products and Home Remedies Can Cause Permanent Harm: Jamaica Podiatrist
According to podiatrist Eva Reid, if you have corns or bunions, she advises you to go straight to a podiatrist and avoid home remedies because it is likely that you would pick at the areas and make them worse. "Some use over-the-counter treatments that have a lot of salicylic acid, and that can discolor the feet. When this occurs, it is sometimes permanent, and many come after their skin gets discolored and expect us to fix it but we cannot," she explained.

Women who love to wear sandals should take special care of their feet. Leaving your heels exposed to the elements can give you cracked heels. You would be required to moisturize properly and frequently.

UK Podiatrists Demand National Awareness Campaign
Podiatrists in the UK are calling for a national campaign to help build awareness of the work they do for patients. Research revealed that 90 per cent of practitioners questioned across the private and NHS sectors wanted to see more done to promote their profession. Other key findings from the wide-ranging survey included:

90 percent say podiatry has a “poor” or “average” level of awareness among the public;
-
94 percent want more training and education;
-
81 percent would like to be able to offer a wider range of services;
-
43 percent want to employ more people;
-
68 percent would like business mentoring and support to grow their practices;
-
85 percent say work/life balance is their most important work consideration.

Better Survival of Implanted Cells Improves Healing of Bone Fractures
To treat a complicated, non–healing bone defect, surgeons often use an implant with living cells to promote bone repair, but the implanted cells have a small chance of surviving because they are not prepared for a lack of oxygen and nutrients at the fracture site. Scientists from KU Leuven have now improved survival of these bone cells by preconditioning them to withstand the harmful environment before implantation. Their findings were published in Cell Metabolism.
Doctoral student Steve Stegen tested in mice how he could better equip the bone cells for the crucial stage between implantation and ingrowth of the blood vessels. He managed to switch on a survival mode in bone cells by inactivating the oxygen sensor PHD2 before implantation. "As a result, bone cells activate a dual defence mechanism. First, bone cells increase storage of an emergency fuel in the form of glycogen, which is in fact a sugar reservoir. In addition, bone cells start using glutamine — an amino acid — to produce more antioxidants to neutralize the increased production of harmful oxygen radicals. These two adjustments allow bone cells to be self-supporting in terms of energy generation and to protect themselves against an increased level of oxygen radicals."

Query: To Bill or Not to Bill?
We had an unusual experience this morning. A new patient checked in, paid their co-pay, the medical assistants entered his medical information into the EHR, and he was roomed. At the very moment my medical assistant started to take the patient’s vital signs, the patient stated he suddenly could no longer lift his left arm. The MA came and got me immediately and I took one look at the patient and asked him to smile for me, revealing obvious left-sided facial droop.
We called 911 and while we waited for EMS to arrive, I explained in detail what was happening and that treatment would likely require t-PA within the next 2-3 hours at the hospital. I did my best to keep the patient and his wife calm and we put together a ...

Dr. Feinberg's posting requires a significant number of words to respond to well beyond the scope of a letter to PM News. My advice to all readers is to review the Part B Medicare Data (BMAD) for the last several years as well as David Freedman's Powerpoint, both of which are available on the APMA website. The trends and statistics bear out the fact that there are fewer shoes being paid for by Medicare for all supplier types, but podiatry remains as the supplier leading the pack with approximately 30% of the shoes and inserts being dispensed. Also, most claims submitted are approved. Speaking with many around the country, there are fewer audits and fewer erroneous findings, especially over the last few months.
Recent conversations with a Medicare Carrier Medical Director as well as the statistics available on the DME MAC C website bear this out. The reason is they have heard complaints (not just from podiatry, but from other supplier types). Much is being done to better educate their staffs, and fewer claims are going beyond the carrier or ZPIC level for appeal.

I believe the increased number of 3-year PMSR-trained podiatrists joining orthopedic groups is simple - the MONEY and OPPORTUNITY! As a recent graduate from a very competitive surgical residency, my fiancee and I opened our own practice - fortunately, I have been blessed with a job that paid well, however, had I not been blessed with this unique opportunity, I would have sought out a job that paid well, whether it be a podiatrist or orthopedist.
I pose this question to Dr. Baum and the many other solo/small group practitioners out there, including myself: how can we compete with offering starting salaries > $100,000 with bonus incentives + moving expenses + malpractice insurance + paid CME conferences + 401k benefits + potential buy-in opportunities for surgery centers and/or group + immediate referral source both inpatient/outpatient like many...

Dr. Deutsch's point is well taken - basic podiatry does not include only C&C, but if the lion's share of non-surgical podiatry is relegated to PAs, nurses, techs, etc., and if foot and ankle surgery done by podiatrists is not reimbursed equally with foot and ankle orthopedics, it will not only will be a disincentive to operate, but place the risk/benefit equation well against the podiatric surgeon. Then what is left for podiatry?
Parity, don't hold your breath. Foot and ankle orthopedists will fight against it. Why? Because it most likely will result in reduced fees for their procedures, not an increase in reimbursement for podiatric surgeons. So, there it is.
