PROFESSIONAL MISCONDUCT
ME Podiatrist Operated on Wrong Foot; 7 Years Later, He Did it Again
 
In July 2024, a podiatrist in Bangor was scheduled to operate on a patient he’d been treating for months. The patient’s left foot was marked in preparation for a procedure to repair the Achilles and peroneal tendon surgery. In the operation room a short time later, however, a stocking and tourniquet were placed on the right leg. The doctor, Adam Darcy of Acadia Foot & Ankle, didn’t notice until it was too late. He operated on the wrong foot.
 
For Darcy, it was the second time in seven years, he made the same mistake. He was previously reprimanded by the Maine Board of Licensure of Podiatric Medicine and sued by a patient for amputating the wrong toe in 2017. Darcy’s surgical privileges were revoked by St. Joseph Hospital in December last year and Northern Light Health the next month. The state’s licensing board issued lifetime restrictions on his surgical license and placed him on three years’ probation, but he is still allowed to practice non-surgical medicine.

Use structured handoff tools (SBAR or a local equivalent), build “handoff required” moments into policy (end of day, cross-coverage, postprocedure) and audit a small sample monthly. AHRQ’s PSNet primer on handoffs is a practical starting point for training and workflow design.

5. Experiment With Different Incisions
 
If you’ve read that a procedure can be done through a different incision than you’re used to—try it. If you’ve been told your gastroc recession incision is too low, too high, or too posterior, test it out. Try a posterior incision and see where you struggle. Try a larger incision and see what new exposures it offers.
 
It is incredibly helpful to visualize the anatomy and understand how small changes in incision placement can impact both exposure and efficiency..

In your situation, I would be looking to hire an in-house biller/insurance verification specialist, with strong podiatry billing experience. But if you absolutely feel you have to go with an outside billing company, while I can’t make any recommendations for one, I can tell you that you really need to do your homework before you hire any company that may be suggested to you.
 
You seem to be indicating in your post that you may have a lot of denials right now? That should never be the case with any practice…podiatric, or otherwise. If demos/insurance info are being entered correctly and verified beforehand by your staff, and claims are being coded correctly by your billing company, based on how you submit them, you should have almost zero denials to deal with. Because everyone in your office, you, and your billing company are...

RESPONSES/COMMENTS (OBITUARIES)
 
It is with great sadness that I learned of the passing of Ben Zier. Ben was a true champion for the podiatric profession. He supported us at the local and state level. He had a great rapport with the students and taught us what we needed to know for our medicine rotations. Personally, Ben helped me through a minor health crisis while in school. I will never forget his kindness, thoughtfulness, and support.
 
My favorite Ben story was how he decided to go into general medicine. He was a medical student doing a surgical rotation. The surgeon asked Ben to cut the suture while closing. "Too long" the surgeon commented on the first suture. "Too short" on the second suture. Before cutting the third suture, Ben asked, "How would you like this one - too long or too short?" The surgeon asked Ben to leave the OR. That's how he decided to become an internist. I've told that story to every resident that I worked with. RIP Ben.
