TX Podiatrist Warns of Diabetes Crisis in San Antonio
“I do think diabetes is a crisis. I don’t want to be amputating. My colleagues don’t want to be amputating. These are things that can be avoided,” said Michael Sobolevsky, DPM, a podiatry specialist at the Texas Diabetes Institute.

Dr. Sobolevsky said people with diabetes in particular must see a doctor the moment they notice a wound on their body that won’t heal. “If you come in sooner, amputation does not have to be the option. It is preventable with good care,” said Dr. Sobolevsky.

| APMA IN THE NEWS | |
APMA Defends Mississippi Scope Modernization
For the past several years, the Center for Professional Advocacy (CPA) at APMA has collaborated closely with the Mississippi Podiatric Medical Association (MsPMA) to modernize the state’s scope of practice to permit podiatric physicians to treat the ankle. The CPA has provided financial grants to support this effort and has reviewed and advised on MsPMA's proposed legislation. APMA also worked with the state to coordinate media relations, such as the comments APMA Executive Director and CEO James R. Christina, DPM, offered in a recent Mississippi newspaper article.

Most recently, the CPA submitted a letter supporting MsPMA directly to the Mississippi State Legislature. The letter, drafted in response to an AMA letter expressing opposition to updating the state scope of practice, focused largely on the education, training, and qualifications of today's podiatrists, which more than support modernizing the state scope of practice in Mississippi. It also highlighted how outdated Mississippi law is and the direct impact on patient access to care. Finally, it addressed the misinformation in the AMA letter, designed to mislead legislators and the public.

According to Monique Mitchell, DPM, MPH, "Heel pain is most often caused by plantar fasciitis, a condition that is sometimes also called heel spur syndrome when a spur is present. Heel pain may also be due to other causes, such as a stress fracture, tendonitis, arthritis, nerve irritation, or rarely, even a cyst.

I read the article quoting William Scherer, DPM and the need for diagnostic testing where he stated a 60% diagnostic confirmation rate for clinical nail fungus.
In my opinion, if it looks like fungus, it is fungus and really with a 60% confirmation rate, I believe that testing is a waste of time and money. I probably test 1% of my patients when I cannot get them better. Did testing help me with my treatment? Not really. If you feel the patient has fungus and they want the toes treated, I am not going to stop treatment if the diagnosis comes back negative as 98% of the time, I can clear up a fungus appearing nail despite what the diagnosis might be.
