PA Podiatrist Provides Advice for High-Heel Wearers

If you’re going to wear heels that are too high, do some leg stretches beforehand. “Using one of your husband’s ugly old ties, sit on the floor with straight legs and wrap the tie around the ball of the foot,” says Philadelphia podiatrist Dr.  Mallory Eisenman. “Grab on to each end and pull.” A resistance band will do the trick, too.

If your foot is irritated in a specific area because of the shoe itself, have a shoemaker spot the shoe (stretch it in just that area) rather than stretch the entire shoe. That way, it won’t become too big and cause your foot to slide. And there’s always inserts. “All over-the-counter products are about the same,” says Eisenman. “If it feels good, use it.”

NYCPM Faculty Member Selected by NIH to Serve as a Grant Reviewer

New York College of Podiatric Medicine Faculty Member Peter Barbosa, PhD, a professor in the College’s Division of Pre-Clinical Sciences, has been selected by the National Institutes of Health (NIH) to serve as a grant reviewer. NIH policy is intended to ensure that grant applications submitted to the NIH are evaluated on the basis of a process that is fair, equitable, timely, and free of bias. The NIH dual peer review system is mandated by statute in accordance with the Public Health Service Act and federal regulations governing "Scientific Peer Review of Research Grant Applications and Research and Development Contract Projects."

The first level of review is carried out by a Scientific Review Group (SRG) composed primarily of non-federal scientists who have expertise in relevant scientific disciplines and current research areas. The second level of review is performed by Institute and Center National Advisory Councils or Boards. Councils are composed of both scientific and lay members chosen for their expertise, interest, or activity in matters related to health and disease. Only applications that are favorably recommended by both the SRG and the Advisory Council may be recommended for funding.

INTERNATIONAL PODIATRY IN THE NEWS

Duchess of Cornwall Commends UK Podiatry Society on Its Centennial 

Camilla Parker Bowles joined celebrations in honour of the centenary of Britain's first podiatry organization. The Duchess of Cornwall was presented with a sculpture of Her Royal Highness by The Society of Chiropodists and Podiatrists to mark 100 years of the group's existence. She commended the organization's hard work for the past century and said that it does a fantastic job at "keeping us all on the road" and that "without chiropodists and podiatrists, we wouldn't have a leg to stand on."

The society, formed in 1912, came to being in 1945 after five English chiropody organizations joined together to create the British branch of the European body. Stuart Baird, chairman of the organization, said: "We are extremely proud of the work and advancements made by the podiatry profession over the last 100 years. We now have consultant podiatrists employed in the NHS in fields of podiatric medicine and podiatric surgery."

Diabetics Must Take Preventive Measures to Avoid Complications: NY Podiatrist

Complications due to diabetes are the No. 1 cause of lower-leg amputations and account for nearly 86,000 amputations per year. Doctors estimate almost 50 percent of these amputations could have been prevented if the person had taken better care of their feet. "I can't emphasize enough how important it is for a person with diabetes to pay rigorous attention to their feet. Foot infections are the most common issue for a person with diabetes and are more severe and take longer to heal than in a person without diabetes," says Dr. Alan Farber, a doctor of podiatric medicine. Farber added, "Proper foot care is simple and includes things like using an antifungal daily, not only to heal, but also to prevent fungal infections, and using a moisturizer daily to heal and prevent dry, cracked skin."

Query: Staple in Diabetic Shoe

I dispensed diabetic shoes from a leading shoe company. The patient called me the next day to inform me that he found two sharp nails inside the shoes at the hallux region. I drove to the patient's house and found a huge staple with the exposed 2 arms inside the shoes. My patient, who is neuropathic, luckily felt the sharp staple and did not sustain any injury, but he demanded compensation. The patient did not call the police or make an incident report. How should I handle a situation like this ethically to be fair to the patient and to the company? Bear in mind that if the patient calls Medicare, I think the company will be hurt. I have the shoes in my possession.

I think we have all seen this particular problem. The rule is: make sure you are happy with your result when the patient is on the table because any problems will not magically get better when your patient goes home. First, make sure that you have no tracking at the MTPJ, as determined by the quality of ROM. If you do, then check the soft tissue for something that you may have missed, or that needs releasing. I  assume that the sesmoids are structurally under the first met head, which is crucial. If there is a complete release and the problem still exists, it is most likely is in the adaptation of the articular cartilage of the 1st MTPJ.

While not a direct link to Crestor, please be aware that there is a well-documented correlation between hyperlipidemia/hypercholesterolemia and Achilles tendon pathology. This is an opportunity to treat the patient not only for the Achilles pathology, but as a screening tool - especially when xanthoma is suspected.

Some of these seemingly “rigid cavovarus” feet have an acquired element of this deformity that is potentially reducible over time with functional foot orthotic therapy. Although most podiatric physicians recognize forefoot supinatus (an acquired inverted position of the forefoot relative to the rearfoot), fewer seem to recognize that the plantarflexed, adducted, and everted non-weightbearing position of the forefoot in the cavus foot may be a partially acquired condition as well.

Root, Weed, and Orien described this condition in Normal and Abnormal Function of the Foot (pages 344-345) as follows: “As the rearfoot inverts with subtalar joint supination, the mechanical advantage of the peroneus longus muscle as a plantarflexor of the 1st ray is increased. I read Dr. Campitelli's comments on Podiatry Today's forum. I had to read it three times to believe that a podiatrist could be so clueless about orthotics and shoes. Dr. Kirby has given me the answer. Dr. Campitelli gives all the appearances of having been "bought" by Vibram 5-Fingers. He is using unscientific urban rumor to support his position. It is a clear violation of his oath and the code of ethics of the APMA and his state society. What will be so damaging is that he will be quoted by this tiny faction of the in-need-of-an-ego-boost subset of runners. On a positive note, it's good for my practice.

I have been using Comcast business class for the past three years now and have been very happy with them. Switching has cut my phone/Internet costs by half, and I now get cable TV as a bonus. The one time that our phones/Internet were down, Comcast had a crew that day working on the line that was damaged, and it was fixed within two hours. Other than that one time, I have had no problems here in Atlanta. I would make the switch.

With all due respect to Dr. Borreggine, if you’re losing patients because of an excessive amount of paperwork for a new visit, you might want to consider what you’re asking them to complete before they get to your office. My patients complete a simple, one-page registration form that captures basic information. The opposite side of this form is a simple privacy policy.
 
We don’t use any forms for medical histories, medications, allergies, etc. Every new patient is brought into the consultation room where I personally take their medical history. It takes less time for me to ask pointed, relevant questions that relate to their issues than it would take for them to tell me about the boil on their buttocks when they were age 16. Taking your own history helps establish the doctor-patient relationship. On average, it takes about 2 minutes for a patient to fill-out the registration form and sign the privacy statement, during which time, we photocopy their insurance card, and less than 5 minutes to sit in a non-medical room and talk about why they need my help.
 
Having the unfortunate experience of being a patient myself in more than my share of doctor’s offices, I can tell you that it’s frustrating for the patient as well as the doctor to feel like you’re buying their house when you come to a new doctor. First impressions last a long, long time.
