The American College of Foot and Ankle Surgeons (ACFAS), a national surgical association of over 7,000 foot and ankle surgeons announced its 2015-2016 Board of Directors. The new Board was installed at the annual business meeting conducted at the ACFAS 73rd Annual Scientific Conference in Phoenix.

OH Podiatrist Receives ACFAS Distinguished Service Award
 
Ankle & Foot Care Centers’ managing partner Dr. Lawrence A. DiDomenico received the American College of Foot and Ankle Surgeons’ Distinguished Service Award at the organization’s Annual Scientific Conference in Phoenix, AZ. The organization’s board of directors presents this annual award to recognize members who volunteer their time, expertise, and service to help advance its mission. DiDomenico is the 12th recipient of the Distinguished Service Award.

DiDomenico is the Fellowship Director at Ankle & Foot Care Centers and Kent State University’s College of Podiatric Medicine. This fellowship program is recognized by the American College of Foot and Ankle Surgeons. He became a member of the American College of Foot and Ankle Surgeons in 1990 and served on its board of directors from 2003-2006. He has been active as a volunteer in the organization’s educational meetings, seminars, cadaver labs, and as a member of various committees.

 FAPA Holds 2015 “Seminar in the Sun”
 
The 20th Annual FAPA “Seminar in the Sun” took place from February 16-19, 2015 at the beautiful all inclusive Dreams Cancun Resort in Puerto Morelos, Mexico. FAPA’s scientific chair Gary Stones, DPM and educational director Al Kanegis, DPM were pleased to feature educational lectures from both past APMA president Dr. Matthew Garoufalis and ACFAP president Dr. Louis J. DeCaro.

Dr. Garoufalis lectured on wound care and diabetic ulcer care. Dr. DeCaro lectured on podopediatrics and the science of foot typing and biomechanics. Each year, the seminar draws DPMs from all around the country to not only immerse themselves in important podiatric topics, but also the beautiful sunshine of a tropical location.

HIPAA UPDATE
Deadline Looms for Notifying HHS of Breaches in 2014
 
Attention HIPAA-covered entities: Has your practice had a breach of protected health information during the past year? March 1 is the deadline for reporting breaches that occurred in 2014 to the HHS Office for Civil Rights, which enforces the HIPAA breach notification rule. Breaches affecting 500 or more individuals require notification to affected individuals and HHS within 60 days of discovery.
 
Covered entities with breaches affecting less than 500 individuals during 2014 must submit notice to HHS within 60 days of the end of the calendar year--March 1, 2015. Covered entities also can report these smaller breaches to HHS during the year if they chose, but a complete report of breaches on an annual basis is required, with a separate notice filled out for each breach.

Medical License Reciprocity is Paving the Way to Telemedicine
 
For physicians in rural states, the "windshield time" -- the hours spent driving from one rural community to another -- is often non-productive and frequently unnecessary. Telemedicine is one alternative, but regulations preventing physicians from treating patients across state lines are a barrier. Now, lawmakers in several states are advancing a proposal, called the Interstate Medical Licensure Compact, to fast-track the licensure process enabling qualified doctors to practice in more than one state. The compact would expand opportunities for eligible providers to practice telemedicine across state lines. It could also increase access through face-to-face visits in rural areas near state borders, allowing doctors to have clinic hours and make house calls on both sides of the line. 
 
To date, the compact has been introduced in over a dozen state legislatures. Chambers in three -- South Dakota, Utah, and Wyoming -- have passed proposals related to the compact. South Dakota's Senate passed the Compact with abundant support, and the House chambers in Wyoming and Utah voted unanimously in favor of the proposal, according to a press release from the Federation of the State Medical Boards (FSMB). The concept is fairly straight forward. Any qualified physician interested in obtaining licensure in multiple states could do so through an alternative pathway, said Lisa Robin, MLA, chief advocacy officer for the FSMB.

Lori Logan, senior vice president at the Provider Solutions clearinghouse unit of payer software vendor TriZetto, believes more insurers are ready for end-to-end testing than they have indicated, but they did not want to bother with it in 2014. Like providers, they used the compliance delay to finish other priority projects. By late 2014, TriZetto had done testing with 177 payers, mostly acknowledgement testing, and Logan expected both acknowledgement and end-to-end testing to increase in the first quarter of 2015.

Prolotherapy has been around since the 1930s and became more popular in the 1950s with Drs. Hemwall and Hackett writing a book about it. Prolotherapy is the original "regenerative medicine." I have used this technique in my practice for many years and it works well for many patients. It is a cash procedure and may take multiple injections. Essentially, this a non-surgical method of ligament, tendon repair. The injectables are called proliferants. The most common injections use dextrose. Fish oil is not injected, rather a fatty acid derived from cod liver known as sodium morrhuate. These injections cause an inflammatory response and they tighten and rebuild ligamentous attachments. The injection technique is very important for results. Injecting randomly does not work. The solution is not injected into a wide area of tissue. The injection is at the bony attachment.
 
Almost all specialties gather each year at the American Association of Orthopedic Medicine conference. Sadly, podiatry is poorly represented. There is a ton of knowledge at these meetings. These techniques are time-tested and supported by research. A well known MD at the University of Kansas, Dr. Dean Reeves, has done Level 1 and 2 studies, which  are on his site. These techniques are taught in many universities and medical programs.

I have been using prolotherapy for over 15 years with great success. But it behooves every practitioner to understand that you can NEVER evaluate any therapy on its own as the only statistical barometer of efficacy. Realize that the indication for prolotherapy is to stimulate connective tissue healing in CHRONIC conditions. 
 
This is the MOST important information because it tells you that you already have a patient whose immune system is struggling to heal. So before you make a diagnosis and fill your syringe...
